Confirmation of the spinal motor neuron gene 2 (SMN2) copy numbers by real-time PCR.

Spinal muscular atrophy (SMA) is an autosomal recessive disease caused by mutation or deletion of the survival motor neuron gene 1 (SMN1). SMN2, a copy gene, influences the severity of SMA and may be used in somatic gene therapy of patients with SMA in the future. The SMA carrier analysis developed at the Institute of Medical Genetics, Catholic University (Rome), on the Applied Biosystems real-time PCR instruments by Dr Danilo Tiziano and his group, provides a robust workflow to evaluate SMA carrier status. In this study, the SMN2 copy number was confirmed on 22 patients by developing our own

Hereditary disorders of connective tissue: a guide to the emerging differential diagnosis.

PURPOSE: To create a practical desk reference for clinicians focused on the differential diagnosis of individuals presenting with features that suggest an inherited disorder of connective tissue. METHODS: We searched the medical literature for distinct clinical entities that shared clinical features with Marfan syndrome and other classical inherited disorders of connective tissue. RESULTS: Thirty-six distinct heritable disorders of connective tissue were identified that have overlapping features. These disorders were organized into two matrices according to clinical characteristics and accor

Management of dyslexia, its rationale, and underlying neurobiology.

Developmental dyslexia is characterized by an unexpected difficulty in reading in children and adults who otherwise possess the intelligence and motivation considered necessary for accurate and fluent reading. Dyslexia is the most common and most carefully studied of the learning disabilities, affecting 80% of all individuals identified as learning disabled. Although in the past the diagnosis and implications of dyslexia were often uncertain, recent advances in the knowledge of the epidemiology, the neurobiology, the genetics, and the cognitive influences on the disorder now allow the disorder

Pathology of Progressive Familial Intrahepatic Cholestasis: An update.

Progressive familial intrahepatic cholestasis (PFIC) comprises a heterogeneous group of rare, autosomal recessive liver disorders characterised by defective bile formation and secretion, leading to progressive cholestasis, pruritus, growth impairment, and early onset liver failure. Advances in molecular genetics have identified distinct causative pathogenic variants involving key hepatocellular transporters of bile salts, and phospholipids, giving rise to multiple PFIC subtypes. Early recognition is crucial, as timely medical management and surgical interventions may delay disease progression,

Advances in Medical Treatment of Primary Aldosteronism.

Primary aldosteronism (PA) is the most common form of secondary hypertension, with its main manifestations including hypertension and hypokalemia. Early identification of PA is extremely important as PA patients can easily develop cardiovascular complications such as atrial fibrillation, stroke, and myocardial infarction. The past decade has witnessed the rapid advances in the genetics of PA, which has shed new light on PA treatment. While surgery is the first choice for unilateral diseases, bilateral lesions can be treated with mineralocorticoid receptor antagonists (MRAs). The next-generatio

Multisystemic Manifestations in Rare Diseases: The Experience of Dyskeratosis Congenita.

Dyskeratosis congenital (DC) is the first genetic syndrome described among telomeropathies. Its classical phenotype is characterized by the mucocutaneous triad of reticulated pigmentation of skin lace, nail dystrophy and oral leukoplakia. The clinical presentation, however, is heterogeneous and serious clinical complications include bone marrow failure, hematological and solid tumors. It may also involve immunodeficiencies, dental, pulmonary and liver disorders, and other minor complication. Dyskeratosis congenita shows marked genetic heterogeneity, as at least 14 genes are responsible for the

SATB2-associated syndrome: Mechanisms, phenotype, and practical recommendations.

The SATB2-associated syndrome is a recently described syndrome characterized by developmental delay/intellectual disability with absent or limited speech development, craniofacial abnormalities, behavioral problems, dysmorphic features, and palatal and dental abnormalities. Alterations of the SATB2 gene can result from a variety of different mechanisms that include contiguous deletions, intragenic deletions and duplications, translocations with secondary gene disruption, and point mutations. The multisystemic nature of this syndrome demands a multisystemic approach and we propose evaluation an

Botanicals in Dermatology: Essential Oils, Botanical Allergens, and Current Regulatory Practices.

