The versatile frontal sinus approach to the floor of the anterior cranial fossa. Technical note.

A technique to expose the anterior cranial base is described with entry through the anterior and posterior walls of the frontal sinus. Burr holes are avoided in the visible portion of the forehead. Expansion of the operative field may be accomplished, if necessary, by supplemental superior frontal or supraorbital rim osteotomy. The technique is rapid, safe, and provides excellent operative exposure and superior cosmetic results.

[Current aspects of xeroderma pigmentosum: apropos of two new cases].

Two recent cases of children treated for xeroderma pigmentosum are reported, and knowledge of the disease updated based on numerous reports published over the last few years, particularly by french authors ( Vaillant et al., 1979 and Mouly , Dufourmentel and Papadopoulos , 1980). Points discussed include the similarity of histology and course of the malignant cutaneous lesions with those of adult outdoor workers (co- carcinogenic role of ultraviolet rays ?), and the effect of the introduction of vitamin A acid into treatment on the course of the general and cutaneous lesions (immunocompetence,

Enchondromatosis with hemangiomas (Maffucci's syndrome).

Maffucci's syndrome in a 17-year-old girl is reported. Since the entity was first described in 1881 by Maffucci, 105 additional cases have been recorded. Several series have reported a small but significant percentage of cases (up to 18%) with malignant change. Our case, although not particularly unique in comparison with other cases of this syndrome, is presented because of the infrequent occurrence of this syndrome.

Enhancing Diagnostic Precision and Clinical Outcomes With FNAC in Parotid Gland Masses.

BACKGROUND: Parotid gland masses encompassing both neoplastic and non-neoplastic conditions present diagnostic challenges. Fine-needle aspiration cytology (FNAC) is commonly used to differentiate between benign and malignant lesions but has limitations such as variable sensitivity and potential sampling errors. This study assesses FNAC's accuracy for parotid gland lesions, evaluates the Milan Salivary Gland Cytopathology Reporting System (MSRSGC), and compares FNAC results with histopathological findings. METHODS: This retrospective study at our medical center analyzed data from 2321 patients

Facial nerve function after petrosectomy.

OBJECTIVES: Evaluation of facial nerve function after petrosectomy in a patient series with facial nerve denudation-decompression, forward or backward rerouting, and facial nerve suture and grafting. STUDY DESIGN: Fifty-six patients with petrosectomies performed for 24 benign and 9 malignant tumors of the petrous bone, 13 malignant tumors of the parotid gland or of the infratemporal spaces with infiltration of the petrous bone, 8 traumatic facial nerve disruptions, and 2 osteoradionecroses were retrospectively evaluated with respect to facial nerve function. Sixteen cases involved a partial,

Neuritis Ossificans in the Facial Nerve: A Case Report.

OBJECTIVES: Neuritis ossificans is a rare condition involving heterotopic ossification of nerves, usually affecting major peripheral nerves. Two cases involving cranial nerves are documented, with none affecting the facial nerve. The aim of this report is to present the first documented case of neuritis ossificans involving the facial nerve (CN VII), with a review of clinical, radiologic, and histologic findings. METHODS: An 88-year-old male with a history of radiation therapy for right ear basal cell carcinoma presented with 18 months of progressive right-sided facial weakness, resulting in

Prophylactic cross-face nerve flap for muscle protection prior to facial palsy.

The facial muscles of a 28-year-old woman with left acoustic neuroma were successfully protected with a vascularised cross-face nerve flap using a vascularised lateral femoral cutaneous nerve along with a perforator of the lateral circumflex femoral system. It was transferred as a vascularised cross-face nerve flap to bridge a 15-cm-long defect between the bilateral buccal branches. Three months after the nerve flap transfer, the total tumour including the facial nerve was resected. Postoperatively, rapid nerve sprouting through the nerve flap and excellent facial reanimation were obtained 3-6

[Facial paralysis in children--study of 99 cases].

