Susceptibility to neuroleptic-induced parkinsonism--age and increased substantia nigra echogenicity as putative risk factors.

OBJECTIVE: Patients treated by neuroleptics often develop neuroleptic-induced parkinsonism (NIP) to a varying extent. The reasons for this are discussed controversially in the literature. Previous transcranial sonography (TCS) findings of the substantia nigra (SN) in patients with idiopathic Parkinson's disease suggest a correlation of echogenicity with nigrostriatal dysfunction. METHOD: One hundred psychiatric patients receiving neuroleptics were included. They underwent clinical examination for NIP (Simpson and Angus-scale) and, independently, TCS of the SN. History of smoking habits and me

[Etiologies of cerebral palsy and classical treatment possibilities].

Cerebral palsy is a non-progressive disorder of the developing brain with different etiologies in the pre-, peri- or postnatal period. The most important of these diseases is cystic periventricular leukomalacia (PVL), followed by intra- and periventricular hemorrhage, hypoxic-ischemic encephalopathy, vascular disorders, infections or brain malformations. The underlying cause is always a damage of the first motor neuron. Prevalence of cerebral palsy in Europe is 2-3 per 1000 live births with a broad spectrum in different birth weight groups. Our own data concerning only pre-term infants in the

Early diagnosis of brain death in patients treated with central nervous system depressant drugs.

BACKGROUND: Among the main causes for the relatively small number of organ donors, the delay in the diagnosis of brain death plays a major role. Administration of drugs causing central nervous system (CNS) depression prevents diagnosis of brain death by clinical and electroencephalographic criteria until serum clearance of the drug has occurred. Confirming brain death by demonstrating persistent intracranial circulatory arrest might decrease the length of the diagnostic process. We have carried out a prospective study to investigate whether 99 mTc-hexamethyl propylenamino oxime (99mTc-HMPAO) b

Spontaneous motility in preterm, small-for-gestational age infants. II. Qualitative aspects.

In order to document in detail the developmental course of qualitative aspects of early spontaneous motility in intrauterine growth-retarded infants, sequential videotape recordings were made in 19 preterm infants with a birth weight below the 5th percentile. The quality of general movements (GMs) was studied longitudinally during the preterm and postterm period until approximately 20 weeks corrected age, using Prechtl's method of quality assessment. An abnormal quality of GMs was present in 15 out of 19 infants. Compared to a low-risk group, consisting of appropriate-for-gestational age prete

Micro-stereometry: a frameless computerized navigating system for open microsurgery.

We are introducing a new type of ultrasonic-based three dimensional (3D) ranging system for the localization of small hand-held surgical tools to be used during operation in open and stereotactic brain surgery. The new device has been extensively tested in the laboratory and showed a linear localization accuracy of +/- .4 mm. The calibration of the system is automatically done by an active calibration frame as well as by pointing to fiducial markers. The position of the tool is sent to a visualization unit and overlaid in realtime on preoperatively scanned computed tomography (CT) and magnetic

Relationships between dopamine infusions and intracranial hemodynamics in patients with raised intracranial pressure.

Dopamine, 1-10 micrograms/kg body weight/min was infused in 6 patients suffering from cerebrovascular diseases with elevated intracranial pressure and a critical cerebral perfusion pressure. Dopamine decreased intracranial pressure in 3 and increased it moderately in the other 3 patients. In all patients, the dopamine-induced rise of mean arterial pressure led to an increase of cerebral perfusion pressure. Transcranial Doppler ultrasonographic recordings of the middle cerebral artery in patients whose intracranial pressure declined revealed a decrease of the pathologically elevated cerebrovasc

The predictive value of head ultrasound in the ECMO candidate.

Cranial ultrasound (US) examination is the screening technique of choice for assessing preexisting neurological damage in potential neonatal extracorporeal membrane oxygenation (ECMO) candidates. Currently, US evidence of intracranial hemorrhage greater than grade I in severity is a contraindication to ECMO at this ECMO center. In the current study, radiological findings were reviewed in 129 consecutive neonatal ECMO cases in an attempt to identify which pre-ECMO US findings were associated with the development of subsequent intracranial complications while on ECMO. Pre-ECMO head US, post-ECMO

Doppler colour flow imaging after carotid endarterectomy.

Sixty-six patients (72 carotid arteries) were examined after carotid endarterectomy (CEA) using Doppler colour flow imaging (DCFI). Examinations were performed 4-18 days (mean: 7 days) after surgery (32 patients, 34 arteries) or between 2 and 100 months (mean: 39 months) after CEA (34 patients, 38 arteries). Minor vessel wall abnormalities were found in 36% in the internal carotid artery (ICA) and in 55% in the common carotid artery (CCA) or bifurcation. One patient had a minor residual ICA stenosis after surgery; two low-grade stenoses and three ICA-occlusions were diagnosed at follow-up. Alt

[Transcranial Doppler sonography in cerebrovascular diseases].

A review of our 1 1/2 years' experience with transcranial Doppler sonography (TCD) is given. In total, 619 TCD examinations were performed. Considering physiological variations of blood flow velocities in the intracranial arteries, TCD findings in stenoses and occlusions of the extracranial and intracranial brain-supplying arteries as well as in intracranial vasospasms were evaluated. In patients with leptomeningeal anastomoses demonstrated by TCD in the region of the peripheral middle cerebral artery, the regional cerebral blood flow was measured by 99mTc-HMPAO-SPECT. The value and limitation

Sonographic and Doppler Sonographic Diagnosis of Posthemorrhagic Hydrocephalus.

