PURPOSE OF REVIEW: Cardiogenic shock represents a very challenging patient population due to the undifferentiated pathologies presenting as cardiogenic shock, difficult decision-making, prognostication, and ever-expanding support options. The role of cardiac surgeons on this team is evolving.
RECENT FINDINGS: The implementation of a shock team is associated with improved outcomes in patients with cardiogenic shock. Early deployment of mechanical circulatory support devices may allow an opportunity to rescue these patients. Cardiothoracic surgeons are a critical component of the shock team who

AIMS: Mechanical unloading by ventricular assist devices (VADs) has become increasingly important for the therapy of end-stage heart failure during the last decade. However, VAD support was claimed to be associated with partial reverse remodelling. Unfortunately, the literature describes the contradictory effects of VAD systems on cardiac fibrosis, a hallmark of cardiac remodelling. To clarify these inconsistent results, the effects on cardiac fibrosis before and after mechanical unloading in 125 patients were examined.
METHODS AND RESULTS: Left ventricular myocardial tissue from ischaemic or

Cardiogenic shock (CS) refractory to conventional therapies continues to be a challenging medical syndrome, with poor prognosis and high complication and mortality rates. The application and use of temporary mechanical circulatory support (MCS) is a component in the treatment of CS patients and should be applied early in the presentation. Crucial to the success of their application, temporary MCS devices should be chosen based on degree of patient acuity and etiology of CS. Not all temporary MCS devices deliver the same degree of hemodynamic support and range from minimal support to systemic s

BACKGROUND: In Japan, ventricular assist devices (VADs) have been used for patients with severe heart failure as a bridge to transplantation (BTT) since 1992. However, it was not until 1997, when the Organ Transplant Law was enacted, that medical devices received approval by the national health insurance system for that use. To encourage research and development of innovative medical devices, the Pharmaceuticals and Medical Devices Agency has established a public-private partnership in collaboration with academic societies, hospitals and manufacturers.
METHODS: The Japanese registry for Mecha

In patients at risk of intraventrcular thrombosis, the benefits of chronic anticoagulation therapy need to be balanced with the pro-hemorrhagic effects of therapy. Blood stasis in the cardiac chambers is a recognized risk factor for intracardiac thrombosis and potential cardiogenic embolic events. In this work, we present a novel flow image-based method to assess the location and extent of intraventricular stasis regions inside the left ventricle (LV) by digital processing flow-velocity images obtained either by phase-contrast magnetic resonance (PCMR) or 2D color-Doppler velocimetry (echo-CDV

Heart failure, whether acute or chronic, remains a major health care crisis affecting almost 6 million Americans and over 23 million people worldwide. Roughly half of those affected will die within 5 years, and the annual cost exceeds $30 billion in the US alone. Although medical therapy has made some modest inroads in partially stemming the heart failure tsunami, there remains a significant population for whom medication is unsuccessful or has ceased being effective; such patients can benefit from heart transplantation or mechanical circulatory support. Indeed, in the past quarter century (an

Mechanical circulatory support (MCS) is increasingly used in patients with cardiogenic shock, advanced heart failure, and severe respiratory failure; however, anticoagulation management remains challenging due to the competing risks of thrombosis, bleeding, hemolysis, and device dysfunction. In this review, we synthesize contemporary evidence on anticoagulant selection, monitoring strategies, and device-specific management across major MCS platforms, including extracorporeal membrane oxygenation, temporary ventricular assist devices, Impella devices, and durable left ventricular assist devices

This study aims to investigate differences in hemodynamic conditions in the thoracic aorta for pulsatile and continuous-flow left ventricular assist devices (LVADs) using computational fluid dynamics (CFD). Patient-specific models were reconstructed from three patients with continuous-flow LVAD (HeartMate II, Thoratec Corporation) and three patients with biventricular assist devices (Excor, Berlin Heart) where only the aortic part was included in the simulations. CFD simulations were performed with constant inflow for the continuous-flow LVADs and time-varying inflow for the pulsatile devices.

An 82-year-old man with a HeartMate II left ventricular assist device presented with low-flow alarms and cardiogenic shock secondary to left ventricular assist device outflow graft obstruction. Given high risk for redo sternotomy, the heart team decided on percutaneous intervention with peripheral stents, a procedure that is currently limited to case reports. (Level of Difficulty: Advanced.).

