The Influence of Household- and Community-Level Sanitation and Fecal Sludge Management on Urban Fecal Contamination in Households and Drains and Enteric Infection in Children.

AbstractUrban sanitation necessitates management of fecal sludge inside and outside the household. This study examined associations between household sanitation, fecal contamination, and enteric infection in two low-income neighborhoods in Vellore, India. Surveys and spatial analysis assessed the presence and clustering of toilets and fecal sludge management (FSM) practices in 200 households. Fecal contamination was measured in environmental samples from 50 households and household drains. Enteric infection was assessed from stool specimens from children under 5 years of age in these household

Implementation of infection control in health facilities in Arua district, Uganda: a cross-sectional study.

BACKGROUND: At least 1.4 million people are affected globally by nosocomial infections at any one time, the vast majority of these occurring in low-income countries. Most of these infections can be prevented by adopting inexpensive infection prevention and control measures such as hand washing. We assessed the implementation of infection control in health facilities and determined predictors of hand washing among healthcare workers (HCWs) in Arua district, Uganda. METHODS: We interviewed 202 HCWs that included 186 randomly selected and 16 purposively selected key informants in this cross-sect

A social marketing approach to building a behavioral intervention for congenital cytomegalovirus.

Congenital cytomegalovirus (CMV) is the most common congenital infection in the United States, causing permanent disabilities in more than 5,500 children born each year. In the absence of a vaccine, a promising means of prevention is through a behavioral intervention that educates women about CMV and promotes adherence to hygiene guidelines during pregnancy. Although effective behavioral interventions have been identified for other infectious diseases with similar transmission modes, current research has not yet identified an effective intervention for CMV. One way to gather evidence and ident

Dirt-binding particles consisting of hydrogenated castor oil beads constitute a nonirritating alternative for abrasive cleaning of recalcitrant oily skin contamination in a three-step programme of occupational skin protection.

BACKGROUND: In occupational fields with exposure to grease, oil, metal particles, coal, black lead or soot, cleansing formulations containing abrasive bodies (e.g. refined walnut shell, corn, wood, plastic or pumice) are used. These may constitute an irritant per se. As an alternative, hydrogenated castor oil (also known as castor wax) beads have been developed as dirt-binding particles. A polar surface contributes to their mechanical cleaning effects in removal of oily grime. OBJECTIVES: Standardized examination of the in vivo effects upon the skin barrier of castor wax beads in comparison w

Significant reductions in methicillin-resistant Staphylococcus aureus bacteraemia and clinical isolates associated with a multisite, hand hygiene culture-change program and subsequent successful statewide roll-out.

OBJECTIVE: To assess the efficacy of a multimodal, centrally coordinated, multisite hand hygiene culture-change program (HHCCP) for reducing rates of methicillin-resistant Staphylococcus aureus (MRSA) bacteraemia and disease in Victorian hospitals. DESIGN, PARTICIPANTS AND SETTING: A pilot HHCCP was conducted over a 24-month period (October 2004 to September 2006) in six Victorian health care institutions (4 urban, 2 rural; total beds, 2379). Subsequently, we assessed the efficacy of an identical program implemented throughout Victorian public hospitals over a 12-month period (beginning betwe

Current and new strategies for the delivery of antiseptic agents.

The present mini-review explores the current methods used for the delivery of antiseptics and topical antimicrobials. Relevance of hand scrub with antiseptic liquid soap (e.g. chlorhexidine, PVP-iodine, triclosan) and alcohol-based hand rub is discussed and compared in terms of bactericidal activity, skin tolerance, and medical staff observance. New strategies for antibacterial delivery focus on the challenge of colloidal drug carrier such as liposomes, micro- and nanoparticles enabling sustained bactericidal effect and effective bacterial targeting.

[Investigation of the cause of contact dermatitis due to heavy metal in a metal spray process in a film-condenser factory].

To investigate causes of contact dermatitis in the metal spray process in a film-condenser factory, we developed a questionnaire survey for workers exposed to metal dust, analyzed the metal dust, and conducted patch tests with the metal dust and its constituents. In the questionnaire survey, we identified 12 workers (12/26, 46.2%) who had had dermatitis. The main symptoms were itching, itchy red skin and itchy papules. Analysis of the metal dust showed that there was no copper oxide, but nickel, not contained in the materials of the metal spray, was detected. One worker with dermatitis had a p

Efficacy of selected hand hygiene agents used to remove Bacillus atrophaeus (a surrogate of Bacillus anthracis) from contaminated hands.

CONTEXT: The intentional use of Bacillus anthracis transmitted via the US mail in October-November 2001 resulted in 22 people developing inhalation or cutaneous anthrax. Glove use with handwashing prior to and after contact with potential contaminated environmental surfaces and cutaneous lesions has been recommended. However, only limited data are available on the susceptibility of B anthracis to antiseptics. OBJECTIVE: To evaluate the efficacy of several hand antiseptics (interventions) and soap and water (control) against Bacillus atrophaeus, a surrogate of B anthracis. DESIGN, SETTING, AN

Training traditional birth attendants in clean delivery does not prevent postpartum infection.

