His-Purkinje conduction system pacing and atrioventricular node ablation in treatment of persistent atrial fibrillation refractory to multiple ablation procedures: A case report.

In patients with symptomatic atrial fibrillation refractory to optimal medical therapy, atrioventricular node ablation followed by permanent pacemaker implantation is an effective treatment option. A 66-year-old woman with symptomatic persistent atrial fibrillation refractory to multiple ablation procedures was referred to our institution. After optimal drug therapy, the patient still had obvious symptoms. Sequential His-Purkinje conduction system pacing and atrioventricular node ablation were performed. Left bundle branch pacing was used as a backup pacing method if thresholds of His bundle p

Neuromodulation for cardiac arrhythmia.

The autonomic nervous system is known to play a significant role in the genesis and maintenance of arrhythmias. Neuromodulation, mostly designed to increase the parasympathetic tone and suppress the sympathetic tone, has become an emerging therapeutic strategy for the treatment of arrhythmias. Emerging therapeutic approaches include cervical vagal stimulation, transcutaneous auricular vagal stimulation, baroreceptor activation therapy spinal cord stimulation, ganglionated plexi ablation, renal sympathetic denervation, and left cardiac sympathetic denervation.

Lyme disease and hemi-diaphragmatic paralysis: A case report and review of literature.

Borrelia burgdorferi is a spirochete that can cause Lyme disease from an infected tick bite causing a myriad of syndromes ranging from erythema migrans to oligoarticular arthritis and/or atrioventricular conduction block in the heart. It can also infect the central nervous system (CNS) and peripheral nervous system (PNS) causing cranial neuropathy, radiculoneuropathy as well as myelopathy. It has rarely been reported to involve the phrenic nerve presenting as dyspnea from diaphragmatic paralysis. Here, we present a case of a patient presenting with orthopnea and dyspnea on exertion who was dia

Low birth weight and risk of cardiac arrhythmias in children.

BACKGROUND: This study aimed to investigate a possible association between low birth weight (LBW) and other perinatal major characteristics and the risk of a child having accessory pathways of conduction tissue and cardiac arrhythmia later in life. METHODS: Thirty-one children aged 1-15 years (12 boys, 19 girls), in whom the persistence of accessory pathways of conduction tissue in the heart and inducible reciprocating supraventricular tachycardia were confirmed by means of programmable transoesophageal pacing, constituted the study group. None of them exhibited signs of either inborn or acqu

Molecular Signatures of Sinus Node Dysfunction Induce Structural Remodeling in the Right Atrial Tissue.

The sinus node (SN) is located at the apex of the cardiac conduction system, and SN dysfunction (SND)-characterized by electrical remodeling-is generally attributed to idiopathic fibrosis or ischemic injuries in the SN. SND is associated with increased risk of cardiovascular disorders, including syncope, heart failure, and atrial arrhythmias, particularly atrial fibrillation. One of the histological SND hallmarks is degenerative atrial remodeling that is associated with conduction abnormalities and increased right atrial refractoriness. Although SND is frequently accompanied by increased fibro

Early differentiation of long-standing persistent atrial fibrillation using the characteristics of fibrillatory waves in surface ECG multi-leads.

We characterized the f-waves in atrial fibrillation (AF) in the surface ECG by quantifying the amplitude, irregularity, and dominant rate of the f-waves in leads II, aVL, and V1, and investigated whether those parameters of the f-waves could discriminate long-standing persistent AF (LPeAF) from non-LPeAF. A total of 224 AF patients were enrolled: 112 with PAF (87 males), 48 with PeAF (38 males), and 64 with LPeAF (47 males). The f-waves in surface ECG leads V1, aVL, and II, which reflect well electrical activity in the right atrium (RA), the left atrium (LA), and both atria, respectively, were

Interatrial conduction times in paroxysmal atrial fibrillation patients with normal atrial volume and their correlation with areas of local prolonged bipolar electrograms.

BACKGROUND: Recent evidence has shown that the presence of abnormal substrate can be demonstrated also among patients with "lone" AF. OBJECTIVES: Interatrial conduction slowing is likely to characterize patients with paroxysmal atrial fibrillation (AF) and it could be correlated to the left atrium area of prolonged local bipolar endocardial electrograms. METHODS: P-wave duration (PWD), amplified PWD and endocavitary interatrial conduction time (IACT), were analyzed in 60 patients; 30 undergoing de novo ablation for paroxysmal AF with normal atrial volumes and without any other cardiac diseas

Electrocardiographic manifestation of the middle fibers/septal fascicle block: a consensus report.

