Optimising the Dyspnoea Challenge: exertional dyspnoea responses to changing treadmill gradients.

BACKGROUND: The Dyspnoea Challenge is a two-minute treadmill walk designed to measure exertional dyspnoea(ED). To efficiently individualise workload, we aimed to assess; 1) whether the Dyspnoea Challenge is responsive to 1% changes in treadmill gradient and 2) the minimum gradient variation necessary to generate a clinically meaningful change in ED (≥1 modified Borg scale). METHODS: Thirty individuals with COPD(GOLD II-IV) (age: 69.2 ± 9.2 years; FEV1: 49.3 ± 19.1%) completed six Dyspnoea Challenges at a fixed treadmill speed of 3 km·h-1 and at a gradient of between 3% and 8%, performed in ra

Effects of interval training versus continuous training on coronary artery disease: an updated meta-analysis of randomized controlled trials.

Objective: Exercise training-based cardiac rehabilitation is a safe intervention to improve aerobic exercise capacity, metabolic parameters, muscle strength, quality of life, and survival in patients with coronary artery disease (CAD). This study aimed to evaluate the effects of interval training (IT) and continuous training (CT) on CAD for appropriate treatment. Methods: We retrieved 12 high-quality randomized controlled trials (RCTs) till August 13, 2019, focused on the effects of IT and CT on patients with CAD from the Pubmed, Embase, and Cochrane Library databases. The following disease-re

The 'aerobic/resistance/inspiratory muscle training hypothesis in heart failure'.

Evidence from large multicentre exercise intervention trials in heart failure patients, investigating both moderate continuous aerobic training and high intensity interval training, indicates that the 'crème de la crème' exercise programme for this population remains to be found. The 'aerobic/resistance/inspiratory (ARIS) muscle training hypothesis in heart failure' is introduced, suggesting that combined ARIS muscle training may result in maximal exercise pathophysiological and functional benefits in heart failure patients. The hypothesis is based on the decoding of the 'skeletal muscle hypot

Role of cardiopulmonary exercise testing in clinical stratification in heart failure. A position paper from the Committee on Exercise Physiology and Training of the Heart Failure Association of the European Society of Cardiology.

Traditionally, the main indication for cardiopulmonary exercise testing (CPET) in heart failure (HF) was for the selection of candidates to heart transplantation: CPET was mainly performed in middle-aged male patients with HF and reduced left ventricular ejection fraction. Today, CPET is used in broader patients' populations, including women, elderly, patients with co-morbidities, those with preserved ejection fraction, or left ventricular assistance device recipients, i.e. individuals with different responses to incremental exercise and markedly different prognosis. Moreover, the diagnostic a

Rehabilitation in patients with pulmonary arterial hypertension.

Exertional dyspnoea is a leading symptom in patients with pulmonary arterial hypertension (PAH). Patients suffering from PAH report poor quality of life, have skeletal muscle dysfunction and in the absence of advanced medical therapy deteriorate progressively due to right heart failure which can lead to death. For decades, patients with PAH were advised to avoid exercise in fear of exacerbated right heart failure. Recently, it has been shown that a highly supervised rehabilitation programme in expert centres leads to significant improvements in symptoms, quality of life, exercise capacity and

Aerobic capacity is associated with improved repeated shift performance in hockey.

Current research has found conflicting results regarding the relationship between maximal oxygen uptake ((Equation is included in full-text article.)) and the repeated sprint ability (RSA) of hockey players. The purpose of this study was to use sport-specific testing methods to investigate this relationship. Forty-five (range, 18-24) college hockey players completed a graded exercise test on a skating treadmill to ascertain their (Equation is included in full-text article.). An on-ice repeated shift test was then conducted to evaluate each player's susceptibility to fatigue. First gate, second

Women and sport.

Women have historically taken part in sports for many centuries. The first recorded female game competitions were the Herean Games in approximately 1,000 BC, named after the Goddess Hera. Held at Olympia in Greece, these games were for women alone and were thought to have originated as part of ancient fertility rights. Historically there is evidence of sporting activities involving women, but nothing of significance until after the 1948 summer Olympic Games, when 385 female athletes participated. Over the last six decades there has been a noted rise in the number of female athletes, reaching i

Mechanisms of exercise limitation and pulmonary rehabilitation for patients with neuromuscular disease.

Indications for exercise and pulmonary rehabilitation extend to neuromuscular diseases tough these conditions pose particular challenges given the associated skeletal muscle impairment and respiratory muscle dysfunction. These challenges are compounded by the variety of exercise prescriptions (aerobic, muscle strengthening, and respiratory muscle training) and the variety of neuromuscular disorders (muscular, motor neuron, motor nerve root, and neuromuscular transmission disorders). Studies support a level II evidence of effectiveness (i.e., likely to be effective) for a combination of aerobic

Recovery from infectious mononucleosis after altitude training in an elite middle distance runner.

OBJECTIVES: This investigation was designed to monitor altitude acclimatisation in an elite cohort of distance runners and follow the subsequent recovery from infectious mononucleosis which developed in one of these athletes. METHODS: Twenty six national standard distance runners performed treadmill tests 24 days before they travelled to an altitude camp (1500 to 2000 m). One of these athletes was diagnosed as suffering from infectious mononucleosis 14 days after return to sea level. A physician prescribed an individualised training programme which was designed to maximise recovery from the c

Abnormal EKG stress test in rats with type 1 diabetes is deterred with low-intensity exercise programme.

The focus of this study was to determine whether minimal levels of exercise could halt the formation of diabetes-induced heart pathology. Seven-week-old male rats were divided into four groups: sedentary nondiabetic, exercise-trained non-diabetic, sedentary diabetic and exercise-trained diabetic. Individualised exercise programmes were based on the animal's tolerance, and continued for 7 weeks after the induction of diabetes. At the completion of the study, no differences were found in skeletal muscle citrate synthase activity between diabetic sedentary and exercise-trained rats, indicating th

Sex differences between postmenopausal women and similar-age men in response to orthostatic stress following 2 wk of bed rest.

Reduced orthostatic tolerance is common following periods of bed rest that are associated with illness or surgery, putting individuals at higher risk for syncope and falls following hospitalization. Following menopause, mechanisms of female cardiovascular regulation change, which may be associated with sex-specific responses to orthostatic stress following bed rest. The purpose of our experiment was to investigate sex differences between healthy postmenopausal women and similar-age men (age: 55-65 yr) for their orthostatic tolerance and cerebrovascular responses to standing following bed rest.

Is exercise training safe and beneficial in patients receiving left ventricular assist device therapy?

Because a limited number of patients receive heart transplantation, alternative therapies, such as left ventricular assist device (LVAD) therapy, have emerged. Published studies have shown that LVAD implantation, by itself, improves exercise tolerance to the point where it is comparable to those with mild heart failure. The improvement in exercise capacity is maximally achieved 12 weeks after LVAD therapy and can continue even after explantation of the device. This effect varies, depending on the type of LVAD and exercise training. The available data in the literature on safety and benefits of…
