Potential factors contributing to extreme longevity in the Greenland shark.

The Greenland shark (Somniosus microcephalus) is a deep-sea vertebrate inhabiting the cold waters of the North Atlantic and Arctic Ocean and is renowned for its exceptional longevity, with individuals estimated to live for more than 400 years. It has also been proposed as a candidate species exhibiting negligible senescence. This narrative review synthesizes current knowledge on the biological mechanisms that may contribute to this phenotype. Potential contributing factors include its extreme environment, low metabolic rate and remarkably late sexual maturation, all of which may reduce cumulat

Whole-genome resequencing to investigate the genetic diversity and the molecular basis underlying key economic traits in indigenous sheep breeds adapted to hypoxic environments.

Under the combined effects of long-term natural selection and artificial domestication, Tibetan sheep on the Qinghai-Tibet Plateau have evolved distinct ecotypes to survive extreme high-altitude conditions, including hypoxia, cold, and low oxygen levels. These ecotypic variations not only serve as an ideal model for studying plateau livestock adaptation but also harbor valuable genetic diversity. However, the lack of comprehensive genetic analyses on their adaptive and phenotypic traits has hindered the effective conservation and utilization of these resources. Using whole-genome resequencing,

A rare cause of respiratory distress in preterm infants: a case report of acquired subglottic cysts.

BACKGROUND: The Subglottic Cysts (SGCs) are a rare cause of respiratory distress in infants. Typical risk factors include male gender, extreme prematurity, gastro-oesophageal reflux and invasive ventilation, the latter being associated with mucosal damage and blockage of the subglottic cysts' ducts. We describe a case of acquired subglottic cysts in a premature infants presented with respiratory distress. CASE PRESENTATION: A premature male infant was born at 25 weeks + 2 days with a history of monochorionic diamniotic twin pregnancy with twin-to-twin transfusion syndrome. During hospitalizat

Nav1.7 gain-of-function mutation I228M triggers age-dependent nociceptive insensitivity and C-LTMR dysregulation.

Gain-of-function mutations in Scn9a, which encodes the peripheral sensory neuron-enriched voltage-gated sodium channel Nav1.7, cause paroxysmal extreme pain disorder (PEPD), inherited erythromelalgia (IEM), and small fiber neuropathy (SFN). Conversely, loss-of-function mutations in the gene are linked to congenital insensitivity to pain (CIP). These mutations are evidence for a link between altered sodium conductance and neuronal excitability leading to somatosensory aberrations, pain, or its loss. Our previous work in young adult mice with the Nav1.7 gain-of-function mutation, I228M, showed t

Cold and warmth intensify pain-linked sodium channel gating effects and persistent currents.

Voltage-gated sodium channels (Nav) are key players in excitable tissues with the capability to generate and propagate action potentials. Mutations in the genes encoding Navs can lead to severe inherited diseases, and some of these so-called channelopathies show temperature-sensitive phenotypes, for example, paramyotonia congenita, Brugada syndrome, febrile seizure syndromes, and inherited pain syndromes like erythromelalgia (IEM) and paroxysmal extreme pain disorder (PEPD). Nevertheless, most investigations of mutation-induced gating effects have been conducted at room temperature, and thus t

Excited Catatonia - A Delayed Neuropsychiatric Complication of COVID-19 Infection.

An increasing number of patients have been presenting with neuropsychiatric signs and symptoms associated with coronavirus disease (COVID-19). We present a case of a 69-year-old female with no prior psychiatric history who was brought to the emergency department due to bizarre behavior and paranoid thoughts for four to six weeks, worsening over the last two weeks. Psychiatric evaluation found that the patient had extreme restlessness and agitation, poor eye contact, paranoid delusions, visual hallucinations, and a flat affect with stereotypic repetition of speech and loose associations. The pa

Diabetes Insipidus: Pathogenesis, Diagnosis, and Clinical Management.

Diabetes insipidus (DI) is an endocrine condition involving the posterior pituitary peptide hormone, antidiuretic hormone (ADH). ADH exerts its effects on the distal convoluted tubule and collecting duct of the nephron by upregulating aquaporin-2 channels (AQP2) on the cellular apical membrane surface. DI is marked by expelling excessive quantities of highly dilute urine, extreme thirst, and craving for cold water. The two main classifications of DI are central diabetes insipidus (CDI), characterized by a deficiency of the posterior pituitary gland to release ADH, and nephrogenic diabetes insi

Heat-Related Illnesses.

Heat-related illnesses comprise a spectrum of syndromes resulting from disruption of thermoregulation in people exposed to high environmental heat. Symptoms range from heat edema and exercise-associated muscle cramps to exercise-associated collapse, heat exhaustion, and life-threatening heat stroke. Athletes, outdoor laborers, and military personnel are at greatest risk. Several intrinsic and extrinsic factors increase the risk of heat-related illness, including medical conditions, environmental factors, medication use, and inadequate acclimatization. Proper recognition and treatment are effec

Molecular cloning and expression of ghrelin in the hypothalamus-pituitary-gastrointestinal tract axis of the Yak (Bos grunniens) in the Qinghai-Tibetan Plateau.

Ghrelin is a very important brain-gut peptide that modulates appetite and energy metabolism in mammals. The yak is the only large mammal that can adapt to the cold temperatures and hypoxia conditions present in the Qinghai-Tibet Plateau. However, there are no reports on ghrelin molecular characterization and expression in the hypothalamus-pituitary-digestive tract axis of the yak to date. In this study, the coding region sequence of the yak ghrelin, containing a complete ORF (351) encoding for 117 amino acids, was cloned. Immunohistochemistry analysis of the yak samples showed that ghrelin-imm

Environment-Related Variation in the Human Mid-Face.

