Two cases are described of anterior dislocation of the proximal interphalangeal joint which could not be reduced because of interposition of the central slip of the extensor mechanism. In common with previously reported cases the injuries were sustained in a spin drier. The literature on anterior dislocation of the proximal interphalangeal joint is reviewed. Three distinct patterns of dislocation are recognised and their management is discussed.

The use of Fowler's central slip release is reported in five patients considered to be failures of closed management in a personal series of 100 consecutive "mallet fingers" seen over a period of 3 years. Fracture-subluxation of the distal interphalangeal joint, avulsion fractures of the extensor tendon, and distal phalangeal epiphyseal injuries are excluded to regidly control the data interpretation. With the help of thick transparent cross-sections of fingers, manufactured by plastination histology, it has been shown that fibres of the extensor aponeurosis insert not only the way described in current textbook on the basis phalangis distalis and on the capsule of the distal interphalangeal joint. Our studies prove that parts of the dorsal extension plate insert dorsally to the nail matrix and others run over the edge of the basis phalangis to the proximal part of the diaphysis where they intermingle with the periost of the phalanx.

Carpometacarpal (CMC) joint dislocations without fractures are rare and often missed due to subtle clinical and radiographic signs. We report a case of a 28-year-old male patient involved in a motorbike rollover who sustained open comminuted radius and ulna fractures, an ulnar styloid fracture, and dorsal dislocation of multiple CMC joints without metacarpal fractures. OBJECTIVE: To evaluate the interobserver reliability among 14 experts in musculoskeletal ultrasonography (US) and to determine the overall agreement about the US results compared with magnetic resonance imaging (MRI), which served as the imaging "gold standard".
METHODS: The clinically dominant joint regions (shoulder, knee, ankle/toe, wrist/finger) of four patients with inflammatory rheumatic diseases were ultrasonographically examined by 14 experts.