There are fibers in the left ventricle (LV) (LV middle network) that in around one third of cases may be considered a true septal fascicle that arises from the common left bundle. Its presence and the evidence that there are 3 points of activation onset in the LV favor the quadrifascicular theory of the intraventricular activation of both ventricles. Since the 70s, different authors have suggested that the block of the left middle fibers (MS)/left septal fascicle may explain different electrocardiographic (ECG) patterns. The 2 hypothetically based criteria that are in some sense contradictory

The characteristics of an abnormal electrogram on the atrialized right ventricle in a patient with Ebstein's anomaly.

Catheter ablation with the use of radiofrequency energy has been used as a treatment for atrioventricular reciprocating tachycardia in patients with Ebstein's anomaly. However, the success rate in these patients is generally low. We experienced a case in which the decision regarding precise accessory pathway (AP) localization was impaired by the presence of an abnormal electrogram in the atrialized right ventricle (ARV) in a patient with Ebstein's anomaly. After the AP conduction was abolished, the abnormal electrogram was observed behind the ventricular wave in the ARV during sinus rhythm. Fu

Development of strategies for guiding cardiac resynchronization therapy.

This article illustrates the highly interesting possibilities of cardiac resynchronization therapy (CRT) yet also discusses several problems for optimal implementation. The authors have presented possible strategies to improve CRT by designing and using better measurements of mechanical dyssynchrony or discoordination, leading to better patient selection. In addition, application of CRT can be improved by developing techniques to reach the optimal pacing site(s) easier and algorithms for easier programming of optimal atrioventricular delay and interventricular interval. Also, application of CR

Simplifying the diagnosis and management of pulseless electrical activity in adults: a qualitative review.

OBJECTIVE: The algorithms provided for advanced cardiac life support by the American Heart Association and the European Resuscitation Council Guidelines for Resuscitation for the diagnosis and treatment of pulseless electrical activity (PEA) correctly stress the importance of searching for potentially treatable causes, and suggest contributing factors that should be considered. This study sought evidence to support the factors that they mention in the algorithms. DATA SOURCE: Human and animal studies in MEDLINE. STUDY SELECTION: Putative causes of PEA along with the string "AND (PEA OR pulse

[Paroxysmal, distal, complete atrio-ventricular block without escape rhythm].

BACKGROUND: The relationship between paraspinal muscle degeneration and low back pain (LBP), disability, and structural changes has been investigated in the literature, but it is still a matter of debate. We differentiated paraspinal muscle magnetic resonance imaging by quality and quantity, focusing on fatty infiltration (FI) and paraspinal muscles cross-sectional area (CSA) from T12 to S1 in patients with and without chronic LBP. We aimed to determine whether paraspinal muscle quantity (CSA) and quality (FI) are positively associated with LBP or degenerative/spinopelvic changes in the spine.

The anatomical basis of complete atrioventricular block in cats with hypertrophic cardiomyopathy.

The cardiac conduction system was examined histologically in 13 feline cases of hypertrophic cardiomyopathy (HCM) with complete atrioventricular (AV) block. Marked degeneration and fibrous replacement of the AV conduction system were consistently observed in the combined regions of the branching portion of the AV bundle and the upper portion of the left bundle branch. These changes were associated with extensive fibrosis of the central fibrous body and endocardial and myocardial fibrosis in the upper border of the ventricular septum. Chondrometaplastic lesions or osseous lesions, or both, pres

The impact of changing oedematous states on the QRS duration: implications for cardiac resynchronization therapy and implantable cardioverter/defibrillator implantation.

Increased ECG QRS duration (QRSd) in patients with dilated cardiomyopathy (DCM) or heart failure (HF) is a well-known phenomenon. The QRSd is not a static ECG measurement but shows fluctuations, and its recent inclusion among the parameters used in referring patients for implantable cardioverter/defibrillators (ICDs) or cardiac resynchronization therapy (CRT) has led to renewed interest in its natural course and its determinants. Although clinical deterioration has been traditionally associated with increasing QRSd, its changes often are left unexplained. Also, the recent description of a decr

Differences in the morphology and duration between premature P waves and the preceding sinus complexes in patients with a history of paroxysmal atrial fibrillation.

BACKGROUND: Atrial fibrillation (AF) is the most common arrhythmia encountered in clinical practice. Experimental and human mapping studies have demonstrated that perpetuation of AF is due to the presence of multiple reentrant wavelets with various sizes in the right and left atria. HYPOTHESIS: Atrial fibrillation may be induced by atrial ectopic beats originating in the pulmonary veins, and premature P-wave (PPW) patterns may help to identify the source of firing. METHODS: To evaluate the morphology and duration of PPWs, 12-lead digital electrocardiogram (ECG) strips containing clearly defi
