Intravascular fasciitis is a very unusual variant of nodular fasciitis. A unique case of this lesion occurring in the proximal portion of the superficial vein of the forearm in an otherwise healthy 26-year-old man is reported. The intravascular polypoid lesion grew longitudinally along the vascular lumen, was loosely attached to the intimal layer, and was partly anchored beyond the internal elastic lamina into the medial smooth muscle layer. The mode of treatment used in 6 cases of combined supracondylar fracture of the humerus and ipsilateral forearm fracture is presented. Results were rated good, fair and poor according to both functional and radiological assessments. Three cases were good and 3 fair. One stage reduction of both fractures with plaster of Paris immobilization or olecranon traction has been performed. The results achieved are corresponding with those presented in the bibliography.

A reversed radial forearm fascial flap has been used as a vascularized tube through which tendon grafts may glide and in which nerve grafts may rest. It may have applications in the treatment of the severely damaged flexor system at wrist level. Thirty-five patients who had multiple cartilaginous exostosis and deformities of the wrist and forearm were evaluated to determine the natural history and prognostic factors for the deformities. The skeleton of a 35- to 40-year-old male (A.D. 1250 to 1300) with distal phocomelia (intercalary transverse) of the right forearm and severe (90-100 degrees) right-sided scoliosis is examined. Congenital malformation of the the right forearm resulted in absence of the radius and ulna, and attachment of the upper arm and hand to the trunk. Lesser abnormalities consist of a-hypoplastic right scapula and misshapened ribs and sternum. This rare deformity reflects the variability and antiquity of congenital malformations of the upper limb.

Six patients with extensive hand and forearm thromboembolic disease were treated by means of intraarterial infusion of urokinase, with good clinical results. Four significant complications occurred, including a possible stroke. Embolization of pericatheter thrombus was a possible etiologic factor in this case. Antegrade brachial artery puncture should be used in the setting of prolonged upper extremity thrombolytic therapy to avoid the cerebral vasculature. Thrombolysis is an effective technique for tissue salvage in cases of inoperable hand thrombosis.

Use of the island radial forearm flap (RFF) for soft-tissue coverage of hand and forearm following mutilating injuries, chemotherapeutic injection sloughs, and tumor excisions are discussed. Twenty-eight flaps were used in 28 injured upper extremities. Partial flap loss occurred in three patients. Minor sloughing of the skin graft of the donor site occurred in four. Open fractures of both left ulna and radius, in which a segment of the radius was found to be missing, occurred in a 27-year-old man. The fractures were treated by open reduction, internal fixation with ASIF semitubular plates, and bone graft. Because of slow radiological union, regrafting was required twice. Approximately six months after the third operation, roentgenograms showed that the bone plate was broken, even though the fractures had been protected in a long-arm cast.
