Incontinence in patients with dementia.

The high prevalence of incontinence in dementia sufferers will only increase as the population ages (Alzheimer's Society, 2007), but the evidence-based knowledge for management of these long-term disorders combined is lacking (Hägglund, 2010). Management techniques for incontinence need to be developed to ensure that dementia patients receive the best care, as current methods such as behavioural techniques may not be appropriate for people with limited cognitive function. This article will address issues that arise with current incontinence management for dementia sufferers and possible course

Differences in anal sphincter function and clinical presentation in patients with pelvic floor descent.

Perineal descent is found in patients with idiopathic fecal incontinence and patients with the descending perineum syndrome, who have little or no incontinence but present with a symptom pattern that suggests obstructed defecation. To investigate why patients with perineal descent present in different ways, manometric, radiologic, and neurophysiologic studies were performed in 53 patients with radiologically proven perineal descent and 34 control subjects. Thirty-two patients exhibited incontinence to rectally infused saline, while 21 patients presented with obstructed defecation but exhibited…

Preventing deconditioning and incontinence in older people in acute care settings.

Hospital-associated deconditioning is a recognised and well-known condition. It refers to a loss of physiological fitness and functional ability in a patient resulting from prolonged immobility, inactivity or bed rest during a period of hospitalisation. Many patients who experience deconditioning also have new or existing continence issues, although the link between deconditioning and incontinence is not well understood. This article explores deconditioning and incontinence among older people in acute care settings. It outlines the multifactorial links between the two conditions and reviews cu

The neurogenic hypothesis of stress incontinence.

There are a number of causes of incontinence. The common forms of urinary incontinence, faecal incontinence or double incontinence, are stress related, in that voiding of urine or faeces occurs in response to a sudden increase in pressure in the bladder or anorectum that is not opposed by an adequate pressure increase in the sphincteric region. This weakness of the sphincter mechanism is due to chronic partial denervation of the striated sphincter muscles of the pelvic floor, comprising the external and sphincter muscle and puborectalis (puboanalis) components of the voluntary anal sphincter m

Quality of life outcomes in children with Hirschsprung disease.

BACKGROUND: Morbidity following repair of Hirschsprung disease (HD) is common. However, quality of life (QoL) results focused on HD children are contradictory. We aimed to measure QoL outcomes in HD children using validated questionnaires. METHODS: Patients with HD, managed at a large tertiary pediatric institution between 2004 and 2013, were identified. Parents completed validated questionnaires. Results were compared with published healthy population controls. QoL outcomes were measured using Pediatric Quality of Life (PedsQL) and Fecal Incontinence and Constipation Quality of Life (FIC QOL…

Role of the enterostomal therapy nurse in ostomy patient rehabilitation.

Enterostomal therapy (ET) nurses specialize in the management of patients with urinary and fecal diversions, draining wounds and fistulas, fecal and urinary incontinence, and chronic wounds such as pressure ulcers and vascular ulcers. ET nurses have much to offer in the management of patients with cancer. Such nurses play a major role in the rehabilitation of patients undergoing fecal or urinary diversions. Preoperative services include: counseling regarding planned surgical procedure, the impact of an ostomy on the patient's life, and the basics of ostomy management; sexual counseling; and st

Lateral internal sphincterotomy is superior to topical nitroglycerin for healing chronic anal fissure and does not compromise long-term fecal continence: six-year follow-up of a multicenter, randomized, controlled trial.

PURPOSE: Although there is enthusiasm for nonoperative management of anal fissures, most trials have been of short duration (6-8 weeks) and … r surgical treatment, and patient satisfaction. Furthermore, patients were assessed for symptoms of fecal incontinence using the Jorge and Wexner Fecal Incontinence Score and the Fecal Incontinence Quality of Life questionnaire. RESULTS: Overall, 51 of the original 82 patients (62 percent, 27 nitroglycerin, 24 lateral internal sphincterotomy) completed our survey. …

Repair techniques for obstetric anal sphincter injuries: a randomized controlled trial.

OBJECTIVE: To compare one-year outcomes of primary overlap versus end-to-end repair of the external anal sphincter after acute obstetric anal sphincter injury. METHODS: Women who sustained third-degree (3b = greater than 50% external anal sphincter thickness, 3c = internal sphincter injury) or fourth-degree (including anorectal epithelium) perineal tears were randomly allocated to either immediate primary overlap or end-to-end repair. They were prospectively followed up for 12 months postrepair with serial questionnaires. The primary outcome was fecal incontinence at 12 months. Secondary outc…

Surgical Technique for Sphinkeeper® Implantation.

Fecal incontinence is a distressing condition characterized by the involuntary loss of solid and liquid stool and gas, It affects a significant proportion of the general population, with a reported prevalence ranging from 1% to 20%. Despite its considerable impact on quality of life, therapeutic options for fecal incontinence remain limited. Current treatment modalities for fecal incontinence include conservative approaches such as dietary modifications, pelvic floor exercises, and pharmacotherapy. Surgical interventions, including sphincteroplasty or sacral nerve stimulation, may be considere…

[A speed-constant rectal lavage apparatus prepared in laboratory].

OBJECTIVE: To design a speed-constant rectal lavage apparatus for assessing the rectal and anal function in liquid stool continence and the degree of fecal incontinence. METHOD: The apparatus was assembled by four common rectal lavage apparatuses, each with a container and a drainage tube. RESULTS: The device could output a pulsed and speed-constant water stream with high pressure, capable of overcoming any rectal resistance change. The test for liquid continence in 12 patients with incontinence (n=12) and 12 normal control subjects (n=12) found that the retained liquid volume was significan…

[Role of defecography in female posterior pelvic floor abnormalities].

Pelvic floor abnormalities often impact significantly the quality of life and result in a variety of symptoms, including chronic pelvic pain, fecal incontinence, and obstructed constipation. Fluoroscopic defecography and MR defecography enable identification of rectocele, rectal prolapse, enterocele, sigmoidocele with high prevalence in female patients with obstructed constipation, fecal incontinence, and chronic pelvic pain. In this manuscript, we describe the techniques and indications of the two techniques of defecography. We discuss the abnormalities of the posterior pelvic floor compartme…
