OBJECTIVE: This retrospective review of surgically treated distal tibia fractures was undertaken to determine whether treatment with open reduction and internal fixation (ORIF) was more efficacious in achieving fracture union than one of two external fixation methods.
METHODS: Of the 60 study patients with pilon fractures, 21 patients were treated with an ankle-spanning half-pin external fixator, 15 patients with a single-ring hybrid external fixator, and 24 patients with ORIF. The severity of injuries was similar across groups.

The incidence and nature of stress fractures and the relationship of potential risk factors to stress-fracture history were investigated retrospectively in a group of 53 female competitive track-and-field athletes. Forty-five stress fractures, diagnosed by clinical findings and bone scan, radiograph, or CT scan, were reported in 22 women. Tibial fractures were the most common (33%). One thousand consecutive closed diaphyseal tibial fractures, treated with prefabricated functional below-knee braces, were analyzed by statistical methods to determine factors predictive of final fracture outcome. Motor-vehicle accidents often result in lower limb injuries with biosseous fractures. The present study aimed at comparing multi-slice computed tomography (MS-CT), micro-computed tomography (micro-CT) and external fractography for the analyses of experimentally produced biosseus leg fractures. Briefly, 48 human legs amputated for medical reasons were defleshed and then experimentally fractured using a 3-point dynamic bending model (70,6 J of impact energy at the middle of the anterior surface of the tibia) producing 38 biosseous and 10 mono-osseous fractures with a total of 86 fractured bones.

We report a case of fulminant fat embolism syndrome treated with high frequency oscillatory ventilation (HFOV). Calcific myonecrosis is a rare post-traumatic sequel of the lower limb, presenting in a delayed manner several years after the initial injury. We report a case of a 43-year-old gentleman who presented emergently with an enlarging painful mass in the lateral side of his left leg, that although present for several years had recently worsened with respect to the pain and erythema. Following laboratory and radiological investigations this was diagnosed to be calcific myonecrosis, which was then treated conservatively, and patient discharged when symptomatically improved.

BACKGROUND: Congenital pseudarthrosis of the tibia (CPT) is one of the most challenging conditions in pediatric orthopedics. Multiple objectives must be achieved to correct this condition successfully, including obtaining union, prevention of refracture, management of limb-length discrepancy (LLD), and correction of deformities of the ankle and leg. This combined treatment conjoins the improved union rate and improved limb-length equalization of Ilizarov fixation and the controlled alignment and refracture prevention associated within tramedullary nailing.

The authors discuss the aetiology, diagnosis and therapy of false aneurysms and mention their possible occurrence at different sites, whereby they emphasize the rare occurrence of pseudoaneurysms in conjunction with fractures of the bones of the extremities.
