Anatomy of the dysplastic hip and consequences for total hip arthroplasty.

Total hip arthroplasty in dysplastic hips is challenging because of the modified anatomy of the proximal femur and acetabulum. We studied three-dimensional anatomic parameters in 247 dysplastic hips from 218 adult patients using radiographs and computed tomography, and analyzed the consequences for total hip arthroplasty. A cohort of 310 primary osteoarthritic hips was used as a control group. According to the classification of Crowe et al, 78 of the dislocated hips were graded Class I, 26 as Class II, 20 as Class III or IV, and 169 dysplastic hips had no subluxation. Compared with primary ost

Does the Proximal Femur Geometry Predict Early Functional Outcome after Plate Fixation of Geriatric Pertrochanteric Fractures?

BACKGROUND: The proximal femur geometry determines the hip force distribution. The femoral neck axis length (FNAL), the hip axis length (HAL), the femoral head diameter, and the femoral neck-shaft angle (FNSA) could influence the risk and outcome of pertrochanteric fractures. Restoring these parameters to their prefracture values could predict early hip function. AIM: To determine if the postoperative proximal femur geometry of geriatric patients with plating for pertrochanteric fractures predicts the early functional outcome. MATERIALS AND METHODS: The study was a prospective study carried

Fat embolism syndrome.

OBJECTIVES: To assess the incidence and risk factors for fat embolism syndrome. MATERIALS AND METHODS: Data from the National Hospital Discharge Survey (NHDS) were analyzed using International Classification of Diseases, Ninth Revision, Clinical Modification (ICD-9-CM) codes. RESULTS: From 1979 through 2005 among 928,324,000 patients discharged from short-stay hospitals in the United States, 41,000 (0.004%) had fat embolism syndrome. Among 21,538,000 patients with an isolated fracture of the femur (any site), tibia, fibula, pelvis, ribs, humerus, radius, or ulna, 25,000 (0.12%) developed fat

Surgical maneuvers placing the sciatic nerve at risk during total hip arthroplasty as assessed by somatosensory evoked potential monitoring.

The sciatic nerve in 52 hip arthroplasties was evaluated using intraoperative somatosensory evoked potentials (SSEPs). Twenty-nine of these cases involved the lateral transtrochanteric approach, and 23 involved the posterior approach. A total of 11 incidents of SSEP changes occurred in eight patients. Six episodes occurred during lateral retraction of the proximal femur, and three occurred during anterior retraction of the proximal femur. Tracings returned to baseline with prompt cessation of femoral retraction in each case. One SSEP change occurred in a revision following reduction of the pro

Femoroacetabular impingement: role of imaging.

The femoroacetabular impingement (FAI) is an impingement characterized by repetitive abutment between the femur and the acetabular rim during hip motion due to loss of joint clearance (Imam and Khanduja in Int Orthop 35(10):1427-1435, 2011; James et al. in AJR Am J Roentgenol 187(6):1412-1419, 2006). Femoroacetabular impingement (FAI) can be classified as either cam or pincer type, and it can be differentiated on the basis of a predominance of either a femoral or an acetabular abnormality (Pfirrmann et al. in Radiology 244(2):626, 2007; Ganz et al. in Clin Orthop Relat Res 466(2):264-272, 2008

Cannulated Screw Fixation For Femoral Neck Fractures : A 5-year Experience In A Single Institution.

ABSTRACT: Cannulated screw fixation is a widely accepted surgical method for management of fractures of the neck of femur especially in patients with poor premorbid conditions, minimally displaced fractures and those from a younger age group. A five year retrospective study was carried out in 53 consecutive patients between 2006 to 2010 to determine the pattern of injuries, management, outcomes and the associated predictive factors. All the patients underwent cannulated screw fixation, with 37 (69.8%) having had surgery within 24 hours and the remaining 16 (30.2%) 24 hours after the initial inj

[Bone mineralization in insulin-dependent diabetes mellitus].

One of insulin-dependent diabetes mellitus (IDDM) complications are bone mineral disorders called diabetic osteopathy. Because of many controversies of this subject we took a research of bone mineralisation in IDDM patients before 40 years old. The evaluation of BMD was performed with the use of X-ray dual energy absorptiometry--DEXA in the AP projection for lumbar part of vertebral column (BMD L2-L4), left femur neck (BMD-neck) and total skeleton (BMD-total). Bone tissue metabolism was evaluated with the aid of biochemical tests. The examined group consisted of 99 patients with IDDM (45 women…

[Topography of tumoral and pseudotumoral lesions of the proximal femur].

This study of the topographic distribution of tumoral and pseudotumoral lesions of the proximal femur shows that certain lesions have a preferential site, for example osteoid osteoma affects the internal cortex of the neck and diaphysis or the intertrochanteric zone; chondroblastoma occurs in zones of epiphyseal ossification of the head; fibrous dysplasia affects the femoral neck, while sparing the epiphyseal femoral head and trochanters. The island of osteosclerosis is situated, at least partially, in the support fan; so-called physiological cysts are situated on or above the midline of the n

[The role of static-dynamic factors in genesis of femoral head and neck deformation in dysplastic hips. A physical-mathematical interpretation ].

