Economic evaluation of hearing aid use and quality of life in older adults with hearing impairment in India.

Hearing impairment is a growing public health challenge among older adults in India, with low uptake of hearing aids despite potential benefits for communication, social participation, and health-related quality of life (HRQoL). This study evaluated the cost-utility of hearing aid provision for older adults (≥60 years) with hearing impairment in India to inform policy and financing decisions. A cost-utility analysis compared hearing aid use with no hearing aid over a 3-year decision tree and a 15-year Markov model. Data were collected in 2023 from 636 participants (276 users, 360 non-users). H

Accidents and trauma care--1983.

Except for categoric grants, the federal government has divested itself of financing emergency medical services. Now the leadership for regional planning of emergency medical services must come from the state, usually from the health department. If we are to obtain improved hospital care of trauma patients, it is critical that we separate hospitals that have made a genuine commitment to the care of the multiply injured patient from hospitals that have not in order to avoid the tragedy of patients being delivered to hospitals that have inadequate resources or commitment or organization to meet

Health system and environmental factors affecting global progress towards achieving End TB targets between 2015 and 2020.

BACKGROUND: Health system and environmental factors play a significant role in achieving the World Health Organization (WHO) End Tuberculosis (TB) targets. However, quantitative measures are scarce or non-existent at a global level. We aimed to measure the progress made towards meeting the global End TB milestones from 2015 to 2020 and identify health system and environmental factors contributing to the success. METHODS: We obtained data from ten different online data repositories and used principal component analysis to create domain-specific health system performance measures. We used radar

Integrating oral health into planetary health governance: a framework for resilient and equitable oral healthcare.

Oral health is largely absent from climate-adaptation and planetary-health governance, even though oral health and oral healthcare depend on supplies of clean water, stable electricity, secure supply chains, and a retained workforce, which climate change is destabilizing. In this Perspective, we argue that this omission is a strategic failure for health security, and draw on the contrasting experiences of seven climate-exposed settings (Australia, Fiji, Indonesia, New Zealand, the Solomon Islands, South Africa, and the United Kingdom) to show how environmental stressors disrupt progress in pro

Africa's unpaid outbreak frontline workers: The political economy of community health worker neglect.

Community health workers (CHWs) in Africa perform indispensable functions across the outbreak response cycle. These include early case detection, contact tracing, risk communication and zoonotic interface monitoring. They depend on community embeddedness, an asset the formal health system cannot replicate in emergencies. Yet despite a substantial evidence base, more than 80% of Africa's 1.4 million CHWs operate under volunteerism frameworks, without formal employment, salaries or enforceable protections. This is not a consequence of resource scarcity. It is a deliberate programme design choice

Antimicrobial resistance and social inequity: Why the one health approach must centre poverty.

Antimicrobial resistance (AMR) is one of the most consequential threats to global public health. In 2021, bacterial AMR was associated with an estimated 4.71 million deaths globally, including 1.14 million directly attributable to resistance. Yet, despite its designation as a global emergency, policy responses remain heavily focused on surveillance, antimicrobial stewardship, diagnostics, and pharmaceutical innovation. These interventions are necessary but insufficient without sustained action on the social and structural conditions shaping infection exposure, access to care, and antibiotic us

Breast cancer early detection: A phased approach to implementation.

When breast cancer is detected and treated early, the chances of survival are very high. However, women in many settings face complex barriers to early detection, including social, economic, geographic, and other interrelated factors, which can limit their access to timely, affordable, and effective breast health care services. Previously, the Breast Health Global Initiative (BHGI) developed resource-stratified guidelines for the early detection and diagnosis of breast cancer. In this consensus article from the sixth BHGI Global Summit held in October 2018, the authors describe phases of early

Hong Kong Chinese aging adults voice financial care expectations in changing family and sociocultural contexts: implications for policy and services.

Despite being one of the world's wealthiest cities, approximately one-third of Hong Kong older adults live below the poverty line. Innovatively using the Photovoice research method, this study invited 36 Hong Kong Chinese aging adults to photograph images and voice their concerns and expectations regarding financial care. Insufficient government support, diminishing family support, insecurity and fear regarding future finances, and strong desire for self-sufficiency through early preparation and bridge employment were recurring themes observed in the participants' photographs and narratives. Epidemiology and prevention of arterial hypertension in Poland.

The authors review the present situation in epidemiology and prevention of arterial hypertension in Poland. In 2002, the NATPOL PLUS survey on representative sample of adults (n=3051, age range 18-93) was conducted. Prevalence of hypertension, diagnosed on basis of three separate visits, was 29%, awareness 67% and efficacy of treatment 12.5%. Thus, in Poland, one-third of 8.6 million hypertensives are unaware of their disease. A comparison with data from other countries should be careful due to the different methods (age range, number of readings and visits) used in the studies. The data, in c

Factors influencing the institutionalization of digital pharmacovigilance tools in LMICs: a scoping review using the non-adoption, abandonment, scale-up, spread, and sustainability framework.

BACKGROUND: Digital tools for pharmacovigilance (PV) have been widely promoted to strengthen adverse drug reaction (ADR) reporting in low- and middle-income countries (LMICs). Despite growing evidence on the implementation of digital pharmacovigilance tools, limited evidence has examined the factors influencing their institutionalization in LMICs. This scoping review examines the institutionalization of digital PV tools in LMICs and identifies factors shaping their long-term integration. METHODS: A scoping review was conducted following PRISMA-ScR guidelines. 2 reviewers independently screene

Vaccine-Preventable Disease Control in the WHO African Region After the COVID-19 Public Health Emergency of International Concern: Implications for Recovery, Resilience, and System Transformation.

BACKGROUND: The end of the COVID-19 public health emergency of international concern (PHEIC) in May 2023 marked a transition from disruption to recovery and rebuilding of health systems. The WHO African Region entered this period with declining routine immunization coverage, widening inequities, and fragile surveillance systems. We conducted a critical narrative synthesis of post-PHEIC recovery and the transformation of immunization systems in the region from 2023 to 2025. METHODS: We thematically analyzed publicly available data from the WHO and other sources using a systems-oriented framewo

The path from a volunteer initiative to an established institution: evaluating 15 years of the development and contribution of the Lighthouse trust to the Malawian HIV response.

BACKGROUND: The HIV epidemic has triggered the development of new health institutions with a special focus on HIV care. The role of these relatively new institutions within the health systems of low-income countries like Malawi is not clearly determined. We evaluate and describe the development of one example, the Lighthouse Trust (Lighthouse), over a period of 15 years (2000-2015). METHODS: Data from multiple sources, including a document review, participatory observation and interviews were analysed, triangulated and synthesized. The institution's development, function, cooperation, financi
