Ebstein's Anomaly with Various Unexplained Arrhythmias in a 57-Year Old Man.

There are very few congenital cardiac disease which may remain asymptomatic up to adulthood, Ebstein's anomaly is one of them. With rapid advancement in the field of echocardiography many new cases identified in day-to-day practice. Timely surgical intervention in these cases may distinct survival advantage. We present a case of Ebstein's anomaly of adult age group which is seen with typical manifestation and seen in less than 5% cases.

[Altered Z-disks of myofibrils in the cardiomyocytes from patients with Ebstein's anomaly].

UNLABELLED: The aim of the study was to examine the peculiarities of the changes in Z-bands of myofibrils in the cardiomyocytes from patients with Ebstein's anomaly. MATERIAL AND METHODS: Electron microscopy assay of intraoperative biopsies of the right heart chambers in 41 patients aged from 9 months to 57 years was performed. RESULTS: Some patients exhibited Z-disk alterations of two types in individual cardiomyocytes, namely: local symmetrical bead-like expansions of Z-disks or longitudinal deposits of Z-material of different lengths along the myofibrils. Z-disk alterations were more comm

Electronic fetal heart rate monitoring in five fetuses with Ebstein's anomaly.

AIM: It is not clear whether fetuses with Ebstein's anomaly are likely to show abnormalities on electronic fetal heart rate (FHR) monitoring during labor. METHODS: We performed retrospective analysis of FHR monitoring results obtained between 2.5 and 0.5 h prior to delivery in all of five fetuses prenatally diagnosed as having Ebstein's anomaly and born at our institution during the period between March 2005 and July 2011. All five mothers attempted to give birth vaginally. The results of FHR monitoring in 100 consecutive women with otherwise healthy singletons who attempted vaginal delivery

Cone reconstruction and ventricular septal defect closure for neonatal Ebstein's anomaly.

Neonatal presentation of Ebstein's anomaly is associated with significant morbidity and mortality. We illustrate the technical aspects of the cone procedure and the closure of a ventricular septal defect, to achieve biventricular repair in a neonate. This includes the assessment of the leaftlet apparatus, detachment, division of chordae, annular plication, leaflet rotation and reinsertion in the neoannulus.

An unusual first presentation of Ebstein's anomaly in a 72-year-old patient.

We report the unusual case of a 72-year-old woman with a primary presentation of right heart failure in the setting of recently diagnosed Ebstein's anomaly with concomitant atrial fibrillation (AF). The patient had New York Heart Association (NYHA) class III dyspnea for 12 months, with refractory right heart failure prior to undergoing surgical management in the form of a tricuspid annuloplasty ring and plication of the atrialized ventricle. The patient had an uneventful postoperative recovery and enjoyed an improvement in her exercise tolerance (NYHA class I) with minimal echocardiographic ev

Interventional closure of atrial septal defects in adult patients with Ebstein's anomaly.

Ebstein's anomaly is frequently associated with interatrial communications. In patients with severe tricuspid regurgitation standard treatment is the surgical repair or replacement of the tricuspid valve and patch closure of the atrial septal defect. We sought to evaluate the feasibility and short-term outcome of interventional device closure of interatrial communications in Ebstein patients with mild to moderate tricuspid regurgitation and various degrees of clinical symptoms. In this case series of 9 patients the device closure could be performed safely and 8 of 9 patients improved in their

Cone reconstruction in Ebstein's anomaly repair: early and long-term results.

BACKGROUND: The main Ebstein anomaly (EA) repairs are based on the monocusp reconstruction of the tricuspid valve and are limited by the frequent need for replacement or the high recurrence of valve regurgitation. OBJECTIVE: To evaluate the feasibility and effects of anatomical repair of Ebstein's anomaly using the cone reconstruction technique on patients' clinical evaluation, tricuspid valve function and right ventricular morphology. METHODS: We compared the clinical, echocardiographic and radiological data of 52 consecutive patients, with a mean age of 18.5 ± 13.8 years, submitted to the …

[Catheter ablation of accessory pathway tachycardias in three patients with Ebstein's anomaly].

