Diabetes insipidus in craniopharyngioma: postoperative management of water and electrolyte disorders.

Pre-operative central diabetes insipidus has been reported in 8-35% of patients affected with craniopharyngioma, and in 70-90% after surgery. The management of postoperative polyuria and polydipsia can be challenging and fluid balance needs to be closely monitored. The classical triphasic pattern of endogenous vasopressin secretion--an initial phase of symptomatic diabetes insipidus occurring 24 hours after surgery; a second phase of inappropriate vasopressin secretion potentially causing hyponatraemia; and a third phase with a return to diabetes insipidus occurring up to 2 weeks later--is oft

Acid-base and electrolyte abnormalities in dogs with gastrointestinal foreign bodies.

Gastrointestinal foreign bodies occur commonly in dogs. The objective of the study was to describe the acid-base and electrolyte abnormalities identified in dogs with gastrointestinal foreign bodies and determine if these abnormalities are related to the site or type of foreign body present. Medical records of 138 dogs were reviewed, and information on signalment, initial venous electrolyte and acid-base values, surgical findings, relevant historical information, imaging modalities used, cost of hospital visit, intra- or postoperative complications, and survival was obtained. The site of the f

A mathematical model of electrolyte and fluid transport across corneal endothelium.

To predict the behavior of a transporting epithelium by intuitive means can be complex and frustrating. As the number of parameters to be considered increases beyond a few, the task can be termed impossible. The alternative is to model epithelial behavior by mathematical means. For that to be feasible, it has been presumed that a large amount of experimental information is required, so as to be able to use known values for the majority of kinetic parameters. However, in the present case, we are modeling corneal endothelial behavior beginning with experimental values for only five of eleven par

Electrolytic deposition of calcium etidronate drug coating on titanium substrate.

Wear debris-induced osteolysis is the major cause of aseptic loosening and failure of hip implants. One of the promising therapeutic interventions to improve the longevity of hip implants is to administrate bisphosphonate drug to inhibit osteoclastic bone resorption. This study aimed at developing new techniques of directly combining bisphosphonate with implants to achieve local delivery and controlled release of the drug. Instead of using soluble sodium salt, we proposed to apply sparingly soluble calcium salt of bisphosphonate as a potential long-term antiosteolysis coating on hip implants.

Electrolyte and plasma enzyme analyses during large-volume liposuction.

Substantial fluid shifts occur during liposuction as wetting solution is infiltrated subcutaneously and fat is evacuated, causing potential electrolyte imbalances. In the porcine model for large-volume liposuction, plasma aspartate aminotransferase and alanine transaminase levels were elevated following liposuction. These results raised concerns for possible mechanical injury and/or lidocaine-induced hepatocellular toxicity in a clinical setting. The first objective of this human model study was to explore the effect of the liposuction procedure on electrolyte balance. The second objective was

Keratins modulate colonocyte electrolyte transport via protein mistargeting.

The function of intestinal keratins is unknown, although keratin 8 (K8)-null mice develop colitis, hyperplasia, diarrhea, and mistarget jejunal apical markers. We quantified the diarrhea in K8-null stool and examined its physiologic basis. Isolated crypt-units from K8-null and wild-type mice have similar viability. K8-null distal colon has normal tight junction permeability and paracellular transport but shows decreased short circuit current and net Na absorption associated with net Cl secretion, blunted intracellular Cl/HCO3-dependent pH regulation, hyperproliferation and enlarged goblet cell

Food intake and electrolyte status of ultramarathoners competing in extreme heat.

OBJECTIVE: To relate changes in laboratory indices to dietary intake during extremely prolonged running and to determine if dietary intake influences the ability of runners to finish an 160 km trail race. METHODS: We monitored intake and serum chemistries of 26 runners competing in an 160 km foot race in temperatures which peaked at 38 degrees C. Blood was drawn pre-, mid- and post-race. Dietary intake and incidence of gastrointestinal distress or changes in mental status were determined by interview with runners approximately every 13 km. Twenty-three runners completed at least 88 kms and, o

Effect of electrolyte and pH changes on the sinus node pacemaking in humans.

The importance of plasma electrolytes and pH levels in determining heart rate is not yet well grounded. Hemodynamic and biochemical data were collected during 8 purely diffusive hemodialysis, which allowed changes in extracellular fluid to be achieved without eliciting notable changes in the autonomic control of heart rate. A significant heart rate increase was obtained after potassium decrease and calcium and pH increase, with no significant variations in indices of autonomic activity. Model-based computer simulations were then used to separate the effects of each ionic species and pH on sinu…

[Villous adenoma of the rectum with electrolyte imbalance. McKittrick-Wheelock syndrome].

We present a case of villous adenoma of the rectum, associated with severe fluid imbalance in a 69 year-old, previously healthy woman. Severe dehydration with hyponatraemia, hypokalaemia, acidosis/alkalosis and watery diarrhoea is typical. Clinical presentation may include fatigue, confusion, convulsions or coma. Intensive rehydration is the primary intervention, and causal treatment is always surgery. Normally full recovery is achieved.

