Percutaneous fixation in clubfoot surgery: a radiographic and gait study.

Twenty patients with Type II clubfeet were evaluated an average of 3.1 years after complete surgical soft tissue release. A pinned group had … ed than for the non-pinned group, but the duration of surgery was similar. Similar radiographic and gait measurement findings suggest that clubfoot surgery with and without percutaneous fixation have comparable outcomes.

An examination of the knee function during gait in children with myelomeningocele.

In this retrospective study, 37 patients with myelomeningocele who had undergone gait analysis were examined. Patients were divided into groups based on the level of involvement (29 sides: L4; 26 sides: L5; 19 sides: S1-2). Results showed increased knee flexion and associated knee extensor moments with increasing level of neurologic involvement. The mean coronal plane knee position in stance was normal in all groups and not related to coronal plane knee moment. However, there was an increased incidence of a net knee adductor moment in stance with increasing involvement (mean, 0.02 +/-0.18 N.m/…

Trunk kinematics in hemiplegic gait and the effect of walking aids.

OBJECTIVE: To establish baseline measurements of trunk movements during hemiplegic gait, to assess the relationship between trunk movements and walking ability, and to investigate the effect of walking aids on the trunk movements. METHOD: Twenty subjects with a chronic hemiplegia from a stroke, who could walk independently, were recruited. Lateral and vertical movements of the pelvis, and symmetry of these movements were measured using CODA (a three-dimensional movement analysis system) as the subjects walked at their own pace without an aid. They were also tested as they walked with a stick …

[A case of frontal gait apraxia caused by hypoxic encephalopathy].

We reported a rare case who had hypoxic-encephalopathy causing frontal apraxia of gait. The patient, a 34-year-old female, was admitted in July, 1994, complaining of difficulty in walking after anoxic brain damage caused by ventricular arrhythmia. She had difficulty in raising her feet, which appeared to be rooted to the floor. There was no evidence of motor paralysis, spasticity, rigidity or sensory loss, but she did show frontal lobe signs such as foot grasp reflex and Gegenhalten. Cranial MRI showed slight atrophy of the frontal lobe. On T2 weighted image, high-intensity areas were detected…

Astasia and gait failure with damage of the pontomesencephalic locomotor region.

Although there is evidence in experimental animals of the importance of the pedunculopontine area in locomotion, clinical lesion reports are lacking. An 83-year-old woman became unable to stand and to generate stepping movements after a hemorrhage at the pontomesencephalic junction involving the right pedunculopontine area. Her deficit resembled the gait failure present in some elderly individuals.

Hip function and gait in patients treated for bladder exstrophy.

Patients with bladder exstrophy demonstrate pubic diastasis with external rotation of the ilia and posterolateral orientation of the acetabula. Iliac osteotomy is performed to approximate the pubis and thus facilitate the genitourinary repair. The role of iliac osteotomy in the correction of gait abnormalities is controversial. Our goals in this study were to define the presence of hip pathology and gait abnormalities in patients treated for bladder exstrophy and to evaluate the effect of iliac osteotomy. We reviewed pelvic radiographs and gait studies in 15 children. The treatment included bi…

The Rancho EMG analyzer: a computerized system for gait analysis.

This paper describes a computer system which accurately defines the EMG patterns of the lower extremities during gait. Footswitches are used to identify the temporal relationships and determine the phases of the gait cycle. Fine wire electrodes, inserted in the desired muscles of the patient being tested, provide EMG signals for comparison with a normal database. The system is also usable with surface electrodes when an appropriate normal database for surface electrodes is incorporated. Descriptive qualifiers (such as 'premature onset', 'delayed cessation', 'no clinically significant EMG', 'co…

Correlation of creatine kinase and gait measurement in the postpolio population.

Measurements of stride length, gait speed, and distance walked during seven days were obtained from 15 postpolio and eight control subjects. Pedometers were used to measure distance walked. Measurements of stride length and speed were performed three times, and there was a high correlation between tests (R = .852-.969). The pedometers failed to record accurately in some postpolio subjects, and these subjects were dropped from analysis when ambulation distance was used as a variable. There were significant differences between the postpolio subjects and controls with respect to gait speed (47.7 …

The role of the contralateral limb in below-knee amputee gait.