Ebstein's anomaly (EA) is a malformation of the tricuspid valve characterized by a downward displacement of the septal and often the posterior tricuspid valve leaflets to the atrialized right ventricle. Among all congenital anomalies, EA is the most related malformation with accessory pathways. In 5%-25% of patients with EA, accessory atrioventricular pathways may present on the surface electrocardiogram. Radiofrequency catheter ablation is the first-line treatment of EA patients having supraventricular tachyarrhythmias. The presence of a dysplastic tricuspid annulus and electrically distingui

Ebstein's anomaly of the tricuspid valve with rheumatic mitral stenosis and aortic incompetence.

The case is reported of Ebstein's anomaly of the tricuspid valve with rheumatic mitral stenosis and aortic incompetence. In this extremely rare clinical entity, right ventricular dysfunction and respiratory dysfunction occur due to severe mitral stenosis and tricuspid regurgitation. The present case was managed with aortic and mitral valve replacement and Danielson's repair of the tricuspid valve. The general management issues of the condition are also discussed in detail.

[Anesthetic management for surgery of the ankle joint fracture in a patient with Ebstein's anomaly].

The authors describe the successful anesthetic management of a patient with Ebstein's anomaly and Wolff-Parkinson-White syndrome who underwent repair for foot joint fracture. Anesthesia was provided by combined spinal-epidural block to minimize hemodynamic changes which cause an increase in right-to-left shunt and hypoxemia. We estimated hemodynamic status using radial arterial pressure, central venous pressure and ECG from the start of anesthesia. During anesthesia, his vital signs were unchanged to any significant degree. The recovery was rapid and the postoperative course was uneventful.

Pericardial patch augmentation of both anterior and septal leaflets in Ebstein's anomaly.

In Ebstein's anomaly, the aim of surgical intervention is the restoration of tricuspid valve competence. In some circumstances, however, paucity of the leaflet tissue precludes successful repair and negatively influences the long-term durability of the valvuloplasty procedure. We report a modified technique that provides satisfactory results for right atrioventricular valve function.

Sildenafil use in a symptomatic neonate with severe Ebstein's anomaly of the tricuspid valve.

Ebstein's anomaly (EA) is associated with poor outcome in symptomatic neonates. Management typically includes prostaglandins or surgical creation of a Blalock-Taussig shunt. Right ventricular afterload reduction may help by improving the forward flow across the pulmonary valve. We report the successful use of oral sildenafil, a pulmonary vasodilator, in a neonate with severe EA.

A novel approach incorporating sildenafil in the management of symptomatic neonates with Ebstein's anomaly.

The neonatal presentation of Ebstein's anomaly is a distinct cyanotic lesion for which little can be done other than to maintain ductal patency or perform palliative surgery if improvement does not occur. We report two cases in which sildenafil was introduced to assist previously failed attempts at weaning of prostaglandin.

Ebstein's anomaly presenting as Wolff-Parkinson white syndrome in a postpartum patient.

Ebstein's anomaly is a common congenital abnormality in the Wolff-Parkinson white syndrome (WPW). The term WPW is applied to patients with both preexcitation on ECG and paroxysmal tachycardias. In this case review, we describe a female with a history of intermittent palpitations who presented in the postpartum period with WPW. Subsequent testing revealed an underlying Ebstein's anomaly. In the United States, heart disease is responsible for 10% of maternal deaths. Although pregnancy is well known to exacerbate symptoms in patients with WPW, postpartum exacerbation has not been clearly describe

Ebstein's anomaly, atrial paralysis and atrio-ventricular block: an uncommon association.

We report here the case of a 22-year-old female patient with an incomplete Ebstein's anomaly, complete heart block and atrial standstill. Atrial paralysis associated with Ebstein's anomaly is the most important feature, since there is a report of familial Ebstein's anomaly associated with atrial standstill but isolated cases have not been described. The patient presented with atypical chest pain and a symptomatic bradycardia of 37 beats per minute. A VVIR pacemaker was implanted. She has subsequently been symptom free.
