Developing evidence-based guidance for assessment of suspected infections in care home residents.

BACKGROUND: The aim of this study was to update and refine an algorithm, originally developed in Canada, to assist care home staff to manage residents with suspected infection in the United Kingdom care home setting. The infections of interest were urinary tract infections, respiratory tract infections and skin and soft tissue infection. METHOD: We used a multi-faceted process involving a literature review, consensus meeting [nominal group technique involving general practitioners (GPs) and specialists in geriatric medicine and clinical microbiology], focus groups (care home staff and residen

Consensus and evidence-based medication review to optimize and potentially reduce psychotropic drug prescription in institutionalized dementia patients.

BACKGROUND: Dementia patients often show neuropsychiatric symptoms, known as behavioral and psychological symptoms of dementia (BPSD). These are a common motive for medical consultations, hospitalizations, and nursing home stays. Various studies have suggested that the high prevalence of psychotropic drug use to treat BPSD in institutionalized dementia patients may lead to impaired cognitive capacity, rigidity, somnolence, and other complications during the course of the illness. The aim of this study was to design a consensus-based intervention between care levels to optimize and potentially

Hypertensive Black Men's Perceptions of a Nurse Protocol for Medication Self-Administration.

A factor contributing to uncontrolled hypertension in older persons is medication nonadherence. Older Black men in a rural cardiology clinic were not taking blood pressure medication as prescribed resulting in uncontrolled hypertension. A nurse protocol to assist with self-administration of hypertensive medication was proposed to address the problem. This qualitative key informant study identified a purposive sample of 10 hypertensive Black men who were 65-70 years of age and nonadherent in taking their medication. The nurse conducted teaching sessions using the protocol to review evidence-bas

Outcomes of a Quality Improvement Initiative to Prevent Unnecessary Packed Red Blood Cell Transfusions Among Extremely Low Birth-Weight Neonates.

BACKGROUND: Extremely low birth-weight (ELBW) infants frequently receive packed red blood cell (PRBC) transfusions. Recent studies have shown that more restrictive PRBC transfusion guidelines limit donor exposure and reduce transfusion-related costs without any increase in adverse clinical outcomes. PURPOSE: We developed and implemented an evidence-based PRBC transfusion guideline for ELBW infants treated in our unit and then measured provider adherence to this guideline. METHODS/SEARCH STRATEGY: We performed a retrospective review of all PRBC transfusions given to ELBW infants in 2012 (preg

Risk Factors for Nosocomial Bacteremia Secondary to Urinary Catheter-Associated Bacteriuria: A Systematic Review.

A systematic appraisal of evidence suggests that male patients in hospital may be at higher risk for bacteremia following urinary catheter-associated bacteriuria than females. Other risk factors include immunosuppressant medication, red blood cell transfusion, neutropenia, malignancy, and liver disease.

The cognitive effects of opioids.

Successful opioid therapy often depends on achieving a balance between analgesic effectiveness and side effects. The risk of opioid-induced cognitive impairment often hinders clinicians and patients from initiating or optimizing opioid therapy. Despite subjective experiences of mental dullness and sedation, objective tests of cognitive functioning do not always demonstrate marked changes following opioid administration. To guide clinical practice, as well as patient and family teaching, pain management nurses should be familiar with literature regarding this topic. The purpose of this article

Who should provide routine antenatal care for low-risk women, and how often? A systematic review of randomised controlled trials. WHO Antenatal Care Trial Research Group.

Many activities, the timing and the frequency of visits of conventional antenatal care provided to low-risk pregnant women have most often been introduced without proper scientific evaluation. Few trials, to date, have been conducted to evaluate the effectiveness of antenatal care programmes for low-risk women with varied number of antenatal visits and type of care providers. We have performed a systematic review of these randomised controlled trials. Five randomised controlled trials were identified in which the effectiveness of a schedule of reduced number of antenatal visits (n = 12,883) wa

Championing Quality and Safety Through Perioperative Nurse Advocacy.

Perioperative nurses use evidence-based practice to achieve optimal outcomes when caring for patients. Translating evidence-based guidelines into practice, however, can be difficult, especially when barriers to compliance are present. Compliance barriers may arise from organizational culture, knowledge deficits, absence of policy, shortcuts, competing priorities, time constraints, physician pushback, or system-level barriers. When evidence-based practices are not followed, patient safety events and suboptimal outcomes may occur. This article addresses how legislative advocacy and policy provid

[Recommendations for the specialist further training of nursing personnel on intensive care units in the treatment of abdominal aortic aneurysms: results of a modified Delphi procedure with experts].

INTRODUCTION: The medical development in the previous 15 years and the changes in treatment reality of the comprehensive elective treatment of abdominal aortic aneurysms necessitate a re-evaluation of the quality assurance guidelines of the Federal Joint Committee in Germany (QBAA-RL). In the current version this requires a specialist further training quota for nursing personnel in intensive care wards of 50%. The quota was determined in 2008 based on expert opinions, although a direct empirical evidence base for this does not exist. METHODS: Representatives from the fields of patient represe

A cluster-randomized trial of a complex intervention to encourage deprescribing antidepressants in nursing home residents with dementia: a study protocol.

BACKGROUND: The effectiveness of psychotropic medication on behavioral and psychological symptoms of dementia (BPSD) is limited, while associated with a higher risk of adverse events. Non-pharmacological treatment of BPSD is advocated as treatment of first choice. However, many general practitioners (GPs) find it difficult to initiate deprescribing, and when attempting to discontinue psychotropic medication in nursing home residents, they face many barriers. Therefore, we hypothesize that an intervention aimed at improving communication with and involvement of nursing home staff, relatives, an

Nursing Roles in Diabetic Foot Ulcer Prevention, Wound Management, and Digital Monitoring.

Diabetic foot ulcers (DFUs) remain a major cause of infection, hospitalization, impaired quality of life, and non-traumatic lower-limb amputation among people with diabetes. Effective management requires coordinated prevention, structured foot-risk assessment, pressure offloading, wound care, infection surveillance, patient education, and multidisciplinary follow-up. This narrative review examines the nurse-led and nurse-supported components of contemporary DFU care. Nurses contribute through routine foot and wound assessment, reinforcement of preventive behaviors and treatment adherence, supp

Hospital Digital Transformation, Nursing Data Burden, and Leadership and Governance Capacity: A Critical Narrative Review.

AIM: To critically synthesize how hospital digital transformation reconfigures nursing work and data burden, and to examine the distinct contributions of nurse-leader capability and organizational governance capacity. BACKGROUND: Digital systems can support documentation, surveillance, coordination, and clinical decision-making. The same systems can add redundant entry, fragmented attention, excessive notifications, cognitive workload, and data-quality responsibilities. Technology effects, nursing burden, and implementation leadership are often discussed separately, limiting their usefulness
