Necrotizing enterocolitis after red blood cell transfusion in preterm infants with patent ductus arteriosus: a case series.

AIM: Both patent ductus arteriosus (PDA) and packed red blood cell (PRBC) transfusion are risk factors for necrotizing enterocolitis (NEC). The combination of PDA and PRBC transfusion may have a synergistic effect on the intestinal circulation. METHODS: We present four cases of NEC in very low birth weight (VLBW) infants within 14 h after PRBC transfusion. RESULTS: All infants were growing on full enteral feeding, and they all had a PDA. CONCLUSION: We are concerned that the simultaneous presence of a PDA and PRBC transfusion in VLBW infants may place the infant at even greater risk of NEC

Hospital and neurodevelopmental outcomes of extremely low-birth-weight infants with necrotizing enterocolitis and spontaneous intestinal perforation.

OBJECTIVE: We sought to determine the incidence of necrotizing enterocolitis (NEC) and spontaneous intestinal perforation (SIP) in surviving extremely low-birth-weight (ELBW, <1000 g birth weight) infants and to establish the impact of NEC on outcomes by hospital discharge and at 18 to 22 months adjusted age in a large, contemporary, population-based practice. STUDY DESIGN: Hospital outcome data for all ELBW infants born in the greater Cincinnati region from 1998 to 2009 were extracted from the National Institute of Child Health Neonatal Research Network Database. Neurodevelopmental outcome a

Patent ductus arteriosus, indomethacin and necrotizing enterocolitis in very low birth weight infants: a population-based study.

BACKGROUND: Patent ductus arteriosus is a risk factor for the development of necrotizing enterocolitis. The use of indomethacin to treat patent ductus arteriosus in preterm infants may either decrease the incidence of necrotizing enterocolitis by stabilizing or closing the ductus arteriosus or increase its incidence by a direct constricting effect on mesenteric blood vessels. The authors sought to evaluate the interrelationship between patent ductus arteriosus, treatment with indomethacin and the risk of necrotizing enterocolitis in very low birth weight infants. METHOD: The Israel National d

Exploring the impact of spontaneous intestinal perforation and necrotizing enterocolitis treatment modality on shunt outcomes in premature infants.

OBJECTIVE: Spontaneous intestinal perforation (SIP), necrotizing enterocolitis (NEC), and posthemorrhagic hydrocephalus (PHH) are common complications in premature infants. SIP and NEC may require surgery, including peritoneal drainage (PD) and/or laparotomy. PHH typically requires shunting, generally using either a ventriculoperitoneal (VPS) or ventriculoatrial (VAS) shunt. The authors hypothesized that prior abdominal instrumentation with possible adhesion formation in patients undergoing laparotomy may affect VPS placement and longevity. This study examined the impact of SIP or NEC treatmen

Negative pressure wound therapy (NPWT) in the open abdomen in a preterm newborn with necrotizing enterocolitis: case report.

BACKGROUND: Necrotizing enterocolitis (NEC) totalis is a devastating intestinal disease that affects ~ 5% of preterm neonates; its appropriate surgical management is still controversial. Although the temporary open abdomen associated with negative pressure wound therapy (NPWT) has become an increasingly used therapeutic option, its use is still relatively limited in premature babies with NEC. CASE REPORT: On the 6th day of life a male preterm neonate born at 32 + 3 weeks of gestational age (1570 g) developed bowel perforation. He underwent a bed-side laparotomy and a NEC totalis was found with

Development and validation of a nomogram model for predicting the occurrence of necrotizing enterocolitis in premature infants with late-onset sepsis.

BACKGROUND: Necrotizing enterocolitis (NEC), a devastating gastrointestinal disease in preterm infants, is strongly linked to sepsis, and 34-57% of NEC cases develop post-sepsis. However, the risk factors for sepsis-associated NEC are still unclear. Therefore, this study aimed to identify predictive factors associated with the occurrence of NEC in premature infants with late-onset sepsis (LOS) and establish a nomogram for early NEC prediction. METHODS: This single-centre, retrospective cohort study included preterm infants who were diagnosed with LOS and admitted to a tertiary neonatal intens

MicroRNA-320а as a novel biomarker at preclinical stage of necrotizing enterocolitis in term neonates with congenital heart defects.

BACKGROUND: Necrotizing enterocolitis (NEC) remains a prominent gastrointestinal emergency among infants, particularly term infants with congenital heart defects (CHD) being at high risk. The molecular processes that contribute to NEC have yet to be completely understood. The high mortality rates necessitate an active search for noninvasive biomarkers that can aid in the preclinical diagnosis and prognosis of NEC. MicroRNAs (miRs), which are involved in many biological processes in both health and disease, have been discovered to play an important role in regulating inflammation and immune res

LncRNA and mRNA profiles of human milk-derived exosomes and their possible roles in protecting against necrotizing enterocolitis.

Necrotizing enterocolitis (NEC) is one of the most severe diseases commonly afflicting premature infants. Our previous studies suggests that human milk-derived exosomes (HM-Exos) have a potential therapeutic effect on NEC. In this study, we investigate the potentially therapeutic role of HM-Exos in an NEC animal model via comprehensive lncRNA and mRNA expression profiles. A rat model of NEC was induced through hypoxia, hypothermia and formula feeds. We extracted exosomes from the colostrum of healthy lactating mothers and identified their functions in an NEC animal model. Furthermore, high-thr

Clinical Characteristics and Potential Pathogenesis of Cardiac Necrotizing Enterocolitis in Neonates with Congenital Heart Disease: A Narrative Review.