Ninety-nine cases of peripheral facial paralysis in children treated at Ste. Justine's Hospital over a 10 year period were reviewed. Sexes were nearly equally affected with a slightly higher incidence in female. Cases were distributed quite evenly throughout childhood except for a definitely higher incidence in the 0--2 year old group. Trauma, Bell's palsy, and otomastoid inflammation were found to be the most frequent causes of facial palsy in that group of cases. Cases of otomastoid inflammation did generally well on antibiotics and nearly 70 per cent so recovered completely. Twenty per cent

A retrospective analysis on the management of pigmented lesions using a picosecond 755-nm alexandrite laser in Asians.

BACKGROUND AND OBJECTIVES: Photo-aging in Chinese often presents with benign pigmentary lesions. Q-switched lasers for pigmentary lesions in Asians had reported a risk of post-inflammatory hyperpigmentation (PIH) up to 25%. Longer pulse widths in the millisecond domains were advocated with reduced risk of PIH. Recently, picosecond lasers of various wavelengths were introduced with proven efficacy in tattoo removal. The objective of this study is to assess the efficacy and safety of a novel picosecond 755-nm alexandrite laser for the treatment of benign pigmented lesions in Asians retrospective

[Facial aesthetics part I - the significance of the triangle of yarbus].

When humans observe a face, the eyes, mouth and nose regions are the preferred targets of the centre of gaze, with the outlines of eye position recordings approximating a triangle with vertices located in the centre of the eyes and mouth [31]. We studied the significance of cutaneous asymmetries inside and outside the Yarbus triangle on the basis of the assessment of digitally-manipulated images by 201 independent judges. We show that certain facial variations (such as naevi) are more attractive when asymmetrical than symmetrical (p<0,001). They appear more attractive when located laterally ra

Radiotherapy for maxillo-facial hemangiomas in children. Dental and periodontal long term effects.

Vascular maxillo-facial malformations, common pathologies in children, represent 7% of all benign tumors. They are divided into vascular malformations and hemangiomas. In the 70s-80s radiotherapy was the most applied technique, but because of its misuse or overdose, it could lead to relevant and hard consequences for the impact on the salivar glands, periodontium and growing maxillo-facial nuclei. The authors describe the case of a 38-year-old man, who, during infancy, was submitted to radiotherapy for a hemangioma to his chin and cheek. He was under observation for an agressive periodontitis,

[Squamous epithelial carcinoma as a complication of lupus vulgaris].

Lupus vulgaris represents a type of cutaneous tuberculosis that, if not correctly treated, presents a progressive chronic development whose long-term complications include cutaneous neoplasms. We report on a 55-year-old man presenting a 10-year-old ulcerated and bleeding lupus vulgaris lesion on the left cheek containing a squamous cell carcinoma. Reviewing the literature, we search for possible causes of delay in diagnosis that even now allowed therapy only after discovery of the tumoral complication.

Imaging features and clinical significance of perineural spread or extension of head and neck tumors.

Perineural spread of head and neck tumors is a form of metastatic disease in which tumor disseminates to noncontiguous regions along the endoneurium or perineurium. Both computed tomography (CT) and magnetic resonance (MR) imaging can help detect perineural spread, although MR imaging is the modality of choice because of its multiplanar capability, its superior soft-tissue contrast, and the decreased amount of artifact from dental hardware. Perineural spread most commonly occurs in adenoid cystic carcinoma and squamous cell carcinoma. Nerve enlargement may lead to foraminal enlargement and, ul

Naevi spili, Café-au-lait spots and melanocytic naevi aggregated alongside Blaschko's lines, with a review of segmental melanocytic lesions.

This is the first case in the literature describing naevi spili, café-au-lait spots and melanocytic naevi aggregated along-side Blaschko's lines. The pattern of melanocytic lesions in our patient is different from the congenital pigmentary syndromes and the segmental distribution of melanocytic naevi, the quadrant distribution of dysplastic naevi or the partial unilateral lentiginosis which here are shortly reviewed. The distribution may be a result of a somatic mutation occurring at an early stage of embryogenesis when neural structures had already been formed.
