Evaluation of the recombinant antigens Wb14 and WbT for the capture antibody diagnosis of lymphatic filariasis.

BACKGROUND: Lymphatic filariasis (LF) is a parasitic disease caused mainly by the Wuchereria bancrofti worm and that affects up to 120 million people worldwide. LF is the second cause of chronic global deformity, responsible for 15 million people with lymphedema (elephantiasis) and 25 million men with scrotal hydrocele. Its diagnosis is still associated with numerous difficulties, such as the sample collection periods (microfilaria nocturnal periodicity) and limited diagnostic kits. OBJECTIVES: The aim of this work was to evaluate two recombinant antigens (Wb14 and WbT) as part of an enzyme-l…

Repurposing auranofin as a lead candidate for treatment of lymphatic filariasis and onchocerciasis.

Two major human diseases caused by filariid nematodes are onchocerciasis, or river blindness, and lymphatic filariasis, which can lead to elephantiasis. The drugs ivermectin, diethylcarbamazine (DEC), and albendazole are used in control programs for these diseases, but are mainly effective against the microfilarial stage and have minimal or no effect on adult worms. Adult Onchocerca volvulus and Brugia malayi worms (macrofilariae) can live for up to 15 years, reproducing and allowing the infection to persist in a population. Therefore, to support control or elimination of these two diseases, e…

Therapeutic efficacy and macrofilaricidal activity of doxycycline for the treatment of river blindness.

BACKGROUND: Onchocerca volvulus and lymphatic filariae, causing river blindness and elephantiasis, depend on endosymbiotic Wolbachia bacteria for growth, development, fertility, and survival. Clinical trials have shown that doxycycline treatment eliminates Wolbachia, causing long-term sterilization of adult female filariae and effecting potent macrofilaricidal activity. The continual reinfection by drug-naive worms that occurs in these trial settings dilutes observable anti-Wolbachia and antifilarial effects, making it difficult to estimate therapeutic efficacy and compare different doxycyclin…

Tailoring consent to context: designing an appropriate consent process for a biomedical study in a low income setting.

BACKGROUND: Currently there is increasing recognition of the need for research in developing countries where disease burden is high. Understanding the role of local factors is important for undertaking ethical research in developing countries. We explored factors relating to information and communication during the process of informed consent, and the approach that should be followed for gaining consent. The study was conducted prior to a family-based genetic study among people with podoconiosis (non-filarial elephantiasis) in southern Ethiopia. METHODOLOGY/PRINCIPAL FINDINGS: We adapted a me…

Genital manifestations and reproductive health in female residents of a Wuchereria bancrofti-endemic area in Tanzania.

To assess the significance of lymphatic filariasis for the development of chronic genital manifestations and for reproductive health in women, we conducted a cross-sectional study of 2 villages in north-eastern Tanzania including interview and gynaecological examination of adult women, focusing primarily on reproductive history and genital health. In a population of 2165 residents, prevalence of Wuchereria bancrofti microfilaraemia was 28%, and geometric mean intensity of microfilariae (mf) was 722 mf/mL. Leg lymphoedema (elephantiasis) was present in 4.2% of adults aged > or = 15 years, and h…

Podoconiosis: a comprehensive clinical review and strategies for control and elimination.

Podoconiosis, a neglected tropical disease, manifests as nonfilarial lymphoedema, primarily affecting the lower limbs. It arises due to prolonged exposure of bare feet to irritant red-clay soils in genetically susceptible individuals. The disease is preventable and manageable but remains a public health challenge in endemic areas, including several countries in Africa, Asia and Latin America. This review explores the clinical spectrum, pathogenesis, diagnostic challenges, and strategies for the prevention and management of podoconiosis. Early intervention, including consistent use of footwear

Community views on mass drug administration for soil-transmitted helminths: a qualitative evidence synthesis.

BACKGROUND: Soil-transmitted helminth (STH) infections are amongst the most common infectious diseases worldwide, with an estimated 24% of the world's population currently infected. Mass drug administration (MDA) is the periodic medicinal treatment, without prior individual diagnosis, of at-risk people living in endemic areas. The World Health Organization currently recommends MDA for STHs. MDA programmes are complex health interventions; achieving adherence is important to their success. Adherence is influenced by the target population's perceptions of the drug, the programme, and those deliv

Vulval Lymphedema: An Under-Recognized Clinical Manifestation of Lymphatic Filariasis.

Lymphedema is the accumulation of lymph in soft tissue due to lymphatic dysfunction, often resulting from valvular damage and vessel dilation, which leads to impaired lymph flow. In endemic areas, lymphatic filariasis (LF) is the most common cause of lymphedema, primarily affecting the lower limbs. Vulval lymphedema is often under-recognized and under-reported. Here, two cases of vulval lymphedema treated at a Filariasis Management Clinic are reported. Since both patients reside in an LF-endemic area, vulval lymphedema could be a sequela of filarial infection, particularly in the presence of o

Subnational Projections of Lymphatic Filariasis Elimination Targets in Ethiopia to Support National Level Policy.

BACKGROUND: Lymphatic filariasis (LF) is a debilitating, poverty-promoting, neglected tropical disease (NTD) targeted for worldwide elimination as a public health problem (EPHP) by 2030. Evaluating progress towards this target for national programmes is challenging, due to differences in disease transmission and interventions at the subnational level. Mathematical models can help address these challenges by capturing spatial heterogeneities and evaluating progress towards LF elimination and how different interventions could be leveraged to achieve elimination by 2030. METHODS: Here we used a

Understanding gender and its intersection with social stratifiers on prevention and care seeking behavior of lymphatic filariasis in Nepal.

BACKGROUND: Lymphatic filariasis (LF) is a debilitating and painful neglected tropical disease and is one of the leading causes of permanent disability. In many countries, the intersection of gender with various social stratifiers has influenced exposure to LF and ultimately impacting the disease burden and its elimination. This study aimed to explore the influence of gender and its intersection with other social stratifiers for the prevention and care seeking behavior of LF in Nepal. METHODS: This study employed qualitative research methods: in-depth interviews (IDIs) and focus group discuss

Prevalence of zoonotic (brugian) filariasis in Asia: A proportional meta-analysis.

Lymphatic filariasis is a public health problem and targeted for global elimination. WHO recommends mass drug administration to interrupt transmission of the parasites involved. There are concerns that transmission interruption may be difficult in areas of zoonotic filarial infections. This study aimed to estimate the pooled prevalence of zoonotic brugian filariasis, and to compare the pooled prevalence of brugian filariasis in human and animal populations in the same area based on available studies. A comprehensive literature search was conducted in health-related electronic databases (PubMed

"Whenever I tell her to wear slippers, she turns a deaf ear. She never listens": a qualitative descriptive research on the barriers to basic lymphedema management and quality of life in lymphatic filariasis patients in a rural block of eastern India.

BACKGROUND: Chronic lymphatic filariasis patients in Bihar, India, need management of lymphedema to live a disability-free life. For patients who have recurrent attacks of acute dermato-lymphangio-adenitis (ADLA), World Health Organization (WHO) has recommended simple home-based measures that include maintaining hygiene, skin care, and limb movement. However, patients in rural areas are unable to adopt them, resulting in a vicious cycle of ADLA attacks. There might be multiple realities from patients' and healthcare workers' perspectives that were unexplored. Qualitative research was deemed be
