Extensive corneal disease secondary to microbial keratitis can result in frank or impending corneal perforation requiring a large penetrating keratoplasty. In an 8-year period, 26 penetrating keratoplasties with recipient beds of > or = 9.5 mm were performed on 22 eyes: 11 for bacterial keratitis, 10 for fungal keratitis, and one for a mixed bacterial and fungal keratitis. The graft failed in 18 of 19 eyes (94.7%), with a median time to failure of 12.9 weeks in bacterial keratitis and 4.0 weeks in fungal keratitis. After large keratoplasty, 17 of 20 eyes (85.0%) maintained the structural integ

BACKGROUND: To correlate vision-limiting complications of open-globe trauma with anatomical zone and mechanism of injury.
METHODS: Retrospective review of 235 patients with open-globe injuries at the Massachusetts Eye and Ear Infirmary. Vision-limiting complications were assessed at 2 to 3 months after the injury.
RESULTS: Traumatic cataracts and corneal scarring were the most prevalent vision-limiting complications in patients with zone I (cornea-only) lacerations. The most common vision-limiting factors in eyes with zones II and III lacerations (involving sclera) were cataracts and retinal

INTRODUCTION: Optic nerve (ON) enhancement alone without ON thickening on contrast-enhanced magnetic resonance imaging (CE-MRI) can be associated with post-laminar optic nerve invasion (PLONI) in eyes with group E retinoblastoma. A few case reports and retrospective studies in the literature show a poor correlation between ON enhancement on MRI and ON invasion on histopathological examination (HPE). There is no universal consensus on the management of such cases. It is desirable that the presence and extent of a true ON invasion be reliably picked up before planning upfront enucleation in orde

BACKGROUND: On account of loss of eye following Rubella infection, a 13-month-old baby girl patient required a maxillofacial prosthesis to restore her facial esthetics and social health as she grows.
CASE PRESENTATION: The process of prosthesis fabrication began at the time of enucleation where a conformer was given. Post healing the procedure was completed in a span of 2 days taking adequate trials and cross references for the like-like appearance of the prosthesis.
CONCLUSION: A heat-cure acrylic resin based prosthetic eye was delivered with adequate instructions on the insertion and remov

BACKGROUND: Lymphoma with intraocular metastasis is an uncommon and serious disease. We describe a case of diffuse large B-cell lymphoma (DLBCL) with iris metastasis. Meanwhile, we refer to published case reports retrieved via a PubMed search to summarize this rare disease.
CASE PRESENTATION: Glaucoma and uveitis symptoms were found in the left eye of a 50-year-old woman upon admission to the hospital. After treatment and pathological examination, the iris of her left eye was diagnosed with DLBCL. Given the patient's unfavorable treatment options in the local hospital, primary enucleation was

Benign smooth muscle tumor, leiomyoma, can occur throughout the whole body, including the most common sites of the genitourinary and gastrointestinal tracts, less frequently in the skin, and rarely in the deep soft tissue or uveal tract. A 23-year-old Chinese woman presented with a 4 month history of a pigmented ciliary body mass in her left eye. The tumor's clinical appearance and ancillary data were initially consistent with malignant melanoma. However, instead of enucleation, sclerotomy with a tumor biopsy was performed because of the patient's young age and the possibility of benign tumors

BACKGROUND: Adenocarcinoma of the retinal pigment epithelium (RPE) is an uncommon intraocular tumor which has been rarely if ever diagnosed prior to enucleation. Our review of the literature suggests that when a melanotic and tuberous-shaped tumor presents in a woman with signs of uveitis one should consider the diagnosis of adenocarcinoma of the RPE.
METHODS: We report a case in which a 4.9-mm-tall, mushroom-shaped, moderately reflective melanotic tumor found to be present in a 50-year-old woman. Other findings included a 12 x 10-mm dense transillumination shadow and 270 deg of posterior iri

Four hundred and forty ocular and orbital tumours seen in Ibadan over 11-year-period (1980-1990) are reviewed. Three quarters of the 252 patients who had enucleation had malignant neoplasms, of which 55% were retinoblastomas. The mean age of patients with retinoblastomas (3.2 years) was less than that of patients with carcinoma and malignant melanomas (47 years). Malignant melanomas were rare neoplasms in Ibadan. The commonest orbital lymphoma was Burkitt's lymphoma. Only 24% of the enucleated eyes had inflammatory conditions, of which 38% gave a positive history of trauma. Inflammatory lesion

Intraocular cutaneous melanoma metastasis (ICMM) is a rare event, accounting for only 5% of all metastases to the eye and orbit. The vast majority of such metastases primarily affect the choroid and vitreoretinal structures. Only three previous cases of predominant lens structure ICMM have been reported in the literature. Histological examination, in all three past cases, was performed on enucleation specimens of painful blind eyes. We present the first case of ICMM to the lens capsule in a comfortable, seeing, pseudophakic eye. This was histologically confirmed following diagnostic pars plana

