Peristalsis prevents ureteral dilation.

PURPOSE: The etiology of ureteral dilation in primary nonrefluxing, nonobstructing megaureters is still not well understood. Impaired ureteral peristalsis has been theorized as one of the contributing factors. However, ureteral peristalsis and its "normal" function is not well defined. In this study, using mathematical modeling techniques, we aim to better understand how ureteral peristalsis works. This is the first model to consider clinically observed, back-and-forth, cyclic wall longitudinal motion during peristalsis. We hypothesize that dysfunctional ureteral peristalsis, caused by insuffi

Pregnancy in women with dilated cardiomyopathy.

Only a few studies describe the pathophysiology and outcomes of dilated cardiomyopathy (DCM) in pregnancy, which the authors aim to review here. DCM causes enlargement of the ventricles and reduced systolic function. Fluid overload and raised cardiac output in pregnancy may contribute to cardiac complications that lead to cardiac remodeling and heart failure, a common cause of maternal mortality. The risk of cardiac complications is higher in women with New York Heart Association class III and IV. Fetal and neonatal complications are common with coexisting obstetric risk factors. Hence, prepre

Diagnostic biomarkers of dilated cardiomyopathy.

BACKGROUND: Dilated cardiomyopathy (DCM) is a condition involving dilation of cardiac chambers, which results in contraction impairment. Besides invasive and non-invasive diagnostic procedures, cardiac biomarkers are of great importance in both diagnosis and prognosis of the disease. These biomarkers are categorized into three groups based on their site; cardiomyocyte biomarkers, microenvironmental biomarkers and macroenvironmental biomarkers. AIMS: In this review, an overview of characteristics, epidemiology, etiology and clinical manifestations of DCM is provided. In addition, the most impo

Echocardiographic Advances in Dilated Cardiomyopathy.

Although the overall survival of patients with dilated cardiomyopathy (DCM) has improved significantly in the last decades, a non-negligible proportion of DCM patients still shows an unfavorable prognosis. DCM patients not only need imaging techniques that are effective in diagnosis, but also suitable for long-term follow-up with frequent re-evaluations. The exponential growth of echocardiography's technology and performance in recent years has resulted in improved diagnostic accuracy, stratification, management and follow-up of patients with DCM. This review summarizes some new developments i

Rhinovirus-associated dilated cardiomyopathy.

Rhinovirus is the main cause of the common cold. There is little to no published literature of rhinoviral associated myocarditis. We report a rare case of rhinovirus infection in a patient with myocarditis leading to dilated cardiomyopathy. Infection is an established cause of myocarditis. Prodromal "flu-like" symptoms in a young patient with unexplained heart failure should raise concern for viral myocarditis. Diagnosis is often made by clinical presentation and not by endomyocardial biopsy due to invasiveness. Polymerase chain reaction is a rapid test that supports the diagnosis and may eluc

Precision Medicine and Dilated Cardiomyopathy.

As the most common cardiomyopathy, dilated cardiomyopathy (DCM) is currently defined as a heart muscle disease which is characterized by left ventricular (LV) or biventricular dilation and systolic dysfunction at the exclusion of either pressure or volume overload or severe coronary artery disease sufficient to explain the dysfunction. For established DCM patients, treatment is directed at the major clinical manifestations of heart failure and arrhythmias, including pharmacological treatment, device therapies, and heart transplantation. But this traditional strategy is incompletely effective a

Indications for Eustachian tube dilation.

PURPOSE OF REVIEW: Eustachian tube dilation is a controversial surgical procedure because the intended problem for which it is meant to address, obstructive Eustachian tube dysfunction, is a clinical diagnosis without a reliable diagnostic tool to test.In the past 10 years, balloon dilation Eustachian tuboplasty (BDET) has been the most commonly performed Eustachian tube dilation procedure. The present review seeks to identify the patients who may benefit from this procedure and thus propose the indications. RECENT FINDINGS: Two randomized controlled trials for BDET published in the past 2 ye

Symptoms in Dilating Venous Disease.

Lower extremity venous diseases or insufficiency include clinically deteriorating conditions with morphological and functional alterations of the venous system, including venous hypertension, vascular wall structural abnormality, and venous valvar incompetency in association with an inflammatory process. In fact, the same pathophysiological processes are the main underlying mechanisms of other venous insufficiencies in different vascular territories such as Peripheral Varicose Vein (PVV), varicocele, Pelvic Varicosities or Congestion Syndrome (PCS) and Hemorrhoidal Disease (HD). Regarding the

Aortic dilation in pediatric patients.

UNLABELLED: Aortic dilation at the level of the aortic root can be caused by a variety of congenital or acquired conditions that lead to weakening of the aortic wall. In this retrospective study, we sought to determine the frequency of different associated diagnoses from children with aortic dilation seen at a single institution. A total of 377 children (68 % male) met study inclusion criteria. Patients were classified based on the suspected or confirmed associated diagnosis in one of the following categories: congenital heart disease (241/377, 64 %), chromosomal (34/377, 9 %), Marfan syndrome

Immunoadsorption therapy in dilated cardiomyopathy.

Dilated cardiomyopathy is a common myocardial disease characterized by ventricular chamber enlargement and systolic dysfunction that result in heart failure. In addition to genetic predisposition, viral infection and myocardial inflammation play a causal role in the disease process of dilated cardiomyopathy. Experimental and clinical studies suggest that activation of the humoral immune system, with production of circulating cardiac autoantibodies, plays an important functional role in the development and progression of cardiac dysfunction in patients with dilated cardiomyopathy. Small open-co

[Congenital dilations of the biliary tract].

Congenital dilations of the biliary tract are a group of rare conditions, commonly associated with the presence of an abnormality at the junction of the common bile duct and pancreatic duct, which may lead to the reflux of pancreatic juice within the biliary tree. The main clinical symptoms are, abdominal pain, cholangitis and acute pancreatitis, and the most serious complication is malignant degeneration. The treatment of choice is cholecystectomy and complete excision of the bile duct from its bifurcation to its intra-pancreatic segment.

Dilated cardiomyopathy in epidermolysis bullosa.

Dilated cardiomyopathy (DC) is a rare but potentially fatal complication of epidermolysis bullosa. No clear cause for it has been identified, but iron overload, low carnitine, low selenium, concomitant viral illness, chronic anemia, and medications have been proposed as possible contributors to the development of DC in reported cases. Early detection allows for medical treatment that delays clinical progression and prolongs survival.

Winer's dilated pore of the eyelid.

A 71-year-old woman had a whitish mass of the outer right lower eyelid with an eccentric pore plugged with keratin. Histopathologic evaluation disclosed an epidermoid cyst that opened widely to the surface and was filled with flaky keratin. The wall displayed a proliferation of rete ridge-type structures that projected in the surrounding dermis. The lesion was diagnosed as a dilated pore of Winer and has not recurred after simple excision.