Largely because of their perceived safety, the use of essential oils and other botanically derived products has become increasingly popular. Recent evidence raises concern about the safety of these products, frequently found in cosmetics and sought as an alternative to standard medical treatments. Essential oils are challenging to standardize because of the variable growing conditions, genetics, and harvesting of botanicals. There exists a potential for adverse reactions, in particular allergic contact dermatitis. Furthermore, these products are often sold without prior Food and Drug Administr

Troubling "environments": postgenomics, Bajan wheezing, and Lévi-Strauss.

Postgenomics is intended to move beyond the search for genes to explore disease as a result of genes interacting with their environment, revealing how they have relevance for health. This addition of environment confers genomic research with new cultural life, making it relevant to public health discourse, government interventions, and health disparities. Drawing on ethnographic research following an American genetics of asthma study conducted in Barbados, I explore the ways environment gets construed by the multiple communities involved-U.S. researchers, Bajan officials, medical practitioners

Movement disorders in women: a review.

The field of women's health developed based on the recognition that there are important sex-based differences regarding several aspects of medical illnesses. We performed a literature review to obtain information about differences between women and men for neurological movement disorders. We identified important differences in prevalence, genetics, clinical expression, course, and treatment responses. In addition, we found that female life events, including menstruation, pregnancy, breast feeding, menopause, and medications prescribed to women (such as oral contraceptives and hormone-replaceme

Molecular cytogenetic aspects of hematological malignancies: clinical implications.

The field of molecular cytogenetics has had a great impact on many aspects of medical and basic sciences. During the past 30 years, the application of molecular cytogenetic methodologies has resulted in remarkable advances in the field of cancer genetics and cytogenetics. These advances have led to the establishment of chromosome patterns as diagnostic and prognostic indexes in an array of acute and chronic leukemias and lymphomas, as key information in BMT, and as guides for the localization of oncogenes and tumor suppressor genes that are apparently responsible for the development of neoplas

Autosomal dominant polycystic kidney disease - clinical and genetic aspects.

Autosomal dominant polycystic kidney disease (ADPKD) is one of the most common inherited disorders in humans. It accounts for 8-10% of the cases of end-stage renal disease worldwide, thus representing a serious medical, economical and social problem. ADPKD is in fact a systemic disorder, characterized with the development of cysts in the ductal organs (mainly the kidneys and the liver), also with gastrointestinal and cardiovascular abnormalities. In the last decade there was significant progress in uncovering the genetic foundations and in understanding of the pathogenic mechanisms leading to

[The individual with the diagnosis of a genetic condition as a key informant in the field of rare diseases - a perspective from the sociology of diagnosis].

Adopting a different viewpoint from most of the work in the field of so-called rare diseases, this paper crosses the boundaries of the associations to reach people living with the diagnosis of a genetic condition, which is understood as being a rare disease, namely neurofibromatosis (NF). In this respect, the incipient Sociology of Diagnosis is utilized to identify both the impact and the consequences of the diagnosis in people's lives. As a result, the consensus is that it is necessary to transcend the charitable outlook on people who experience the diagnosis of a genetic condition, by percei

Childhood vitiligo.

Vitiligo is a common skin disorder in which pigment is lost from the skin and adnexae. Half of all cases begin before the age of 20 years. Until recently, there was a paucity of published data regarding effective therapy of vitiligo. Current therapy is quite effective, and while many patients may not experience complete repigmentation, 50% to 75% repigmentation of vitiligo with recent onset routinely can be expected with available therapeutics.

[Genomic investigations in genetic cardiology: status and prospects].

The paper discusses relations of new disciplines in medical science, such as genetic cardiology and genomic medicine. It outlines today's achievements and prospects of genomic studies for some groups of cardiovascular diseases: congenital developmental defects of the heart and great vessels, cardiac arrhythmias and cardiomyopathies, and common diseases with complex inheritance, such as atherosclerosis and arterial hypertension. Accumulated knowledge on new candidate genes of these forms of pathology specifies their pathogenesis, makes it possible to outline new gene-specific therapeutical appr

[The human genome--chromosome 16].

Haemoglobinopathies and thalassaemia caused by qualitative and quantitative disorders of the function of the alpha-chain of haemoglobin are associated with chromosome 16 on the short arms of which two of its loci are placed localized mapped. The majority of the human population has four active alpha-chain genes. Reduction of their number to two or less leads to increasing symptoms of their insufficiency manifested as alpha-thalassaemia--complete loss being lethal. On the 16th chromosome is the cause of the dominantly hereditary disease--polycytosis of the kidney so far inaccurately defined at