A 74-year-old man presented with light perception and presumed early bacterial endophthalmitis in the left eye after cataract surgery. Vitreous tap biopsy and core vitrectomy were performed immediately, along with injection of antibiotic agents (ceftazidime and vancomycin). Culture of the vitreous tap revealed Pseudomonas aeruginosa sensitive to ceftazidime. The eye remained inflamed despite 2 additional intravitreal ceftazidime injections. Orbital cellulitis with perforation of the globe was suspected and confirmed on magnetic resonance imaging, and enucleation was performed. Endophthalmitis

Optic nerve aplasia is a rare congenital defect invariably associated with other ocular or systemic disorders. We examined a 3-year-old girl with monocular microphthalmos who had optic nerve aplasia on histopathologic examination of the eye after enucleation. Magnetic resonance imaging verified the presence of unilateral optic nerve aplasia, and demonstrated hemichiasmal hypoplasia on the affected side and bilateral optic tracts. The visually evoked cortical response revealed increased signals over the occipital cortex ipsilateral to the aplastic nerve, suggesting misdirection of axons from th

We report two brothers aged 6.6 and 5 years old with the De Sanctis-Cacchione variant of Xeroderma Pigmentosa. They had typically severe skin, and ocular and neurologic involvement. Both had epibulbar squamous cell carcinomas. The oldest presented with a large, invasive, epibulbar mass of the left eye which required enucleation. He also had an invasive squamous cell carcinoma of the right lower lid. The younger brother presented with a smaller papillomatous lesion at the medial limbus of the right eye. After removal, this was found to be squamous cell carcinoma in-situ. Flow cytometry done on

A 17-year-old woman with mitral and tricuspid valve prolapse and myxomatous degeneration presented puerperal infection by Staphylococcus aureus with clinical picture of sepsis and multiple septic embolism (right eye, left thumb, spleen, and left calf). She underwent total hysterectomy on the 10th day postdelivery and right eye enucleation on the 16th. Temporary total AV block occurred on the 14th day with temporary external pacing during the next couple of days. Acute endocarditis with acute mitral regurgitation was diagnosed on the 13th day, demanding immediate valve replacement. On the 46th …

We present the case of a 22-year-old woman who underwent surgical excision of a malignant melanoma of the skin three years previously and whom we saw for a pigmented tumor of the iridocorneal angle of her right eye associated with end-stage secondary glaucoma. After the clinical diagnosis of ring melanoma, she underwent enucleation of the eye. The pathologic examination showed a lesion predominantly appearing as a nevus with areas of borderline melanoma formation. The main features of this case were the young age of the patient, the predominant nevoid appearance of this diffuse ciliary body tu

A 26-year-old woman underwent enucleation of a blind, painful eye because of a perforated corneal ulcer. Histopathologic examination of the eye disclosed an acute, necrotizing keratitis surrounding myriad small (2.5 to 3 x 4.5 to 5 micrometer), oval, faintly visible structures. Under polarized light, they were partially birefringent and stained intensely positive with the acid-fast technique. The organisms were located mainly within the cytoplasm of histiocytes in the deep corneal lamellae. They were interpreted as a protozoa of the genus Nosema. The diagnosis was confirmed by electron microsc

PURPOSE: To evaluate anatomical and functional prognosis of urgent penetrating keratoplasty in perforated infectious corneal ulcers.
METHODS: Seven eyes of seven patients had an urgent penetrating keratoplasty for perforated infectious corneal ulcers. Anatomical success was defined by eradication of infection and preservation of eye from enucleation and phthisis bulbi. Corneal graft survival was defined by the presence of a clear graft.
RESULTS: The patients'mean age was 40.5 years. Four eyes had bacterial corneal ulcer and three eyes had herpetic corneal ulcer. Anatomical success was obtain

PURPOSE: Eye plaque brachytherapy (EPBT) is the most common treatment for uveal melanoma with high local control rates of 95-100%. When local recurrences occur following EPBT, salvage options include enucleation, transpupillary thermotherapy (TTT), external beam radiation, or re-irradiation with EPBT. The purpose of this study is to report our institution's experience with EPBT re-irradiation for locally recurrent uveal melanoma.
METHODS AND MATERIALS: Patients were included if they were previously treated for uveal melanoma with EPBT, experienced local recurrence, and were subsequently treat

PURPOSE: To report the efficacy of primary retrobulbar chlorpromazine injections for the management of blind and seeing painful eyes.
PATIENTS AND METHODS: Retrospective case series from three university hospitals was done. Twenty eyes of 20 patients with at least 3 months of follow-up are reported. Ten of these 20 eyes did not receive any other ophthalmic medications either 2 weeks before chlorpromazine injection or at any time point after injection. Treatment was considered successful if pain control lasted for at least 3 months, without the need for reinjection or enucleation.
RESULTS: Si

Ophthalmomyiasis interna posterior (OIP) means infection of the posterior part of the eye with the larvae of flies. Few cases have been reported in which the larvae have been removed from the eye and the visual acuity preserved. We report four cases of OIP with living larva in the vitreous cavity. The larvae produced severe uveitis and were removed alive with vitrectomy and a procedure for removing foreign bodies. They were identified as first-stage reindeer warble fly larvae (Oedemagena tarandi). We also observed dead larva in the vitreous cavity of a patient who suffered severe uveitis in 19
