Secondary Aortoesophageal Fistula Treated by Covered Esophageal Stent and Muscle Wrapping.

Secondary aortoesophageal fistula is rare but is associated with serious complications and high mortality. We devised a treatment strategy involving a covered esophageal stent, instead of esophageal resection, and placing the latissimus dorsi muscle around the infected aorta without removing the aorta and the stent graft. A 72-year-old man with a history of total arch replacement with a frozen elephant trunk for a thoracic dissecting aneurysm developed aortoesophageal fistula and underwent surgical treatment using our strategy. He recovered well, with no evidence of reinfection 6 months after

Florida elephant trunk hybrid aortic repair (FETHAR): a novel frozen elephant trunk technique with complete debranching sparing endograft modification.

Treatment of aortic arch pathology is rapidly evolving. Current techniques utilize a combination of open and endovascular approaches. Existing techniques such as B-SAFER require endograft modification for complete arch debranching which may predispose the graft to failure or endoleak. We developed the FETHAR, a novel method for hybrid aortic repair and demonstrate its use in a patient who presented with an acute dissection with contained rupture. Following repair, we demonstrate excellent seal without endoleak on the post-operative CT scan. Ultimately, this repair allows for complete arch debr

Chronic abdominal aortic dissection, endovascular treatment using a new Stent-graft for in situ Fenestration.

BACKGROUND: Although endovascular treatment of the thoracic aorta (TEVAR) has become an elective procedure for treatment of complicated type B aortic dissection, its role in treating post dissection thoraco-abdominal aortic aneurysm (TAAA), is still limited. This is a case of aortic vascular disease, which reports the use of a new endovascular device. CASE PRESENTATION: We present the case of a 62 year old male patient with a history of hypertension, active smoker, who presented penetrating descending thoracic aortic ulcer in the setting of a chronic abdominal aortic dissection. The patient w

En bloc video-assisted thoracoscopic vertebrectomy for a spinal tumour.

Complex thoracic vertebral tumours remain a surgical challenge in terms of the surgical approach to ensure a complete en bloc vertebrectomy with healthy margins, along with optimal control of the thoracic structures next to the spine. A combined three-port left thoracoscopic posterior approach, with the patient placed in a prone position with selective double-lumen intubation, can be performed in patients with spinal tumours involving soft tissues, for direct access to the thoracic structures, even with T10-T11 vertebral tumours next to the diaphragm. The video thoracoscopic technique with an

4D Flow Cardiovascular Magnetic Resonance Uncovers the Hemodynamic Clue in Aortic Dissection.

BACKGROUND: Persistent or recurrent aortic coarctation after surgical repair can create hemodynamic stress that predisposes to aneurysm formation and dissection. CASE SUMMARY: A 58-year-old man with prior extra-anatomic bypass repair of coarctation presented with acute interscapular pain and an upper-to-lower limb pressure gradient of >40 mm Hg. Computed tomography showed a thrombosed graft, a descending thoracic aortic aneurysm, and a Stanford type B dissection. Multimodal imaging, including 4D flow cardiovascular magnetic resonance, showed a high-velocity jet traversing the coarctation and

Pulmonary hypertension as a rare cause of postoperative chylothorax.

Chylothorax in adult occurs most commonly in the wake of cardiac and thoracic procedures. Injuries to the common thoracic duct in the thorax or its branches in the mediastinum, injuries to the thymus tissues, dissection of the superior vena cava or ascending aorta, dissection of the aortic arch, disruption of the accessory lymphatics in the left or right thorax, and increased pressure in the systemic vein exceeding that of the thoracic duct (usually in the superior vena cava thrombosis, Glenn Shunt, and hemi-Fontan) have been proposed as the possible causes of chylothorax after surgery for con

Marfan's syndrome and other aortopathies in pregnancy.

Aortopathies, or disease affecting the aorta, are associated with a significant mortality risk for the mother and foetus during pregnancy because of an increased rate of aortic dissection. The hereditary aortopathies; Marfan's syndrome, bicuspid aortic valve, Loeys-Dietz syndrome, Ehlers-Danlos (type IV) syndrome, Turner's syndrome and nonsyndromic familial thoracic aortic aneurysm and dissection are all associated with an increased risk of aortic dissection particularly during the third trimester and early postpartum period. Maternal outcome in pregnancy depends on the underlying disorder and

Characterization of vasomotor responses in different vascular territories of C57BL/6J mice.

