Acute kidney injury in critically ill patients with traumatic brain injury: A single-center retrospective cohort study.

BACKGROUND: Traumatic brain injury (TBI) is the second most common presentation of trauma victims. Among the various non-neurological complications after TBI, acute kidney injury (AKI) is not uncommon. AIM: To establish the incidence, risk factors, and predictors of AKI in TBI victims. The secondary aim was to study the impact of AKI development on the outcomes of patients with TBI. METHODS: This was a single-center retrospective cohort study of TBI victims with a Glasgow coma scale (GCS) ≤ 11 in an apex trauma center in a metropolitan city. RESULTS: The incidence of AKI after TBI was 11%.

Predictive factors for shunt dependency in patients with spontaneous intraventricular hemorrhage.

Intraventricular hemorrhage (IVH) occurs in approximately 30-50% patients with spontaneous intracerebral hemorrhage (ICH), with a high 30-day mortality rate. Excess accumulation of parenchymal or ventricular blood associated with the development of acute hydrocephalus leads to poor outcomes. The prediction of shunt dependency is important to identify patients susceptible to requiring permanent shunt placement and benefit from the diversion. This retrospective analytical study aimed to establish a predictive model of shunt dependency in patients with IVH. This study included 179 patients with p

Case 324.

A 69-year-old right-handed man with a history of substance use disorder, hypertension, and diabetes presented to the emergency department in an unresponsive state. Upon examination, apart from tachycardia (heart rate, 108 beats per minute), vital signs were within normal ranges (blood pressure, 134/102 mm Hg; temperature, 97.9°F [36.6°C]; respiratory rate, 16 breaths per minute; oxygen saturation, 96%). He had a Glasgow coma scale score of 8. Otherwise, the physical examination revealed no abnormalities. His prior psychiatric and surgical histories were unremarkable. There was no history of re

Risk Factors for Infection Following Operative Treatment of Traumatic Upper Extremity Amputation Injury.

Background: Identification of the risk factors for surgical site infection (SSI) can be a straightforward and cost-effective measure to reduce or prevent the occurrence of SSI. However, there are no studies that revealed risk factors for SSI for traumatic upper extremity amputation. The aim of this study is to investigate the risk factors that promote SSI after surgery for traumatic upper extremity amputation using a large nationwide database. Methods: We used data from the Japan Trauma Data Bank. Diagnoses were defined using the Abbreviated Injury Scale code. We applied multivariate logistic

Spinal level and cord involvement in the prediction of sepsis development after vertebral fracture repair for traumatic spinal injury.

OBJECTIVE: Despite understanding the associated adverse outcomes, identifying hospitalized patients at risk for sepsis is challenging. The authors aimed to characterize the epidemiology and clinical risk of sepsis in patients who underwent vertebral fracture repair for traumatic spinal injury (TSI). METHODS: The authors conducted a retrospective cohort analysis of adults undergoing vertebral fracture repair during initial hospitalization after TSI who were registered in the National Trauma Data Bank from 2011 to 2014. RESULTS: Of the 29,050 eligible patients undergoing vertebral fracture rep

Morbidity and mortality associated with hypernatremia in patients with severe traumatic brain injury.

OBJECTIVE Hypernatremia is independently associated with increased mortality in critically ill patients. Few studies have evaluated the impact of hypernatremia on early mortality in patients with severe traumatic brain injury (TBI) treated in a neurocritical care unit. METHODS A retrospective review of patients with severe TBI (admission Glasgow Coma Scale score ≤ 8) treated in a single neurocritical care unit between 1986 and 2012 was performed. Patients with at least 3 serum sodium values were selected for the study. Patients with diabetes insipidus and those with hypernatremia on admission

Polyuria in relation to dysnatraemias in neurocritical care.

INTRODUCTION: Polyuria has the potential to cause severe water and sodium imbalance. We studied the epidemiology of polyuria in association with dysnatraemias and whether polyuria is an independent risk factor for higher mortality and poorer outcome in neurocritical care. METHODS: We performed an analysis of a 3-year prospective database containing 902 neurocritical care patients. Polyuria was defined as diuresis above 4000 ml/day, hyponatraemia as a serum sodium (SeNa) < 135 mmol/l and hypernatraemia as SeNa > 150 mmol/l. RESULTS: We identified polyuria in 236 (26.2%) patients (639 days). P

Reversible hypophosphatemia during moderate hypothermia therapy for brain-injured patients.

