Radial Head Incarceration After Closed Reduction of a Pediatric Elbow Dislocation With a Radial Neck Fracture: A Case Report.

CASE: A 10-year-old girl sustained a radial neck fracture with a posterior elbow dislocation. She was treated with closed reduction of the elbow with subsequent intra-articular displacement of the radial head, which necessitated open reduction and pinning of the radial neck fracture. CONCLUSION: Displacement of the radial neck fracture from impingement of the capitellum on the anterior radial head during closed reduction of the elbow dislocation is a rare injury pattern. It is important to examine the radial neck in high-energy posterior elbow dislocations before attempted reduction. We prese

Challenges in closing the gap between evidence and practice: International survey of institutional surgical stabilization of rib fractures guidelines.

BACKGROUND: Surgical stabilization of rib fractures (SSRF) has gained increasing interest over the past decade, yet few candidates who could benefit from SSRF undergo operative management. We conducted an international survey of institutional SSRF guidelines comparing congruence between practice and contemporary evidence. We hypothesized that few guidelines reflect comprehensive evidence to facilitate standardized patient selection, operation, and postoperative management. METHODS: A request for institutional rib fracture guidelines was distributed from the Chest Wall Injury Society. Surgical

Preliminary report in treatment of proximal humeral fracture with closed reduction and DOS external fixation System: a multicentric study.

INTRODUCTION:: Proximal humerus fractures are the seventh most frequent fracture in adults, and the third in patients over 65 years old, 5.7% of whole diagnosed fractures. Most of these fractures can be treated conservatively and achieve good results. However, more and more frequently we are confronted with dislocated and multifragmentary fractures, and with elderly and high functional demanding patients. In patients with osteoporosis and poor general conditions external fixation can be performed as rapid and mininvasive procedure with good outcome and low complication rates. The authors inves

The Use of the Ilizarov Fixator for the Treatment of Open and Closed Tibial Shaft and Distal Tibial Fractures in Patients with Complex Cases.

INTRODUCTION: Open and closed fractures of the tibial shaft or distal tibia can be challenging for surgeons to treat if the fractures are accompanied by aggravating conditions, such as various accompanying diseases, pronounced soft tissue injuries, osteomyelitis, and/or noncompliance. The aim of this retrospective study was to present our approach and results with the Ilizarov fixator as a treatment option for such individually complex cases. MATERIALS AND METHODS: Between 2005 and 2018, 20 patients were treated with the Ilizarov fixator for fractures of the tibial shaft/distal tibia. The ind

The Effect of a Local Injection of Flurbiprofen Ester Microspheres on Systemic Inflammatory Model Rats With a Closed Femoral Shaft Fracture.

Background: Periarticular injections with a combination of local anesthetics, non-steroidal anti-inflammatory analgesics (NSAIDs), and epinephrine are becoming increasingly popular in the perioperative analgesia of artificial joint replacement. However, data on the efficacy and safety of local injection NSAIDs are still scarce. The purpose of this study was to investigate the efficacy and safety of a local injection of Flurbiprofen Ester Lipid microspheres into the inflammatory model of femoral shaft closed fractures in rats. Methods: A systemic inflammatory model was induced in SD rats (60) b

Open vs Closed Management of Condylar Fracture Our Experience of 100 Cases in a Suburban Tertiary Care Hospital.

STUDY DESIGN: Retrospective Observational Study. OBJECTIVE: Mandibular condyle fractures are distinctive among maxillofacial injuries in which they disrupt mandibular function in a way that other traumatic injuries do not. Condylar fractures can be treated using either the conservative (closed reduction and immobilisation) or surgical (open reduction and internal fixation) approaches. Both of these modalities of treatment have advantages and disadvantages, as well as indications and contraindications. The purpose of this study is to compile and compare our experience in the management of cond

Successful Closed Reduction of Distal Radius Volar Locking Plate Plastic Deformation with Associated Fracture: A Case Report.

CASE: Distal radius fractures are among the most common orthopaedic injuries. The volar locking plate is now the most utilized internal fixation method for these fractures. There are few reports of mechanical failure of volar locking plates within the literature. In this case report, we describe successful closed reduction of traumatic volar locking plate plastic deformation with associated fracture. In addition, we discuss existing literature regarding plastic deformation of distal radius plates. CONCLUSION: Traumatic, plastic deformation of distal radius volar locking plates with associated

Occlusion Outside the Norm: A Unique Case of the Pulseless, Well-Perfused Hand Following Closed Humeral Shaft Fracture.