The mouse is a quickly reproducing, inexpensive animal and often used for transgenic approaches. Due to its small size, only the aorta is frequently taken to assess vascular function. However, atherosclerosis is a generalized disease and becomes symptomatic when the perfusion of specific organs is impaired. We have therefore compared the thoracic and abdominal aorta with carotid, femoral, mesenteric, renal and coronary arteries to see whether aortic vasomotion can indeed serve as a surrogate for other, organ-specific vascular territories. Arterial segments of male C57BL/6J mice were dissected

Endovascular approaches to complex thoracic aortic disease.

Diseases of the thoracic aorta remain among the most lethal and difficult to treat conditions. In 2005, the US Food and Drug Administration approved the first endoprosthesis for the treatment of aneurysms of the descending thoracic aorta; at present, there are 3 thoracic devices approved by the US Food and Drug Administration. Although approved only for the treatment of descending aneurysms, thoracic endografting has other potential off-label applications, including acute and chronic aortic dissection and traumatic aortic transection. Endovascular repair of thoracic aortic pathology is emergin

Identification of T-cell exhaustion-related biomarkers in aortic dissection via integrated multi-omics analysis and mendelian randomization.

BACKGROUND: Aortic dissection (AD) is a life-threatening cardiovascular emergency characterized by high acute mortality. While immune dysregulation is known to drive AD pathogenesis, the specific involvement of T-cell exhaustion-related genes (TEXRGs) remains largely elusive. METHODS: Differentially expressed TEXRGs (DETEXRGs) between AD and control samples were identified using transcriptomic datasets. Functional enrichment and Mendelian randomization (MR) analyses were performed to investigate potential causal associations with AD risk. Key diagnostic biomarkers were selected through integr

Current options and recommendations for the use of thoracic endovascular aortic repair in acute and chronic thoracic aortic disease: an expert consensus document of the European Society for Cardiology (ESC) Working Group of Cardiovascular Surgery, the ESC Working Group on Aorta and Peripheral Vascular Diseases, the European Association of Percutaneous Cardiovascular Interventions (EAPCI) of the ESC and the European Association for Cardio-Thoracic Surgery (EACTS).

Since its clinical implementation in the late nineties, thoracic endovascular aortic repair (TEVAR) has become the standard treatment of several acute and chronic diseases of the thoracic aorta. While TEVAR has been embraced by many, this disruptive technology has also stimulated the continuing evolution of open surgery, which became even more important as late TEVAR failures do need open surgical correction justifying the need to unite both treatment options under one umbrella. This fact shows the importance of-in analogy to the heart team-aortic centre formation and centralization of care, w

Ascending-to-descending aortic bypass via posterior pericardium for complex coarctation of aorta.

UNLABELLED: We introduce our surgical experience in treating recurrent aortic coarctation or coarctation associated with intracardiac abnormalities, which is difficult to manage. Four patients underwent the operation with hypothermic cardiopulmonary bypass. During the surgery, the posterior pericardium was opened and the thoracic aorta was dissected and sidebiting clamped, and an end-to-side anastomosis of the artificial graft to the descending aorta was performed. The graft was passed in between the inferior vena cava and the inferior right pulmonary vein and posterior to the pericardium. Aft

Surgical strategies in the management of chronic dissection of the thoracoabdominal aorta.

Most operations for dissection of the thoracoabdominal aorta take place in the chronic phase of the disease, because the acutely dissected distal aorta is almost always initially treated non-surgically with aggressive pharmacological anti-impulse therapy. Identifying patients who are no longer responding to medical treatment is the first step in preventing further disease progression and rupture. Symptomatic aneurysms should be promptly repaired. Asymptomatic patients are followed until significant aortic dilation occurs and reaches a threshold of intervention: current guidelines endorse repai

The retroperitoneal, left flank approach to the supraceliac aorta for difficult and repeat aortic reconstructions.

Between 1986 and 1990, 11 patients with relative or absolute contraindications to standard infrarenal reconstructions underwent supraceliac aortofemoral bypass. The operation was performed through a left-flank incision extended into the eleventh intercostal space with retroperitoneal and extrapleural dissection. Indications included multiple failed infrarenal reconstructions in four patients, previous removal of infected aortofemoral bypass graft with failure of extra-anatomic bypass in five patients, prior para-aortic lymph node dissection and radiotherapy in one patient, and aortic aneurysma
