Congenital anomalies of the aortic valve frequently necessitate intervention in childhood. The most common aortic valve pathologies present in childhood are aortic stenosis and insufficiency. Presentation of aortic valve disease depends on severity and presence of concomitant syndromes and valvular disorders. Treatment options are largely categorised as medical, percutaneous repair or surgical repair and replacement. Surgical techniques have been refined over the last few years making this the mainstay of treatment in paediatric cases. Whilst repair is considered in most instances before repla

Technical developments in valvular intervention culminated in the first percutaneous valve replacement in the pulmonary position (2000) followed by replacement in the aortic position (2002). More recently, with the proven feasibility of percutaneous mitral valve repair, interventional cardiologists and cardiothoracic surgeons have begun to develop a new subspecialty dedicated to the percutaneous treatment of valvular disease. This review describes the clinical status of the current field, the applicability and limitations of new technologies, and the upcoming devices that will soon reach Phase

Carcinoid heart disease is a rare condition affecting mostly tricuspid and pulmonary valves causing right-sided heart failure. Surgical valve replacement is the mainstay of treatment when patients become symptomatic and/or in the presence of right heart remodeling. We present a case of severe pulmonary valve regurgitation secondary to carcinoid heart disease occurring 4 years after a surgical tricuspid replacement, successfully treated with direct transcatheter pulmonary valve implantation without pre-stenting.

Transcatheter aortic valve replacement (TAVR) is increasingly being performed in younger and lower surgical risk patients. Given the longer life expectancy of these patients, the bioprosthetic valve will eventually fail, and aortic valve reintervention may be necessary. Although currently rare, redo-TAVR will likely increase in the future as younger patients are expected to outlive their transcatheter bioprosthesis. This review provides a contemporary overview of the indications, procedural planning, implantation technique, and outcomes of TAVR in failed transcatheter bioprosthetic aortic valv

Valve-in-valve transcatheter aortic valve replacement (ViV TAVR) is an appealing alternative to surgical intervention on failed aortic prosthesis with comparable safety and efficacy. ViV TAVR has proven to be a technical challenge in stentless bioprosthetic aortic valves like free style aortic valve prosthesis. In this case series of two patients we report the utility of transesophageal echocardiogram guidance to help visualize surgical annulus and TAVR deployment.

Transcatheter aortic valve implantation (TAVI) within failed bioprosthetic surgical aortic valves (valve-in-valve TAVI) has become an established procedure, currently approved for patients deemed at high risk for repeat aortic valve intervention. Although less invasive than surgical reoperation, challenges of valve-in-valve treatment include higher rates of malposition, prosthesis-patient mismatch and coronary obstruction. Thus, optimal patient selection and preprocedural planning is of the utmost importance to minimise the risk of these complications. In this review article we provide a fully

Robotic aortic valve replacement is the latest advance in the field of aortic valve intervention and is increasingly being adopted by various centres with favourable early results. It allows surgeons to implant any commercially available aortic valve prosthesis (sutureless, biological, mechanical) in a surgically conventional manner, while offering minimal surgical trauma (sternum-free, rib-free, pectoralis muscle sparing). We present a step-by-step standardized procedure of robotic aortic valve replacement with a mechanical prosthesis for a young female patient with severe symptomatic aortic

Aortic annular enlargement is an important adjunct to aortic valve replacement in patients with a small aortic annulus and restricted aortic root anatomy. The Y-incision technique facilitates implantation of a larger prosthetic valve, reducing the risk of prosthesis-patient mismatch and improving postoperative haemodynamics. This video tutorial demonstrates Y-incision aortic annular enlargement combined with bioprosthetic aortic valve replacement in a 67-year-old woman with symptomatic severe aortic stenosis and a severely restricted aortic root anatomy. Preoperative imaging revealed a 19 mm a

We present a case report of a patient with an infected mitral valve myxoma and a literature review on the subject. A 33-year-old female presented with a history of fever and dyspnea evolving over a few days. On admission, she had a lupus-like syndrome with positive blood cultures for Haemophilus species . Echocardiogram revealed a giant mass involving both mitral leaflets causing severe regurgitation, requiring biological mitral valve replacement. Microscopy showed an infected myxoma and the patient was discharged asymptomatic upon completion of antibiotics. She did well on follow-up. This is

Mitral regurgitation (MR) is the most prevalent form of moderate or severe valve disease in the developed world. MR can result from impairment of any part of the mitral valve apparatus and is classified as primary (disease of the leaflets) or secondary (functional). The presence of at least moderate MR is associated with increased morbidity and mortality. With the goal of avoiding the risks of traditional surgery, transcatheter mitral valve therapies have been developed. The current transcatheter repair techniques are limited by therapeutic target and incomplete MR reduction, and thus transcat

TMVR for degenerative (stenosis and regurgitation) mitral valve prosthesis (valves and rings) results in immediate and sustainable reduction but not normalization of left atrial pressure. Concomitant comorbid conditions such as left ventricular diastolic dysfunction adversely impact left atrial pressure and should be treated. The long-term impact of postprocedural ASD and patient-prosthesis mismatch need to be systematically evaluated so we develop the appropriate data to guide decision making as more transeptal mitral procedures are being performed.

Mitral regurgitation is a common cardiac valve lesion, developing from primary lesions of the mitral valve or secondary to cardiomyopathies. Moderate or higher severity of mitral regurgitation imposes significant volume overload on the left ventricle, causing permanent structural and functional deterioration of the myocardium and heart failure. Timely correction of regurgitation is essential to preserve cardiac function, but surgical mitral valve repair is often delayed due to the risks of open heart surgery. Since correction of mitral regurgitation can provide symptomatic relief and halt prog