Effects of frusemide on electrolyte and acid-base balance during exercise.

This study was undertaken to evaluate the effects of frusemide on the concentration of plasma electrolytes and the relationship between changes in electrolyte concentration and the simultaneous changes in acid-base balance in arterial and venous blood during intense exercise. Five exercise-conditioned Thoroughbred horses were exercised on a high-speed treadmill at a slope of 10% at speeds known to exceed VO2max. Horses participated in 3 randomised exercise trials in which they received either placebo (control), low-dose frusemide (0.5 mg/kg bwt), or high-dose frusemide (1.0 mg/kg) 4 h prior to…

Alcohol abuse: potential role in electrolyte disturbances and kidney diseases.

The present review summarizes the current knowledge on the multiple effects of alcohol overconsumption on the kidney function as well as on water, electrolyte and acid-base homeostasis. In contrast to the well known transitory diuretic effects, the overall long-term effect of chronic alcohol overconsumption is water and salt retention with expansion of extracellular volume. Furthermore, depletion of magnesium, phosphate and calcium is also frequently found in alcohol-dependent patients. These electrolyte disturbances may be associated with the alcohol-induced hypoparathyroidism and parathyroid

Approaches to diuretic therapy and electrolyte imbalance in congestive heart failure.

This review has summarized the current role of diuretic therapy in heart failure, emphasizing those aspects most relevant to this patient population. Recommended diuretic usage is as follows: Asymptomatic left ventricular dysfunction--establish moderate sodium intake, Mild sodium retention--thiazide-type diuretic or low-dose loop diuretic; continue moderate sodium intake; combine with an ACE inhibitor, Moderate sodium retention--loop diuretic, adjusting for renal function, if necessary; continue moderate sodium intake; combine with an ACE inhibitor, Severe sodium retention--large-dose loop diu

The effect of colitis on large-intestinal electrolyte transport in early childhood.

The large intestine plays an important role in the salvage of electrolytes and water, although little is known of its function in early childhood in health or in disease. Using an in vitro voltage clamp technique, we have measured bidirectional sodium (Na) and chloride (Cl) fluxes (J), electrical potential difference (PD), short circuit current (Isc), and tissue resistance (R) in 10 pairs of healthy human infant left colon and four pairs of inflamed colon. In healthy tissue there was net Na and Cl absorption with JNa net approximating Isc. In contrast, inflamed colon exhibited significant redu…

Segmental variability of glucocorticoid induced electrolyte transport in rat colon.

Recent studies suggest that the ability of glucocorticoids to reduce diarrhoea in active colitis may reflect their direct effects on distal colonic electrogenic Na+ transport and water absorption, as well as their anti-inflammatory action. To determine whether glucocorticoids induce similar changes in proximal colon, specific Na+ and K+ channel blockers (amiloride and tetraethylammonium chloride (TEA) respectively) were used to evaluate the cation transport properties of rat proximal and distal colon in vitro after three days treatment with the glucocorticoid agonist dexamethasone (600 microgr

[Water-electrolyte and acid-base disorders. VII. Metabolic alkalosis].

Metabolic alkalosis is defined as a primary increase in plasma bicarbonate concentration. As a consequence of this increase, systemic alkalemia and secondary hypercapnia develop. In most instances metabolic alkalosis arises from loss of acid through the kidney or gastrointestinal tract. The causes of metabolic alkalosis can be separated into two groups. Those forms of alkalosis responsive to chloride salt administration (e.g., vomiting), are associated with extracellular fluid volume and chloride depletion. In contrast, alkalosis resistant to administration of chloride salt (e.g., primary aldo

Heat illness. Fluid and electrolyte issues for pediatric and adolescent athletes.

The primary mechanism for maintaining normal body temperature during physical exercise in the heat is the evaporation of sweat. With profuse sweating, water loss far exceeds electrolyte loss. Rigorous exercise in the heat places the athlete at risk for thermoregulatory dysfunction from dehydration. Because children are inherently less efficient thermoregulators than adults, they are at even greater risk for heat illness. The three primary syndromes of heat illness are heat cramps, heat exhaustion, and heat stroke. Treatment of heat illness is based on reduction of body temperature and rehydrat

Severe electrolyte abnormalities and paralytic ileus complicating delirium tremens.

Pneumonia and cardiac arrhythmias represent the most common life-threatening complications during delirium tremens. Electrolyte abnormalities are common underlying conditions in chronic alcoholics and they may further complicate the management of patients with alcohol-withdrawal syndrome or with delirium tremens. The authors present two cases in which the clinical picture of severe paralytic ileus complicating delirium tremens was closely associated with electrolyte status and postulate that the two were cause-effect related. A careful electrolyte supplement therapy guided by a strict monitori