Left ventricular assist devices (LVADs) are common and implantation carries risk of AKI. LVADs are used as a bridge to heart transplantation or as destination therapy. Patients with refractory heart failure that develop chronic cardiorenal syndrome and CKD often improve after LVAD placement. Nevertheless, reversibility of CKD is hard to predict. After LVAD placement, significant GFR increases may be followed by a late return to near baseline GFR levels, and in some patients, a decline in GFR. In this review, we discuss changes in GFR after LVAD placement, the incidence of AKI and associated mo

BACKGROUND: Heart failure-related cardiogenic shock (HF-CS) is now the leading etiology of cardiogenic shock (CS). Advances in temporary mechanical circulatory support (tMCS), particularly microaxial flow pumps (mAFP), alongside evolving organ procurement and heart-allocation policies, have expanded opportunities for hemodynamic stabilization, recovery, and heart replacement therapy. However, prolonged support capabilities have increased the complexity of clinical decision-making in a heterogeneous population with variable candidacy for heart replacement therapies if native heart recovery (NHR

Valvular heart dysfunction-related cardiogenic shock (VCS) is complex, with high rates of in-hospital mortality. Early recognition with timely intervention is paramount to preserving life. However, treatment of VCS remains clinically challenging. Appropriate use of mechanical circulatory support (MCS) devices may halt disease progression and improve morbidity and mortality. Optimal MCS device selection for VCS remains complex, but evidence supporting its use continues to grow. Following an extensive review of the literature along with clinical expertise, the authors have provided a practical a

The use of left ventricular assist devices (LVADs) has increased in recent years as a destination therapy. The HeartMate 3 (HM3) is currently the only commercially available LVAD for implantation in the United States. Societal guidelines for multimodality cardiac imaging evaluation of LVADs and temporary mechanical support devices were recently published and serve as a comprehensive resource for the evaluation of LVAD patients. Our review provides a strategic approach towards transthoracic echocardiographic (TTE) assessment of LVADs as well as discusses the role and application of multimodalit

BACKGROUND: Left ventricular assist devices (LVADs) are increasingly used in patients with advanced heart failure. As their prevalence grows, emergency physicians (EPs) are more likely to encounter these patients. However, LVAD-supported physiology differs from native cardiac function and can lead to unique complications. Prompt recognition and appropriate initial management are critical.
OBJECTIVE OF THE REVIEW: The objective of this literature review is to examine carefully selected pieces of literature regarding the initial assessment, stabilization, and emergency management of patients wi

Temporary mechanical circulatory support device technology has undergone significant evolution with the introduction of the SmartAssist platform, enabling enhanced monitoring of patient-pump interactions. Continuous-flow pump dynamics and patient hemodynamics are closely interconnected, and pump performance depends on physiologic (eg, afterload, preload) and anatomic (eg, pump position, ventricle size) parameters. The SmartAssist platform readily provides an array of data, including measured and derived variables, to support clinical decision-making; however, these new features are often not w

BACKGROUND AND OBJECTIVE: Ventricular assist devices (VADs) are generally designed to perform continuous flow. However, it has been proven that continuous flow, which is not a physiological hemodynamic state, may cause severe complications such as gastrointestinal bleeding, pulmonary hypertension, and ventricular suction. For these reasons, many pulsating blood pump control strategies have been proposed and have the potential for application in percutaneous ventricular assist devices (pVADs) or microaxial blood pumps. A few cases report extra hemolysis when introducing pulsating speed, while n

PURPOSE: A left ventricular assist device (LVAD) is an implantable cardiac pump that uses a magnetically-levitating rotor to pump blood into circulation for patients with congestive heart failure. The continuous high-frequency motion of the pump can cause significant interference in electroretinography (ERG) recordings. We evaluate filtering methods to improve ERG quality in the presence of LVAD interference.
METHODS: A patient with an implanted LVAD was referred to our clinic for ERG testing on suspicion of a retinal dystrophy. Full-field ERG (ffERG) and pattern ERG (pERG) were performed acc