OBJECTIVE: To compare the maternal outcome, in terms of postpartum infection, of deliveries conducted by trained traditional birth attendants (TBAs) with those conducted by untrained birth attendants. METHODS: The study took place in a rural area of Bangladesh where a local NGO (BRAC) had previously undertaken TBA training. Demographic surveillance in the study site allowed the systematic identification of pregnant women. Pregnant women were recruited continuously over a period of 18 months. Data on the delivery circumstances were collected shortly after delivery while data on postpartum morb

Relationship of blood lead levels to personal hygiene habits in lead battery workers: Taiwan, 1991-1997.

BACKGROUND: The blood lead concentrations of workers in lead battery factories are more than 4 times higher than those of the general population in Taiwan. Therefore, efforts are needed to reduce lead exposure in this high-risk population. A health promotion program on personal habits that reduce lead exposure has been carried out in a lead battery factory since 1991. This study investigated the longitudinal relationship of workers' blood lead concentrations and personal hygiene habits from 1991 through 1997. METHODS: In each of the 7 years of the study, occupational physicians questioned wor

Farm visits and zoonoses.

Recent reports of outbreaks of gastrointestinal illness, such as cryptosporidiosis, associated with visits to farms have drawn attention to the possible risks of acquiring zoonotic infections through contact with farm animals. These risks seem to be small, but it is important that people who work in urban and rural farms, and such establishments as animal sanctuaries where the general public can come into close contact with farm livestock and other animals, should be alerted to the simple precautions needed to prevent transmission of infections.

[Hospital infection in pediatric patients].

Hospital (nosocomial) infection means infection acquired by patients while they are in hospital, or by members of hospital staff. It will occur in modes of three categories; 1) cross-infection (infection acquired in hospital from other people, either patients or staff), 2) self-infection (infection caused by microbes which the patient carries on normal or infected areas of his own body) and 3) self-infection after acquisition of hospital pathogens from other people or the environment. Examples of hospital infections, that we experienced, were presented as follows; cross-infection due to gram-n

Knowledge of cholera, handwashing practice and its associated factors in Hayayaabe IDP camp, Borama, Somaliland: a community-based study.

BACKGROUND: Cholera, a severe diarrheal disease transmitted through contaminated water and food, poses a significant public health threat, particularly in vulnerable populations. Inadequate knowledge and poor hygiene practices are major drivers of its transmission. This study aimed to assess cholera knowledge and handwashing practices among residents of the Hayayaabe Internally Displaced Persons (IDP) camp in Borama, Somaliland. METHODS: A community-based cross-sectional study was conducted in the Hayayaabe IDP camp (500 households) between April and June 2024. Data were collected from 251 ra

SARS-CoV-2: how safe is it to fly and what can be done to enhance protection?

With lockdown restrictions over coronavirus disease 2019 being relaxed, airlines are returning to the skies. Published evidence of severe acute respiratory syndrome (SARS) coronavirus 2 transmission on aircraft is limited, but in-flight transmission of respiratory infections such as tuberculosis, influenza and SARS has been well described. Risk factors include proximity to index patients and sitting in aisle seats. Personal protection on aircraft could be enhanced by always wearing a well-fitting face mask and face shield or sunglasses, wiping surfaces and hands with alcohol-based sanitizers,

Diarrheal correlates associated with enteric bacterial infections among children below five years in Murang'a County, Kenya.

INTRODUCTION: The burden of childhood diarrheal disease has resulted in massive mortality and morbidity globally. Children below 5 years in sub-Saharan Africa are most implicated by diarrheal illnesses resulting in numerous medical consultations, admissions, and deaths despite the disease being easy to prevent and control. The study aimed to determine the correlates of enteric bacterial infection causing diarrhea. METHODS: During the months of April-October 2017, 163 children below five years presenting with diarrhea were randomly selected in Murang'a and Muriranja's hospitals. Bacterial agen

Community-Led Total Sanitation, Open Defecation Free Status, and Ebola Virus Disease in Lofa County, Liberia.

The Ebola virus disease (EVD) epidemic entered Liberia through Lofa County in February 2014 and spread to two health districts where the nongovernmental organization Global Communities had been implementing community-led total sanitation (CLTS) since 2012. By December 2014 the county had 928 Ebola cases (422 of them confirmed) and 648 deaths. Before the epidemic, CLTS was triggered in 155 communities, and 98 communities were certified as Open Defecation Free (ODF). Using mixed quantitative and qualitative methods, we determined that no cases of EVD were found in ODF communities and in only one
