A proposed regulatory and ethical framework for the application of single-case experimental design methodology in rehabilitation research and clinical practice.

The use of single-case experimental design (SCED) methodology is growing across all domains of rehabilitation. SCED methodology may be used for several different purposes, which creates confusion about its ethical, legal and regulatory status. We will argue that aspects of SCED methodology can be helpful in clinical practice to determine the optimal treatment for a given person. Given the use of SCED methods in research, clinicians may assume that ethics committee approval is required, though this is not necessarily the case given that SCED methods may, under certain circumstances, be consider

Should junior doctors strike?

An impasse in negotiations between the Department of Health (DoH) and the British Medical Association in November this year led to an overwhelming vote for industrial action (IA) by junior doctors. At the time of writing, a last minute concession by DoH led to a deferment of IA to allow further negotiations mediated by the Advisory, Conciliation and Arbitration Service. However, IA by junior doctors remains a possibility if these negotiations stall again. Would the proposed action be ethically justifiable? Furthermore, is IA by doctors ever ethically defendable? Building on previous work, we e

Protocol for the economic evaluation of metacognitive therapy for cardiac rehabilitation participants with symptoms of anxiety and/or depression.

INTRODUCTION: Cardiac rehabilitation (CR) is offered to reduce the risk of further cardiac events and to improve patients' health and quality of life following a cardiac event. Psychological care is a common component of CR as symptoms of depression and/or anxiety are more prevalent in this population, however evidence for the cost-effectiveness of current interventions is limited. Metacognitive therapy (MCT), is a recent treatment development that is effective in treating anxiety and depression in mental health settings and is being evaluated in CR patients. This protocol describes the planne

Prospective accuracy study on an artificial intelligence-based ultrasound system for gestational age estimation among pregnant women in Ghana, Kenya and South Africa: protocol.

BACKGROUND: Risk screening for pre-eclampsia relies on accurate gestational age assessment, but routine access to ultrasound-based gestational dating remains challenging in many low- and middle-income countries. As part of the formative work for the 'Preventing pre-eclampsia: Evaluating AspiRin Low-dose regimens following risk Screening' (PEARLS) platform, we aim to validate and implement an artificial intelligence (AI)-based algorithm for estimation of gestational age, using blind sweeps done with a handheld ultrasound device. This study protocol outlines the accuracy cohort for AI-based gest

Protocol for a cervical screening implementation trial comparing two approaches for delivering HPV self-collection in low-resource settings in India: a type 3 hybrid cluster randomised controlled trial (SHE-CAN).

BACKGROUND: Although multiple studies have offered self-collection for human papillomavirus (HPV)-based cervical screening in community settings, there are no randomised controlled trials (RCTs) that have compared implementation outcomes of programme approaches for self-collection. This trial will compare two such approaches in low-resource settings in the states of Tamil Nadu and Mizoram, India. METHODS: A cluster RCT will be conducted over a year, offering self-collection to 3000 women aged 30-49 from 28 clusters (average size 101) in selected districts. Clusters in tribal, rural and urban

Umbrella review protocol of meta-analyses on intraoperative protective ventilation.

INTRODUCTION: Although the principle of lung-protective ventilation is widely recognised for preventing postoperative pulmonary complications, the optimal intraoperative ventilation strategies remain uncertain. Accordingly, this umbrella review aims to summarise the existing evidence on intraoperative ventilation strategy, evaluate the certainty of evidence and provide reliable evidence to guide clinical practice. METHODS AND ANALYSIS: We will search PubMed, Embase, Cochrane Library and Web of Science from database inception to October 2026 for meta-analyses of randomised controlled trials on

Effectiveness of intervention for aromatase inhibitor-associated musculoskeletal symptoms: A protocol for systematic review and meta-analysis.

BACKGROUND: Aromatase inhibitor-associated musculoskeletal symptoms (AIMSS) are among the most common prominent side effects in patients using aromatase inhibitors (AIs) for breast cancer. Muscle and joint pain, morning stiffness, arthritis, and bone loss are common clinical symptoms in individuals. Traditional Chinese medicine (TCM) has been demonstrated to be useful in the treatment of AIMSS in previous investigations, although the sample sizes were limited, and systematic reviews were inadequate. The effectiveness and safety of TCM in the treatment of AIMSS will be investigated in this stud

Focused ethnography protocol for exploring burn pain management practices in clinical settings: methodological considerations and implementation framework.

INTRODUCTION: Effective burn pain management is profoundly influenced by cultural contexts, and the complexity of this context is intensified by migration. However, the limited understanding of the relationship between culture and pain is an important barrier to delivering high-quality, person-centred care. Achieving a deeper understanding of this phenomenon is essential for developing effective care plans. This approach can mitigate long-term consequences such as post-traumatic stress disorder and ultimately improve the quality of life for burn survivors, moving beyond purely physiological tr

Ethical Governance of Open Data Across Biomedical Research, Healthcare, and Public Health: Privacy, Equity, Trust, and Controlled Access.

Open data has become central to biomedical research and public health, but health information is uniquely sensitive and difficult to share responsibly. In this narrative review, open data is considered as a spectrum of health-data sharing arrangements, ranging from public aggregate datasets to controlled-access repositories, federated analysis, and synthetic data. This narrative review synthesizes the scientific and societal rationale for greater openness with the ethical, legal, and governance constraints that shape what "open" can realistically mean in healthcare. We examine how data sharing

Review of Clinical Equipoise: Examples from Oncology Trials.

BACKGROUND: The current standards that govern clinical research have been shaped over the years through many historical, social, and political events. The third principle of the Belmont Report, Justice, guides the scientific community toward the equal distribution of benefits and risks in research involving human subjects. Clinical equipoise is the status of genuine uncertainty by the investigator about the superiority of one treatment arm over the other. The term clinical equipoise was proposed to provide an ethical ground to conduct randomized controlled clinical trials. OBJECTIVE: The obje
