Sixty multiple sclerosis (MS) patients (33 definite, 13 probale and 14 suspected were investigated by computed tomography (CT), magnetic resonance imaging (MRI), multimodality evoked potentials (EPs) and cerebrospinal fluid (CSF) electrophoresis. MRI abnormalities were found in 50 cases, while at least one abnormal evoked potential was detected in each of 52 cases. Brain-stem auditory evoked potentials were more sensitive than MRI for the detection of brain-stem involvement. We studied somatosensory evoked potentials (SEPs) and brainstem auditory evoked potentials (BAEPs) in Japanese patients with multiple sclerosis (MS) and those with neuro-Behcet's disease (NB). Abnormal cortical P37 of posterior tibial nerve SEPs or cervical N13 of median nerve SEPs were more frequently found in the MS patients than in the NB patients. On the other hand, prolongation of the central conduction time of median nerve SEPs or abnormal BAEPs were more common in NB than in MS. In human and animal clinics, the early auditory potential has been used for a long time in the detection of lesions through the internal auditory way in the cerebral trunk. The aim of this work tries to determine if this non invasive test is apt to show up and to follow the evolution of lesions at the level of auditory way in the cerebral trunk of sheep with trypanosomiasis. Short-latency somatosensory evoked potentials (SSEPs) were obtained in a 61-year-old patient with a cervical spinal cord glioma extending from C3 to C7. SSEPs in response to tibial nerve stimulation were normal; however, all components of the median nerve responses were markedly abnormal bilaterally (except for normal brachial plexus responses). This selective abnormality of median nerve SSEPs has not been reported previously in a patient with cervical cord tumor associated with a syrinx and illustrates the use of the SSEP in the anatomical localization of the lesion.

The effect of epidural bupivacaine (9 ml 0.5%) analgesia on early (less than 500 msec) somatosensory evoked potentials (SEPs) with electrical stimulation of the T-10 and L-1 dermatomes was examined in eight patients. Cortical amplitudes decreased only insignificantly after stimulation of both dermatomes, despite the presence of sensory analgesia (pin prick) from T-3.5 +/- 0.4 to L-2.9 +/- 0.4 (mean +/- SEM). Syndromes of hemiparetic lacunar infarction include pure motor hemiparesis and ataxic hemiparesis. Twelve such patients were evaluated with somatosensory evoked potentials. Potentials were delayed or absent in all 4 patients with ataxic hemiparesis and in 1 of 8 patients with pure motor hemiparesis. Unlike pure motor hemiparesis, ataxic hemiparesis appears to be associated with abnormal somatosensory evoked potentials. These abnormalities suggest that disturbance of afferent pathways are important in ataxic hemiparesis.

Evoked potentials, action potentials and intracellular events were recorded in the neostriatum of urethane anesthetized rats to electrical stimulation of frontal cortex white matter, motor cortex and pre-limbic cortex. We studied 8 patients with definite multiple sclerosis (3 with acute relapse, 5 with progression), and 2 patients with acute optic neuritis, who were treated with methylprednisolone infusions, 1 g daily for 3 days. Visual, brain stem-auditory and somatosensory-evoked potentials were used to test the effect of therapy. There were no manifest changes of the evoked potentials parameters parallel to the clinical effect of high-dose therapy.

The intravital diagnosis of central pontine myelinolysis has become possible with the introduction of computed tomography and magnetic resonance into neurological diagnostics. These tools are of special value when neurological signs of a ventral pontine lesion are lacking, as in the case we describe. Auditory evoked potentials likewise confirm their diagnostic value with regard both to the site of the lesion and to its dorsal extent toward the pontine tegmentum.

The results of recording intracranially from the auditory nerve, lower brainstem nuclei, and the inferior colliculus in more than 40 patients operated upon for hemifacial spasm and trigeminal neuralgia are presented. A survey is given of my studies of evoked potentials in patients with multiple sclerosis (MS) and in control subjects. Potentials were recorded following random low-rate auditory stimulation (BAEP), checker-board pattern-reversal stimulation (VEP), somatosensory stimulation (SEP); blink reflexes (BR) were recorded following electrical stimulation of the supraorbital nerve.