Transient arterial injury is a common complication encountered in the management of pediatric supracondylar humerus fractures, often presenting as a pulseless hand that appears well perfused, also known as the "pink, pulseless hand." Arterial injuries in pediatric humeral shaft fractures, on the other hand, are exceedingly rare, especially in closed fractures. The authors report a rare case of a humeral shaft fracture presenting as a pink, pulseless hand. Although this complication was initially managed according to supracondylar protocol, the patient's neurologic status deteriorated. This pro

Failed closed reduction of a bifocal mandibular fracture because of dislocation of the mandibular ramus behind the styloid: case report.

We present an unusual dislocation of the mandibular ramus after a low condylar fracture associated with a fracture of the opposite body of mandible. As closed reduction failed, both fracture sites were exposed. The mandibular ramus was dislocated behind the styloid process, which was not shown on the preoperative radiographs. The intermediate fragment was freed and the fractures treated by miniplate osteosynthesis. The patient made an uneventful recovery with no residual deformity.

Closing the loop on tourniquet safety in distal radius fracture fixation surgery: a two-cycle quality improvement project.

BACKGROUND: The application of intraoperative tourniquets is a fundamental component of distal radius fracture surgery, yet it carries a risk of significant complications. The British Orthopaedic Association (BOA) has published safety guidelines (BOA Standards for Trauma (BOAST)) to standardise practice, but adherence is often suboptimal. This quality improvement project aimed to evaluate and improve compliance with these guidelines in a real-world clinical setting. METHODS: A two-cycle retrospective audit was conducted at a major trauma centre in the UK. The first cycle (January to April 202

Femoral malrotation after trochanteric fracture nailing: what is the safety zone of limb rotation during closed reduction?

INTRODUCTION: Improper closed trochanteric fracture reduction can cause rotational malposition which can lead to serious consequences. The primary objective of this study was to assess the hypothetical threshold at which excessive rotation becomes hazardous due to a significant postoperative malrotation. MATERIALS AND METHODS: We conducted a prospective study focused on closed reduction in intramedullary nailing of trochanteric fractures (AO 31A1-3) in 100 consecutive patients and its influence on final malrotation. Immediately after the closed reduction, the rotation of affected limb was mea

Acute Haemophilus influenzae infection complicating a closed humeral shaft fracture in a pregnant young female: A case report.

BACKGROUND: Humeral shaft fractures are common in young adults following high-energy trauma. While open fractures often result in infections, infections in closed fractures are rare, making such cases particularly challenging to manage when they occur. CASE PRESENTATION: We report a unique case of a 25-year-old pregnant female who sustained a closed humeral shaft fracture after a high-energy injury. Although there was no initial open wound, a purulent infection was discovered at the fracture site during surgery a few days after the injury. Cultures identified Haemophilus influenzae as the cau

Irreducible Tibial Pilon Fracture Treated with Single-Stage Closed Ilizarov Fixation and Gradual Correction: A Case Report.

CASE: A young polytrauma victim of a road traffic accident was diagnosed with comminuted articular fracture of the left distal tibial pilon and open segmental fracture of the fibula. The comminuted distal tibial metaphysis was translocated into the interosseous space and, hence, was irreducible. He underwent single-stage closed Ilizarov device stabilization and gradual postoperative closed reduction to obtain good functional outcome at 9 months. The lung and scapular injuries were treated conservatively. CONCLUSION: Acute complex irreducible tibial pilon fracture can be treated according to p

Finite element analysis of medial closing and lateral opening wedge osteotomies of the distal femur in relation to hinge fractures.

PURPOSE: Intraoperative hinge fractures in distal femur osteotomies represent a risk factor for loss of alignment and non-union. Using finite element analysis, the goal of this study was to investigate the influence of different hinge widths and osteotomy corrections on hinge fractures in medial closed-wedge and lateral open-wedge distal femur osteotomies. METHODS: The hinge was located at the proximal margin of adductor tubercle for biplanar lateral open-wedge and at the upper border of the lateral femoral condyle for biplanar medial closed-wedge distal femur osteotomies, corresponding to op
