Estimating Transition Probabilities for Modeling Major Depression in Adolescents by Sex and Race or Ethnicity Combinations in the USA.

OBJECTIVE: About one-fifth of US adolescents experienced major depressive symptoms, but few studies have examined longitudinal trends of adolescents developing depression or recovering by demographic factors. We estimated new transition probability inputs, and then used them in a simulation model to project the epidemiologic burden and trajectory of depression of diverse adolescents by sex and race or ethnicity combinations. METHODS: Transition probabilities were first derived using parametric survival analysis of data from the National Longitudinal Study of Adolescent to Adult Health and the

Barriers to medication abortion among Massachusetts public university students.

OBJECTIVE: Proposed legislation in Massachusetts would require public university health centers to provide medication abortion services on campus. This study assesses need for these services by investigating current travel time, costs, wait times and insurance acceptance at off-campus, abortion-providing facilities nearest to public universities in Massachusetts. STUDY DESIGN: This investigation projected the total number of students obtaining medication abortions each year at the 13 Massachusetts public universities based on campus enrollment figures and age- and state-adjusted medication ab

Cancer screening as a double-edged sword: short-term cost surge and cross-sectional expenditure patterns among urban retirees in Jiangsu, China.

BACKGROUND: The effectiveness of free cancer screening programs in reducing long-term healthcare expenditures remains debated, yet quasi-experimental evidence is scarce. This study evaluates the impact of a large-scale, government-led free cancer screening program, available to citizens aged 65 and above, on the medical expenditures of urban retirees in Jiangsu Province, China. METHODS: We employed a sharp regression discontinuity design (RDD). Our analysis included a cross-sectional sample of 1,503 retired individuals aged 60-70 from Jiangsu Province, China, who participated in free physical

Community willingness towards a hypothetical new TB vaccine: a qualitative study to explore the enablers and barriers for vaccine acceptability in Nepal.

INTRODUCTION: Tuberculosis (TB) remains a leading cause of death in low- and middle-income countries, including Nepal. Although new TB vaccines are in clinical development, little is known about public willingness to accept them. This study explores the enablers and barriers for public acceptance of a hypothetical new TB vaccine among communities in three TB-affected districts in Nepal. METHODS: We conducted an exploratory qualitative study in three TB-affected districts of Nepal- Morang, Banke and Mahottari. Six focus group discussions were conducted with 60 purposively selected participants

The global, regional, and national patterns of change in the burden of bacterial pyoderma from 1990 to 2019 and the forecast for the next decade.

Pyoderma, commonly known as impetigo, is a bacterial skin infection causing pus formation, prevalent globally, especially in resource-poor areas. It affects both children and adults, including those with conditions like diabetes. Despite its significant impact and economic burden, research on its global epidemiology is limited. This study aims to address this gap by analyzing pyoderma trends from 1990 to 2019 using GBD data. The study aims to analyze global trends in pyoderma epidemiology from 1990 to 2019 using GBD data. Specifically, it investigates Age-Standardized Incidence Rate (ASIR), Mo

Email recruitment for chronic pain clinical trials: results from the LAMP trial.

BACKGROUND: Recruitment for clinical trials and large-scale studies is challenging, especially for patients with complex conditions like chronic pain. Email recruitment has the potential to increase efficiency, to reduce costs, and to improve access for underrepresented patient populations. The objective of this study was to examine the effectiveness, efficiency, and equitability of email versus postal mail recruitment for the Learning to Apply Mindfulness to Pain (LAMP) study, a three-site clinical trial of mindfulness-based interventions for chronic pain. METHODS: Patients with chronic pain

Vasectomy provider decision-making balancing autonomy and non-maleficence: qualitative interviews with providers.

BACKGROUND: Male sterilization, or vasectomy, is 99.9% effective at preventing pregnancy with less than a 2% risk of complications. Despite the high efficacy, low risk, low cost, and gender equity benefits of vasectomy, just 2% of women reported that they and their partners relied on vasectomy as their contraceptive method globally in 2019. Health care providers can be both a facilitator and a barrier in men's health generally, and may be in vasectomy provision as well. This study sought to describe the decision-making rationales of experienced vasectomy providers when evaluating patient candi…

Large Language Models' Simulation of Patient Comprehension of Discharge Summaries: Multimodel Evaluation Study.

BACKGROUND: Large language models (LLMs) are increasingly used to generate and evaluate patient-facing medical materials. Through persona prompting, LLMs can be instructed to simulate responses from individuals with different demographic and experiential backgrounds, offering a scalable approach for approximating patient feedback. However, it remains unclear how closely these simulated persona-based responses reflect real patient perspectives, and whether this alignment is consistent across demographic subgroups. OBJECTIVE: This study assessed whether contemporary LLMs can simulate diverse pa

Geospatial analysis of COVID-19 vaccine access in Kenya: the interplay of travel time, perceived availability, and vaccine uptake.

BACKGROUND: COVID-19 vaccines have reduced severe illness and mortality, yet inequitable distribution delayed rollout in many low- and middle-income countries (LMICs), including Kenya, where only 36% of adults were fully vaccinated by October 2022. While sociodemographic and attitudinal determinants of uptake are well documented, role of geographic access remain underexplored at individual level. This study assessed population coverage by travel time to COVID-19 vaccination sites in Kenya and examined how travel time was associated with perceived vaccine availability and vaccine uptake. Factors affecting human papillomavirus vaccination: A review of individual, interpersonal, and structural factors affecting vaccine uptake among adolescents.

BACKGROUND: Human papillomavirus (HPV) related cancers, particularly cervical cancer, remain a major public health challenge in low- and lower-middle-income countries (LLMICs), which bear a disproportionate share of global HPV-attributable mortality. Despite the availability of safe and effective vaccines for nearly two decades, vaccination coverage remains low in LLMICs. Several barriers ranging from health misinformation to systemic delivery challenges have hindered efforts to scale up vaccination programs in these settings. OBJECTIVES: This systematic review aims to identify and synthesize
