A Simple Scalp Tunneling Technique Using a Foldable Tunneling Tool for Placement of Deep Brain Stimulation Electrode Lead.

OBJECTIVE: In bilateral deep brain stimulation (DBS) with a dual-channel implantable pulse generator (IPG), creating a subgaleal tunnel for the contralateral lead is challenging because existing tunneling devices are either too short or too rigid. Hardware complications, such as infection and erosion, remain among the most common sources of morbidity. We describe a simple and minimal technique using a sterile intubation guide and the straw included with the Boston Scientific tunneling tool. MATERIALS AND METHODS: We developed a tunneling technique that repurposes a sterile 14-Fr intubation st

The fate of syndesmotic screws.

BACKGROUND: A standard protocol for the management of syndesmosis injuries has yet to be established. Debate persists regarding number of screws, screw diameter, number of cortices purchased, and the need for and timing of screw removal. The purpose of this study was to identify factors related to screw fixation that may lead to the ultimate failure of syndesmosis fixation defined as a loss of reduction of the syndesmosis, screw breakage, screw loosening, or widening of the medial clear space. MATERIALS AND METHODS: A retrospective assessment of 137 consecutive patients who underwent open red

Bilateral Asynchronous Displaced Olecranon Fractures in a Patient With Osteogenesis Imperfecta.

Olecranon fractures are uncommon in children. Children with osteogenesis imperfecta (OI) are at an increased risk of olecranon fractures. This is a report of a 12-year-old male patient with known osteogenesis imperfecta type 1 who sustained bilateral asynchronous olecranon metaphyseal avulsion fractures. He sustained a right olecranon avulsion fracture from a fall and underwent open reduction and internal fixation with two Steinman pins and a tension band wire. He was placed in a cast for a month. The Steinman pins were removed at three months. Six months after the first fracture, he sustained

Inherent physiological artifacts in EEG during tDCS.

Online imaging and neuromodulation is invalid if stimulation distorts measurements beyond the point of accurate measurement. In theory, combining transcranial Direct Current Stimulation (tDCS) with electroencephalography (EEG) is compelling, as both use non-invasive electrodes and image-guided dose can be informed by the reciprocity principle. To distinguish real changes in EEG from stimulation artifacts, prior studies applied conventional signal processing techniques (e.g. high-pass filtering, ICA). Here, we address the assumptions underlying the suitability of these approaches. We distinguis

The Impact of Psychological Factors on Device Removal Surgery.

BACKGROUND: Implant removal is a common procedure in orthopedic surgery which can be associated with many complications such as scar formation, hematoma, nerve injury, infection, and refracture. Indications for orthopedic implant removal have declined in recent years. Most studies have considered orthopedic hardware removal as an unnecessary procedure in the absence of severe complications such as nonunion. Some studies have reported the complications of orthopedic hardware removal to be 24% to 50% dependent on their types and locations as well as on other factors such as patient's condition a…

Treatment of Infection After Tibial Intramedullary Nailing With Bone Distraction Combined With Free Anterolateral Thigh Flap for Concomitant Soft-tissue Defect.

A 57-year-old man presented with a tibial shaft fracture treated with tibial intramedullary nail fixation and plate and screw fixation for fibular fracture. Two weeks postoperatively, the patient developed skin sloughing with exposed bone and metal at the fracture site. Three weeks postoperatively, a physical examination revealed swelling of the left lateral malleolus and lower leg, a skin defect of approximately 3 × 5 cm2 at the lower third of the left tibia with an exposed fracture site and hardware in between the fracture fragments. The patient underwent debridement, removal of the intramed

Microdeformational wound therapy: A novel option to salvage complex wounds associated with the Nuss procedure.

BACKGROUND: Complex wounds associated with the Nuss procedure are a resource intensive complication that may lead to significant morbidity with potential removal of the implanted device and abandonment of the repair. We report our management technique of this complication utilizing microdeformational wound therapy (MDWT) that is safe, is efficacious and allows for salvage of the repair. OPERATIVE TECHNIQUE: We defined a complex wound as a wound that became suppurative and drained in the postoperative period and failed to resolve with a trial of conventional wound management and antibiotics. A Removable 3D-Printed Protective Cap for Chronic Cranial Implants in Rhesus Macaques (Macaca mulatta): A Descriptive Case Series and Technical Refinement.

Chronic neurophysiology studies in Rhesus macaques (Macaca mulatta) require implantation of cranial hardware including headposts, recording chambers, and fixation screws. A common complication is persistent picking, scratching and grooming around implant margins, which can result in skin retraction, chronic inflammation, infection, repeated surgical repairs, and premature removal of the animal from research studies. Existing implant-care protocols rely heavily on topical therapies that may inadvertently increase grooming behavior. We developed a removable carbon fiber 3D-printed cranial cap th

Long-term Clinical Outcomes of Screw Removal Versus Unrevised Patients Following Open Latarjet Procedure for Recurrent Anterior Shoulder Instability: A Matched-Pair Analysis.

BACKGROUND: Recurrent anterior shoulder instability often requires surgical intervention, with 2 prominent approaches: anatomic (eg, Bankart) and nonanatomic (eg, Latarjet) procedures. While most surgeons favor arthroscopic Bankart, the open Latarjet procedure offers advantages such as quicker recovery and lower redislocation rates. On the other hand, complications include postoperative pain due to an impingement between hardware and soft tissue, resolved through screw removal. PURPOSE/HYPOTHESIS: The purpose of this study was (1) to assess the Walch-Duplay score for functional outcomes and (

Arthroscopic Anterior-Row Fixation for Anterior Cruciate Ligament Avulsion Injuries.

Anterior cruciate ligament avulsion fractures are more commonly seen in children with open physes than in adults. Arthroscopic fixation is considered the gold standard in the management of such injuries. Our technique of anterior-row fixation for these injuries provides various advantages in the form of physeal-sparing, complete anatomic reduction with no anterior beaking, no arthrofibrosis, no residual instability, no intra-articular hardware, no need for a second operation to remove implants, and finally, a full range of movement with no loss of extension. Compared with other methods of fixa

Delayed Bladder Perforation Resulting From Large Bladder Stone and Gluteal Abscess Caused by Pelvic Plate Loosening: A Case Report.

Delayed bladder injuries resulting from screw or plate loosening, following pelvic ring fractures are rare, and this complication could be prevented. A 63-year-old woman presented with dysuria and lower abdominal pain, 13 years after the open fixation of a pelvic injury. Computed tomography revealed a 5-cm bladder stone and two migrated screws. Six months after the stone was removed, an abscess was noted over the left gluteal region. During the removal of the screw and abscess debridement, we accidentally observed that the anterior pelvic bridge plate had eroded into the bladder and had multip
