The Geriatric Pain Measure: validity, reliability and factor analysis.

BACKGROUND: Pain is a multidimensional experience that should be evaluated beyond an estimate of intensity. A multidimensional pain measure has not been developed for older persons undergoing comprehensive geriatric assessment. OBJECTIVE: To develop and evaluate validity and reliability of a multidimensional pain assessment instrument for older persons. RESEARCH DESIGN: A series of steps in instrument development and evaluation. SUBJECTS: A total of 176 subjects (mean age 84 +/- 6.0 years) in ambulatory geriatric clinics; 64% were women, and 73% had a history of chronic pain. MEASUREMENTS:

Factors related to the length of in-hospital stay of geriatric patients.

The aim of this study was to evaluate factors predicting length of stay in hospital of geriatric patients. Study participants were 402 patients (183 males and 219 females) consecutively admitted to four geriatric wards located in Chieti, Perugia, Pescara and Prato, Italy. Information on potential predictors of length of stay was collected; in particular, we assessed the presence and severity of specific chronic medical conditions, level of physical function, cognitive status, and depressive symptoms. Moreover, information on family and social support was obtained. In general, participants were

Alcoholism in a geriatric outpatient clinic of São Paulo-Brazil.

Alcohol abuse and dependence are an increasing health problem among the elderly, but there is only scanty information about their prevalence and associated risk factors in developing countries. The authors set out to evaluate the prevalence and associated clinical/demographic features of alcoholism in a sample of male elderly subjects attending a Geriatric Primary Health Outpatient Clinic in a State University Hospital in the City of São Paulo-Brazil. Three hundred four patients were assessed with the Brazilian version of the Michigan Alcoholism Screening Test and a semistructured questionnair

Hoarding symptoms in patients on a geriatric psychiatry inpatient unit.

BACKGROUND: While collecting may be a normal behaviour, hoarding is a symptom of various psychiatric disorders, including obsessive-compulsive disorder (OCD) and obsessive-compulsive personality disorder (OCPD). Although anecdotal reports suggest that hoarding is not uncommon in geriatric psychiatry populations, its psychopathological correlates in such samples have not been well characterised. METHODS: The presence of clinically significant hoarding symptoms was screened for in 100 consecutive patients in a geriatric psychiatry inpatient unit. Both patient and collateral histories were obtai

[Analysis of the block design test in a geriatric sample group].

We have carried out a study to determine the normal values of the 'block design test' in a sample group of 58 healthy elderly persons, a test which is usually used in the clinical evaluation of constructive apraxia. We therefore describe a grading scale which has not been previously published, in order to check the possible usefulness of this drawing as a rapid test of cognitive evaluation. A study was made of the possible interaction of the test with sample parameters, age, sex, educational level and also the inter-examiner reliability. We have observed a significant interaction between the v

[Surgery of the abdominal aorta in the geriatric population: characteristics and results].

Refinement in surgical technique and perioperative management have considerably improved morbidity and mortality rates of carotid endarterectomy and abdominal aortic surgery, thus allowing a more aggressive approach in the treatment of carotid stenosis, abdominal aortic aneurysm and aorto-iliac occlusive disease in the elderly population (> 70 years). We review our experience with 446 consecutive patients undergoing surgery of the abdominal aorta: 295 patients (mean age 75.5 years) underwent resection of an abdominal aortic aneurysm (asymptomatic but > 5-6 cm, (n = 193), symptomatic (n = 67) o

Predicting the health-related values and preferences of geriatric patients.

BACKGROUND: Those who must make health care decisions on behalf of persons who lack decision-making capacity often have too little information. The purpose of this study was to determine whether and to what degree sociodemographic factors, social support, religious involvement, and functional status served as predictors of the health-related values and preferences of geriatric patients. METHODS: A retrospective chart review involving 178 cognitively intact patients enrolled at a geriatric clinic at a university medical center was conducted. Patient variables included age, race, sex, marital s

A pilot leg ulcer clinic in a geriatric day hospital.

Twenty-six patients with leg ulcers with a mean age of 80 years were assessed at a day hospital to determine the aetiology and to carry out a trial of four-layer compression bandaging for those suitable. Ten had ankle brachial pressure indices (ABIs) < 0.8. Of nine patients with venous disease who received compression bandaging, only two ulcers healed and have remained healed with fitted below-knee elastic stockings. The remaining patients were unable to tolerate compression or developed deterioration of their ulcers. Nine patients had unhealing arterial ulcers all unsuitable for reconstructiv

[Reliability of the culture of a clean-voided specimen in geriatric patients].

The validity of clean-voided urine culture results was investigated in 100 consecutive admissions to the geriatric ward of a general hospital. Culture of clean-voided urine was compared with urethral catheter specimens as reference. Clean-voided and catheter specimens were obtained in 71 cases. The investigation included 67 cases. One clean-voided specimen was false positive. Two cultures were false negative. Three of the four patients with a urinary tract infection did not present with the typical symptoms, but with a general decline in physical and/or mental function. In this population a cl

Reading performance of geriatric patients post exudative maculopathy.

This study was performed to evaluate any differences in reading speed and duration of comfortable reading speed in geriatric patients with post disciform stage exudative age-related macular degeneration (ARMD) or ocular histoplasmosis. Reading speed and duration with CCTV, illuminated stand magnifier, and spectacle lenses were compared for 37 patients in the Low Vision Program at the Central Blind Rehabilitation Center. Results indicate significantly faster reading speed with CCTV and spectacle lenses as compared to illuminated stand magnifiers. The difference between reading speeds measured w

Evaluation of the causes and severity of geriatric incontinence. A critical appraisal.

The evaluation of urinary incontinence in the elderly differs from that in younger patients because of altered or nonspecific disease presentation, the different spectrum of pathophysiology, greater variation between individuals and, in some cases or settings, variable treatment goals. Evaluation must be multifactorial and extend beyond the genitourinary system, because many age-related conditions and the drugs used to treat them can cause or exacerbate urinary incontinence. Voiding records are a reliable measure of severity, although studies of their validity are still lacking. History-taking

Effect of cerumen removal on the hearing ability of geriatric patients.

The purposes of this study were to estimate the prevalence of impacted cerumen in a population of hospitalized elderly patients, and to evaluate the effect of cerumen removal in reversing hearing impairment. The design was a pre-test/post-test static-group comparison. Over a 1-year period, a random sample (n = 226) was drawn every third day from daily admission lists of English-speaking patients aged 65 or older, admitted to non-intensive care units of one hospital. On either the second or third day of hospital stay, subjects were given a hearing test using an AudioScope and then their ear can

Diagnosing vitamin B12 deficiency, a common geriatric disorder.

Vitamin B12 deficiency in the elderly is a common disorder associated with an increased morbidity if it goes undetected, as often happens. Its diagnosis can be enhanced if the clinician recognizes the associated clinical features of nonspecific symptoms, glossitis, and dermatologic and neuropsychiatric abnormalities, and realizes the limitations of various tests (serum B12 assay, parietal cell and intrinsic factor antibody, mean corpuscular volume, and Schilling tests). Available data indicate it is sufficient to prescribe replacement B12 injections three or four times a year.