Previous studies that have examined mid-facial morphology in geographically dispersed and genetically diverse groups of humans have shown a strong adaptation of the nasal part to extreme cold environments, which was not observed in non-Arctic regions. However, it remains unclear whether different parts of the mid-face area show independent adaptation to nonpolar climates, and if so, how this adaptation impacted the morphology. To address this question, we investigated potential associations between climatic variables and the mid-facial shape in 14 populations, focusing on four aspects of the m…

After conisation of the cervix, the perinatal mortality as a result of preterm delivery increases in subsequent pregnancy.

OBJECTIVE: To determine the effects of one or two conisations on preterm delivery and perinatal mortality in subsequent pregnancies. DESIGN: A population-based cohort study. SETTING: Aarhus University Hospital. POPULATION: Preterm delivery and mortality rates were evaluated in 721 deliveries after one conisation, and in 37 deliveries after two conisations, and were compared with 390 deliveries after dysplasia and 74 552 deliveries that were not preceded by conisation or dysplasia. METHODS: Cox regression was used to evaluate preterm delivery rates and perinatal mortality. MAIN OUTCOME MEA

Temperature extremes, density dependence, and southern pine beetle (Coleoptera: Curculionidae) population dynamics in east Texas.

Previous studies of the southern pine beetle, Dendroctonus frontalis Zimm., established that its population in east Texas responds to a delayed density-dependent process, whereas no clear role of climate has been determined. We tested two biological hypotheses for the influence of extreme temperatures on annual southern pine beetle population growth in the context of four alternative hypotheses for density-dependent population regulation. The significance of climate variables and their interaction with population regulation depended on the model of density dependence. The best model included b

Case series of high pressure paint injection injuries in single digits in a tertiary referral centre over a one year period.

High pressure injection injuries to upper extremities are largely rare occurrences. The direct and indirect effects of the injected substance can cause debilitating consequences for those affected and can lead to digit amputation. This case series includes three patients with high pressure injection injuries admitted to a single tertiary centre over one year. One patient was discharged from both plastic surgery and hand therapy 6 months post injury. He reports good function but has residual numbness and cold intolerance. The second individual is still linked to surgical outpatients and has ong

β-blockers are not all the same: pharmacologic similarities and differences, potential combinations and clinical implications.

β-blockers are a heterogeneous class, with individual agents distinguished by selectivity for β1- vs. β2- and α-adrenoceptors, presence or absence of partial agonist activity at one of more β-receptor subtype, presence or absence of additional vasodilatory properties, and lipophilicity, which determines the ease of entry the drug into the central nervous system. Cardioselectivity (β1-adrenoceptor selectivity) helps to reduce the potential for adverse effects mediated by blockade of β2-adrenoceptors outside the myocardium, such as cold extremities, erectile dysfunction, or exacerbation of asthm

Rethinking next-generation vaccines for coronaviruses, influenzaviruses, and other respiratory viruses.

Viruses that replicate in the human respiratory mucosa without infecting systemically, including influenza A, SARS-CoV-2, endemic coronaviruses, RSV, and many other "common cold" viruses, cause significant mortality and morbidity and are important public health concerns. Because these viruses generally do not elicit complete and durable protective immunity by themselves, they have not to date been effectively controlled by licensed or experimental vaccines. In this review, we examine challenges that have impeded development of effective mucosal respiratory vaccines, emphasizing that all of the

Spontaneous resolution of primary hyperparathyroidism post-biopsy-related neck haematoma.

Spontaneous or fine-needle aspiration (FNA)-induced remission of primary hyperparathyroidism (PHPT) is an extremely rare phenomenon with variable outcomes. We report a 75-year-old Male who initially presented with left ureteric calculi and was found to have PHPT. Imaging studies including ultrasound neck, parathyroid sestamibi scan and computed tomography of thorax, abdomen, and pelvis failed to identify the culprit lesion and exploratory parathyroidectomy was planned. Before surgery, he underwent FNA for cytology of a right cold thyroid nodule which was complicated with a large neck haematoma

Homodigital Unipedicled Islanded Neurovascular Pulp Advancement Flap/Colombo Flap for Fingertip Reconstruction: A Case Series with a Novel Technique.

Fingertip injury is the most common type of upper extremity injury. Operative treatment with flap cover is required for tissue loss causing bone exposure and when there is a compelling need to preserve the length, unless microvascular replantation is possible. There are many techniques described for this purpose offering varying degrees of good outcomes. Yet there are limitations and long-term problems. We report a case series with a novel technique named "Colombo flap," which is a neurovascular islanded advancement flap based on a single pedicle. Five fingers of four consented patients were o

Dry ice in the eye: A rare occupational hazard!

Superficial corneal foreign bodies (FBs) are a common occupational ocular hazard and can cause visually important astigmatism. A 23-year-old male working in a factory, which produced dry ice (solid carbon dioxide), presented to our emergency unit with symptoms of whitish discolouration of the right cornea for 1 day. Visual acuity in both eyes was 20/20 and slit-lamp examination of the right eye revealed an irregularly shaped whitish superficial FB in the inferior paracentral cornea with surrounding superior nebular opacity. Its gentle removal with a moistened cotton bud revealed an underlying

Successful Lower Limb Replantation of Knee-Level Amputation in a Child: A Case Report.

Replantation of a lower extremity amputated at the knee joint level in a child is rare. We present a case of a 3-year-old child with his right leg totally severed from the knee joint in a traumatic accident. After 5 hours of cold ischemia time, together with antishock therapies, the child underwent leg replantation under stable conditions. The replanted lower limb finally survived after several surgical procedures. The patient was discharged 4 weeks after admission and had restored partial motor and sensory functions 6 months after surgery. During follow-ups, the patient underwent sustained re