Severe intracranial hemorrhages occur especially in very immature premature infants born with a gestational age under 28 weeks of gestation and a birth weight below 1000 g. Severe hemorrhages are often complicated by a post-hemorrhagic hydrocephalus (PHH). PHH can be caused by a blockage of the cerebro-spinal fluid pathways or by obliterative arachnoiditis of the posterior cranial fossa. Cerebral sonography can differentiate between both entities. In cases of obstruction of the cerebro-spinal fluid circulation the parts of the ventricular system infront of the obstruction are dilated. Color co

Reference ranges for fetal brain fissure development on 3-dimensional sonography in the multiplanar mode.

OBJECTIVES: To determine reference ranges for measurements of fetal cerebral fissures by 3-dimensional (3D) sonography in the multiplanar mode and to evaluate the reliability and concordance of these measurements. METHODS: A cross-sectional study was conducted on 393 women with normal pregnancies at 22 weeks to 33 weeks 6 days. The distances between the internal bone plate of the fetal calvaria and the sylvian, parieto-occipital, hippocampal, and calcarine fissures were assessed. To obtain the distances for the first 3 fissures, a 3D sweep was made in the axial plane, at the level of the late

Oral intake of γ-aminobutyric acid affects mood and activities of central nervous system during stressed condition induced by mental tasks.

γ-Aminobutyric acid (GABA) is a kind of amino acid contained in green tea leaves and other foods. Several reports have shown that GABA might affect brain protein synthesis, improve many brain functions such as memory and study capability, lower the blood pressure of spontaneously hypertensive rats, and may also have a relaxation effect in humans. However, the evidence for its mood-improving function is still not sufficient. In this study, we investigated how the oral intake of GABA influences human adults psychologically and physiologically under a condition of mental stress. Sixty-three adult

Predictors of hyperperfusion syndrome before and immediately after carotid artery stenting in single-photon emission computed tomography and transcranial color-coded real-time sonography studies.

BACKGROUND: Hyperperfusion syndrome (HPS) is a critical complication after carotid artery stenting (CAS) and carotid endarterectomy (CEA). OBJECTIVE: To identify predictors of HPS before and immediately after CAS. METHODS: We analyzed patients who underwent elective CAS from 2005 to 2008, and underwent single-photon emission computed tomography (SPECT) and transcranial color-coded real-time sonography before and immediately after CAS. HPS was defined as post-CAS deteriorating neurological conditions with headache not secondary to cerebral ischemia. We assessed the measures of blood flow betw

Management of foetal hydrops secondary to Kell isoimmunisation via foetal blood transfusion: a Doppler-guided approach.

We describe a male neonate with foetal hydrops due to foetal anaemia caused by Kell isoimmunisation. The severity of anaemia was monitored by Doppler ultrasonography of the middle cerebral artery peak systolic velocity, and this was used to time the foetal blood transfusions. The 33-year-old Indian mother received a total of five foetal blood transfusions from 21 weeks to 31 weeks of gestation, resulting in resolution of the anaemia and hydrops.

[Clinical manifestation and outcomes of concussion of the brain: comparison of the results in patients admitted to hospital and in those who have refused admission].

The clinical manifestations and outcomes of concussion of the brain (CB) were comparatively analyzed in two groups of victims with acute brain injury (BI): those who had admitted to hospital and in those who had refused admission. A total of 355 individuals aged 16-35 years without concomitant somatic and neurological diseases were examined. The follow-up lasted 1 year. Clinical and neurological studies, computed tomography, and MR-imaging were used. In the acute period, the regression of neurological symptomatology and the better status of health were noted in the victims of both groups almos

Cerebral blood flow velocity changes after bovine natural surfactant instillation.

OBJECTIVE: To determine the effects of bovine natural surfactant (beractant) instillation on cerebral hemodynamics in preterm infants with respiratory distress syndrome (RDS). STUDY DESIGN: Preterm infants who required surfactant for RDS were enrolled. Cerebral blood flow velocity (CBFV) waveforms from the pericallosal artery were analyzed by pulsed Doppler ultrasonography with the anterior fontanel serving as an acoustic window. CBFV was measured before and at 5, 10, 20, and 30 minutes after the first dose of a bolus instillation of surfactant in four aliquots. Simultaneously with CBFV measu

Neurosonographic findings in full-term infants born to maternal cocaine abusers: visualization of subependymal and periventricular cysts.

Neuroanatomical abnormalities have been linked to maternal cocaine abuse since Chasnoff et al's report of cerebral infarction in a single full-term offspring of a cocaine-abusing mother. Neurosonography was performed on 18 full-term offspring of cocaine-abusing mothers and 19 age-matched controls to look for possible abnormalities. There were no cases of infarction or intraparenchymal or intraventricular hemorrhage. There were, however, 6 patients with abnormalities among the cocaine-abusing group: 5 patients with unilateral or bilateral germinal matrix cysts and 3 patients with subtle cystic

Dissection of the cervical internal carotid artery. The role of Doppler/Duplex studies and conservative management.

Cervical internal carotid dissection is not rare. Doppler ultrasound screening of young patients presenting with stroke, identified 10 patients with reduced common and internal carotid blood flow without any evidence of atheroma. Eight, on angiography proved to have a dissection of the cervical internal carotid artery. All were managed conservatively. Seven received anticoagulant therapy, stopping any further neurological symptoms.

Neonatal cranial ultrasound abnormalities: association with developmental delay at age one in low birth weight infants.

Relationships between abnormalities on neonatal serial cranial ultrasound and cognitive development at age one year were examined in 153 low birth weight (LBW) infants. Infants with complex injury (persistent parenchymal echogenicity, lucency, or persistent ventricular enlargement) scored significantly lower on the Bayley Mental Development Index than noninjured infants. Nine of 11 infants with complex injury had severe developmental delay in contrast to 3/110 of the noninjured. Adjusting for birth weight, gestational age, head circumference and social class, infants with complex injury were 3