The clinical material for a physical-mathematical analysis of femoral head and neck deformations in subluxated and luxated hips consisted of 1.574 case histories of children operated on in 1954-1988. In that material 21.9 per cent of the children had preoperative growth disorders of the head, and in the half of the cases also the femoral neck was involved. In 10.79 per cent of the children pre- and postoperative trophic disorders were also observed. On the basis of the physical-mathematical analysis of static-dynamic factors of the hip, it has been determined that the disordered shape of the f

Medullary impairment at early stage of non-traumatic osteonecrosis of the femoral head.

We found that a painful hip without radiological changes presented a modification of the magnetic resonance image of the femoral head and neck with diffuse low T1 weighted and high T2 weighted signal. Core biopsy showed that the bone marrow was replaced from the neck to the head of the femur by fibroblastic tissue and edema without detectable trabecular bone impairment. Classical radiographic, CT, and MRI abnormalities of osteonecrosis of the femoral head were present 14 months later. This report suggested that diffuse femoral head and neck medullary impairment with fat cell replacement by fib

Neck metaphyseal angle guidance in proximal femoral varus osteotomy.

An angle within the femoral arch is defined, and its line is described in selecting the osteotomy site and the position of the fixation device in performing a femoral varus osteotomy. This neck metaphyseal angle was measured with varying degrees of rotation on seven pediatric femurs; it was also studied on 283 randomly selected hip radiographs. The angle varied 25-40 degrees in 90% of the patients and was not age related. The angle is stable within 5 degrees as long as the landmarks, base of the great trochanter, and medial metaphysis of the femoral neck are definable (0-30 degrees of rotation

Femoral nerve block for analgesia in fractures of the femoral neck.

The purpose of this study was to investigate the efficacy of femoral nerve blocks in patients with fractures of the neck of the femur. Thirty-six patients with femoral neck fractures attending the accident department over a three month period received femoral blocks from one of the two authors. The effect of the block was assessed both subjectively and objectively. Thirty patients reported a worthwhile reduction in pain following the procedure and this was supported by objective blockade in 29. In all six patients who reported no benefit, the block failed objectively. There were no complicatio

Significance of endogenic factors in the location of fractures of the proximal femur.

The significance of endogenic factors in the localization of the fractures of the proximal femur was studied in 373 patients, with 402 fractures. A statistically significant relation was found between muscular disturbances of the affected leg and trochanteric fractures, while the incidence of femoral neck fractures was high in osteoporotic but physically active individuals.

'Tumour-induced osteomalacia'.

A 60-year-old man presented 2 years before his diagnosis with long-standing muscle cramping, progressive generalised weakness and chronic hip pain. The patient was found to have bilateral femoral neck pathologic fractures therefore, underwent reamed intramedullary nailing of both femurs. Laboratory studies showed hypophosphataemia. Bone marrow biopsy was negative for malignancy. Positron emission tomography demonstrated fludeoxyglucose uptake only in the posterior neck. Bone scan showed innumerable foci of increased activity throughout the skeleton consistent with pseudofractures seen in osteo

Effect of cortical bone micro-structure in fragility fracture patients on lamellar stress.

This work investigates how changes in cortical bone microstructure alter the risk of fragility fractures. The secondary osteons of non-osteoporotic (by DXA) women with fragility fractures have reduced lamellar width and greater areas of birefringent brightness in transverse sections, a pathological condition. We used hierarchical finite element (FE) models of the proximal femur of two women aged 67 and 88 (younger and older) during one-legged stance. At specific locations of the anterior-inferior neck (ROI), we analyzed micro-models containing osteons comprised of alternating birefringent exti

The relationship between plasma homocysteine levels and bone mineral density in post-menopausal women.

BACKGROUND: Whether or not mild hyperhomocysteinemia and low serum levels of folates or vitamin B12 are risk factors for osteoporosis in the elderly is controversial. AIMS AND METHODS: To investigate whether or not plasma levels of total homocysteine (tHcy) and serum levels of folates and vitamin B12 are associated with bone mineral density (BMD), we carried out a cross-sectional study on 446 post-menopausal women (mean age: 65.1+/-9.4 years), consecutively seen at the Siena Unit (Tuscany region, Central Italy) for BMD evaluation over a two-year period. Alkaptonuric arthropathy with periprosthetic fracture:a case report.

Alkaptonuria or Ochronosis is an autosomal recessive congenital metabolic error due to absence of enzyme homogentisic acid oxidase. Recently, we had an opportunity to diagnose this condition in one of our patients. An old aged man developed a pathological fracture of femur after a trivial fall at home. He was diagnosed to have intracapsular fracture neck of femur. Intraoperatively we noticed blackish discoloration of femoral head which lead us to investigate the case for alkaptonuria. Hemiarthroplasty was done with Austin moore prosthesis. Patient had cutaneous pigmentation and gross arthritic

Cementless anatomic total hip femoral component with circumferential porous coating for hips with developmental dysplasia: a minimum ten-year follow-up period.

Anatomic femoral components are optimized for primary osteoarthritis, and few long-term results for their use are available for developmental dysplasia of the hip (DDH). We reviewed 135 uncemented total hip arthroplasties in which an anatomic femoral component was used in 106 patients with DDH, excluding femurs with neck-shaft angles of >160° and femurs with anteversion of >50°. The mean age of patients at surgery was 49.4 years (range, 33-66 years), and the mean duration of follow-up was 13.5 years (range, 10-18 years). No osteoporotic femurs were included in our study group. Despite 18 aceta