Neonates with congenital heart disease (CHD) are at an increased risk of developing necrotizing enterocolitis (NEC), an acute inflammatory intestinal injury most commonly associated with preterm infants. The rarity of this complex disease, termed cardiac NEC, has resulted in a dearth of information on its pathophysiology. However, a higher incidence in term infants, effects on more distal regions of the intestine, and potentially a differential immune response may distinguish cardiac NEC as a distinct condition from the more common preterm, classical NEC. In this review, risk factors, differen

The impact of a multifaceted quality improvement program on the incidence of necrotizing enterocolitis in very low birth weight infants.

BACKGROUND: Necrotizing enterocolitis (NEC) is a multifactorial gastrointestinal disease which mostly occurs in very low birth weight (VLBW) infants. In addition to decreasing gestational age (GA) or birth weight (BW), artificial formula, delayed initiation or rapidly advanced feeding, severe anemia and systemic infections were associated with NEC. Several studies demonstrated that breast milk, standardized feeding advancement regimens and treatment of anemia are associated with less incidence of NEC. It is not known if including all these interventions in one multifaceted program will lead to

Potential role of endoplasmic reticulum stress is involved in the protection of fish oil on neonatal rats with necrotizing enterocolitis.

Neonatal necrotizing enterocolitis (NEC) is a serious gastrointestinal disease with high death rate in premature infants. Fish oil (FO) and its constituents have been shown to ameliorate intestinal inflammation and mucosal damage. However, the underlying mechanism of action is not known. In the present study, we divided Sprague-Dawley rats into three groups: control group, NEC model group, and FO pre-feeding+NEC model group. Briefly, one week before NEC modeling, in addition to being fed with milk, the FO pre-feeding+NEC modeling group was fed with FO, the NEC group was fed with saline, and th

The Role of Human Milk in Decreasing Necrotizing Enterocolitis Through Modulation of the Infant Gut Microbiome: A Scoping Review.

BACKGROUND: Necrotizing enterocolitis is associated with a high incidence of morbidity and mortality in premature infants. Human milk minimizes necrotizing enterocolitis risk, although the mechanism of protection is not thoroughly understood. Increasingly, dysbiosis of the infant gut microbiome, which is affected by infant diet, is hypothesized to play a role in necrotizing enterocolitis pathophysiology. RESEARCH AIM: The aim of this scoping review was to summarize the state of the science regarding the hypothesis that the gut microbiome composition is a mediator of the relationship between h

Recurrent supraventricular tachycardia and necrotizing enterocolitis: A causative role or a simple association? A case report and literature review.

Insufficiency in mesenteric flow is a risk factor for the development of necrotizing enterocolitis (NEC). Recurrent episodes of supraventricular tachycardia (SVT) can lead to gut ischemia, with subsequent reperfusion injury. We present a term infant who developed NEC at 37 days of life after refractory SVT and reverted to sinus rhythm on day 23 and 25 of life. Resected stenotic ileum and transverse colon demonstrated inflammation with ischemia. This is the first case of NEC following SVT in a term infant without a congenital morphologic abnormality. In view of the temporal sequence of events,

Medical management of motility disorders in patients with intestinal failure: a focus on necrotizing enterocolitis, gastroschisis, and intestinal atresia.

BACKGROUND: Intestinal failure (IF) is the dependence upon parenteral nutrition to maintain minimal energy requirements for growth and development. It may occur secondary to a loss of bowel length, disorders of motility, or both. Short bowel syndrome (SBS) is a malabsorptive state resulting from surgical resection, congenital defect, or diseases associated with loss of absorptive surface area. A particularly vexing problem is associated with whole bowel and/or segmental intestinal dysmotility. Motility disorders within the context of SBS and IF may relate to rapid intestinal transit secondary

Necrotizing Enterocolitis as a Rare Complication of Severe RSV Bronchiolitis in a Term Neonate: A Case Report With Narrative Review.

Respiratory syncytial virus (RSV) bronchiolitis is a leading cause of hospitalization in early infancy, whereas necrotizing enterocolitis (NEC) is rare in term neonates without risk factors. We report a previously healthy, full-term 24-day-old neonate admitted with severe RSV bronchiolitis who developed sudden deterioration with abdominal distension, metabolic acidosis, septic shock, and imaging consistent with advanced NEC, ultimately resulting in death despite intensive medical and surgical management. To contextualize this association, we conducted a narrative review of case reports and sma

The central function and specific molecular mechanisms of the TLR4 pathway underlying impaired intestinal barrier in necrotizing enterocolitis.

Neonatal necrotizing enterocolitisis a fatal gastrointestinal emergency affecting preterm infants, which is linked to destruction of intestinal barrier. The structural overexpression and negative regulation of Toll like receptor 4, a key receptor of lipopolysaccharidefrom Gram-negative bacteria in preterm intestinal epithelial cells, constitute a core hub connecting flora dysbiosis and barrier breakdown. When TLR4 neutrally binds with LPS, it signals the activation of the NF-κB-mediated inflammatory cascade through the MyD88/TRIF pathway which in turn induces apoptosis, necroptosis, and tight
