Potentially Clinically Relevant Drug-Drug Interactions in Oncology Patients Receiving Chronic Opioid Therapy: Prevalence and Associated Factors.

BACKGROUND: Patients with malignant diseases receiving chronic opioid therapy are particularly susceptible to drug-drug interactions because of extensive polypharmacy, multimodal anticancer treatment, supportive care, and comorbidities. This study aimed to determine the prevalence of potentially clinically relevant drug-drug interactions and to identify factors associated with their occurrence. METHODS: This exploratory observational cross-sectional pilot study included 49 adult oncology patients receiving chronic opioid therapy. Complete medication regimens were screened using the Medscape D…

Pharmacokinetic drug-drug interactions with flucloxacillin and other isoxazolyl penicillins: A systematic literature review and practical guide.

Recent years have seen a notable increase in the number of publications concerning pharmacokinetic drug-drug interactions involving the isoxazolyl penicillins cloxacillin, dicloxacillin, flucloxacillin and oxacillin. Given that the findings predominantly rely on clinical observations, the interaction mechanisms and their clinical relevance remain insufficiently elucidated, thereby posing challenges for clinicians in managing these interactions. The aim of this literature review was to provide an overview of all reported pharmacokinetic drug-drug interactions involving isoxazolyl penicillins an…

Physiologically based pharmacokinetic framework for predicting CYP3A4-mediated drug-drug interactions of quetiapine with ketoconazole and carbamazepine.

Quetiapine is primarily metabolized by CYP3A4, and its coadministration with potent CYP3A4 inhibitors (eg, ketoconazole) or inducers (eg, carbamazepine) may substantially alter systemic and central nervous system exposure. Although clinical evidence supports these interactions, quantitative predictions of plasma and brain interstitial concentrations remain limited. This study aimed to develop an integrated physiologically based pharmacokinetic (PBPK) model to quantitatively predict drug-drug interactions (DDIs) among quetiapine, ketoconazole, and carbamazepine, and to propose model-based dose-…

Application of Physiologically-Based Pharmacokinetic Modeling to Support Drug Labeling: Prediction of CYP3A4-Mediated Pirtobrutinib-Drug Interactions.

Pirtobrutinib is a reversible Bruton tyrosine kinase (BTK) inhibitor. In vitro, pirtobrutinib is metabolized by cytochrome P450 (CYP) 3A4 and uridine 5'-diphosphoglucuronosyl transferases (UGTs) and causes reversible and time-dependent inhibition and induction of CYP3A4. Coadministration of itraconazole, a strong CYP3A4 inhibitor, with pirtobrutinib in healthy human subjects, resulted in a pirtobrutinib area under the plasma concentration-time curve (AUC) ratio of 1.49, while rifampin, a strong CYP3A4 inducer, decreased pirtobrutinib AUC by 71%. Oral administration of pirtobrutinib 200 mg once

Are We Considering All the Potential Drug-Drug Interactions in Women's Reproductive Health? A Predictive Model Approach.

Background: Drug-drug interactions (DDIs) may occur when two or more drugs are taken together, leading to undesired side effects or potential synergistic effects. Most clinical effects of drug combinations have not been assessed in clinical trials. Therefore, predicting DDIs can provide better patient management, avoid drug combinations that can negatively affect patient care, and exploit potential synergistic combinations to improve current therapies in women's healthcare. Methods: A DDI prediction model was built to describe relevant drug combinations affecting reproductive treatments. Appro…

Discovering Severe Adverse Reactions From Pharmacokinetic Drug-Drug Interactions Through Literature Analysis and Electronic Health Record Verification.

While drug-drug interactions (DDIs) and their pharmacokinetic (PK) mechanisms are well-studied prior to drug approval, severe adverse drug reactions (SADRs) caused by DDIs often remain underrecognized due to limitations in pre-marketing clinical trials. To address this gap, our study utilized a literature database, applied natural language processing (NLP) techniques, and conducted multi-source electronic health record (EHR) validation to uncover underrecognized DDI-SADR signals that warrant further investigation. PubMed abstracts related to DDIs from January 1962 to December 2023 were retriev…

Systematic review of drug-drug interactions between rifamycins and anticoagulant and antiplatelet agents and considerations for management.

Rifamycins (rifampin, rifabutin, and rifapentine) play an essential role in the treatment of mycobacterial and some nonmycobacterial infections. They also induce the activity of various drug transporting and metabolizing enzymes, which can impact the concentrations and efficacy of substrates. Many anticoagulant and antiplatelet (AC/AP) agents are substrates of these enzymes and have narrow therapeutic indices, leading to risks of thrombosis or bleeding when coadministered with rifamycins. The objective of this systematic review was to evaluate the effects on AC/AP pharmacokinetics, laboratory

Role of organic anion transporter 3 in the renal excretion of biapenem and potential drug-drug interactions.

Biapenem is a carbapenem antibiotic. It is excreted predominantly through the kidney as unchanged forms. However, the molecular mechanism of renal excretion of biapenem and potential drug-drug interactions (DDIs) were still unknown. In the present study, the role of organic anion transporters (OAT) 1/3 and organic cation transporters (OCT) 2 in the renal excretion of biapenem, and the potential DDIs between biapenem and six clinical commonly prescribed antibiotics and antiviral drugs that acted as substrates or inhibitors of OAT3 were evaluated in vitro. Further, the effect of probenecid on th…