Very little quantitative biomechanical research has been carried out evaluating issues relevant to prosthetic management. The literature available suggests that amputees may demonstrate an asymmetrical gait pattern. Furthermore, studies suggest that the forces occurring during amputee gait may be unequally distributed between the contralateral and prosthetic lower limbs. This study investigates the role of the contralateral limb in amputee gait by determining lower limb joint reaction forces and symmetry of motion in an amputee and non-amputee population. Seven adult below-knee amputees and fo…

Gait patterns in spastic hemiplegia in children and young adults.

Four homogeneous patterns of gait were defined in forty-six patients who had spastic hemiplegia secondary to cerebral palsy or other neurological disorders by analyzing kinematic data in the sagittal plane and electromyographic data. In Group I (twenty patients) the primary abnormality was a drop foot in the swing phase. The thirteen patients in Group II had a tight heel cord in the stance phase as well as a drop foot in the swing phase. The five patients in Group III also had more proximal involvement (that is, restricted motion of the knee) as well as an equinus deformity of the ankle. In Gr…

Clinical gait assessment in the neurologically impaired. Reliability and meaningfulness.

This study of subjects with multiple sclerosis or hemiparesis assessed the interrater and test-retest reliability of temporal-distance (TD) gait measures and examined the relationship of TD values to functional ambulation ability. Sixty-one subjects ambulated twice on a 9-m (30-ft) paper walkway and rested 15 minutes between trials. An ink footprint record and ambulation time were used to calculate velocity, cadence, step and stride lengths, stride length to lower extremity length ratio, and step- and stride-time differentials. Subjects were rated on a scale that assessed the amount of manual …

Variation of mechanical energy levels for normal and prosthetic gait.

Mechanical energy levels were investigated for normals and for below-knee amputees during level walking. The weight of the prostheses was varied by attaching 0.5 kg extra weight to the prostheses. The measurements and analyses were made with the ENOCH system consisting of a minicomputer (HP 21 MX), an optoelectronic device for displacement data measurement (Selspot) and a force plate (Kistler) for measurement of ground reaction forces. Results by Winter et al (1976) on the energy changes during normal walking obtained from displacement data on one leg only were verified using data from both le

Gait in childhood and adulthood in persons with myelomeningocele - a retrospective analysis.

BACKGROUND: Gait patterns in children with myelomeningocele (MMC) at various neurological levels have been described, both with and without orthotic support. Although the neurological level of the lesion serves as an important predictor of ambulatory potential, the expected walking ability is not always achieved, as additional factors such as spasticity may influence gait negatively. The aim of this study was to retrospectively compare gait patterns as assessed in childhood with those observed in adulthood. METHODS: Of 59 individuals with MMC aged 18 years or older, 29 had undergone three-dim…

Correlations of gait kinematics and cognitive skills in Parkinson disease.

Cognitive impairments and gait disturbances are often concurrent in Parkinson disease (PD), but the extent to which specific cognitive deficits relate to gait abnormalities remains unclear, especially in early-stage PD. To address this gap, we conducted an observational correlational study to determine if cognitive performance in distinct domains is associated with gait kinematics in PD. This study included a cohort of 19 individuals with early-stage PD who underwent gait analysis with a three-dimensional marker-less motion capture system (Theia Markerless Inc., Kingston, ON, CA) during three …

Neural and dopaminergic substrates of festinating gait in Parkinson's disease.

BACKGROUND: Festinating gait (FSG) is a typical feature of Parkinson's disease (PD). However, the mechanisms are poorly understood. We aimed to determine the neural substrates of FSG in PD and whether the festinating gait is related to striatal dopaminergic activity. METHODS: A total of 70 PD patients without FSG and 22 PD patients with FSG were recruited for voxel-based morphometry and diffusion tensor imaging analyses. Among the subjects, 55 PD patients without FSG and 13 PD patients with FSG were analyzed for striatal dopaminergic activity. The severity of FSG and freezing were evaluated u…