OBJECTIVE: Because plasma potassium, which may similarly change as plasma phosphate (P), decreases during moderate hypothermia, plasma P, a requisite electrolyte for the cell function, may alter during therapeutic moderate hypothermia for brain-injured patients. In 22 such patients who underwent moderate hypothermia or were treated with normothermia, plasma concentrations of P and other chemicals were examined. DESIGN: A prospective study. SETTING: The intensive care unit of a medical university hospital. PATIENTS AND INTERVENTIONS: In 15 consecutive patients with brain injury who underwent

Workflow Patterns and Clinical Consequences of External Ventricular Drain Timing: A 6-Year Analysis of 26 020 Cases From the American College of Surgeons National Trauma Data Bank.

BACKGROUND AND OBJECTIVES: External ventricular drains (EVDs) are critical for monitoring and managing intracranial pressure in acute traumatic brain injury patients. However, there is a lack of consensus on optimal EVD placement timing. Previous studies suggest benefits in early placement but are limited by smaller cohorts and lack of adjustment for illness severity. We sought to characterize EVD timing patterns and examine clinical characteristics and outcomes associated with early placement. METHODS: We performed a retrospective study of patients in the National Trauma Data Bank (2018-2023

Risk factors associated with acute kidney injury in patients with traumatic brain injury: A systematic review and meta-analysis.

PURPOSE: This systematic review and meta-analysis aimed to identify and quantify the risk factors associated with acute kidney injury (AKI) in patients with traumatic brain injury (TBI). METHODS: PubMed, Embase, and Web of Science were systematically searched for articles published up to October 2024. Observational studies published in English that reported on risk factors for AKI in TBI patients were included. Data on AKI incidence and risk factors were extracted. A meta-analysis was conducted using a random-effects model when heterogeneity I2 > 50 % and a fixed-effects model when I2 < 50 %.

Little benefit from mild hypothermia therapy for severely head injured patients with low intracranial pressure.

OBJECT: This study was performed to determine whether mild hypothermia therapy is essential for the treatment of severely head injured patients in whom intracranial pressure (ICP) can be maintained below 20 mm Hg by using conventional therapies. METHODS: Sixteen consecutive severely head injured patients fulfilled the following criteria: the patient's ICP was maintained below 20 mm Hg by using fluid restriction, hyperventilation, and high-dose barbiturate therapy; and the patient had a Glasgow Coma Scale score of 8 or less on admission. After conventional therapies had been applied, the patie

Sequelae of abdominal radiation and their medical management.

As various chemotherapeutic agents are added to treatment routines, the possibility of adverse effects is appreciably increased, particularly in those organs for which the agents have specific toxicity. Symptoms most commonly associated with radiation sickness, such as malaise, anorexia, nausea, vomiting, diarrhea, dysphagia, dermatitis, and depleted hemopoietic elements, are usually seen late in the course of radiation therapy or shortly thereafter. Consequently, they are managed by the physician in charge of radiation or the patient's referring physician. The general physician may be concern

A Comparative Study on the Trends of Blood Glucose, Serum Triglycerides, and CRP-Based Levels in Correlation with GCS among Patients with Traumatic Head Injury.

BACKGROUND: Traumatic head injury, which has a high mortality rate, can present as mild contusions, hemorrhages (subdural, extradural, intraparenchymal), diffuse axonal injuries, or direct penetrating injuries. Glasgow Coma Scale (GCS) is used to assess the severity of head injury. Stress-induced hyperglycemia associated with traumatic brain injury has high mortality compared with hyperglycemia in diabetic patients. Stress-induced hyperglycemia not only occurs due to head injury but also serves as a predictor of its outcome. C-reactive protein (CRP) levels are also associated with severity of

Aspergillus meningitis following spinal anaesthesia for caesarean section in Colombo, Sri Lanka.

We report six cases of Aspergillus meningitis after spinal anaesthesia for caesarean section administered in June and July 2005. Three patients died before a fungal infection was confirmed at the first post-mortem examination in August. Thereafter anti-fungal therapy was successful in saving the lives of the other three patients. Some syringes and spinal needles supplied to the hospitals concerned were found to be contaminated with Aspergillus fumigatus. Investigators found that medical supply storage facilities were substandard following the influx of donations after the tsunami of December 2
