Comparison of joint angles and electromyographic activity of the lower extremities during standing with wearing standard and revised high-heeled shoes: A pilot study.

Revised high-heeled shoes (HHSs) were designed to improve the shortcomings of standard HHSs. This study was conducted to compare revised and standard HHSs with regard to joint angles and electromyographic (EMG) activity of the lower extremities during standing. The participants were five healthy young women. Data regarding joint angles and EMG activity of the lower extremities were obtained under three conditions: barefoot, when wearing revised HHSs, and when wearing standard HHSs. Lower extremity joint angles in the three dimensional plane were confirmed using a VICON motion capture system. E…

Combined flap with Masquelet technique and 3D-printed titanium cage for reconstruction of traumatic composite heel defects: a case report and literature review.

BACKGROUND: Open calcaneal fractures accompanied by substantial segmental bone loss and severe soft-tissue damage represent a rare clinical presentation. The intricate anatomy of the heel region poses a significant challenge to limb-salvage efforts in such cases, for which a standardized treatment protocol remains to be established. CASE SUMMARY: Here we report a case of extensive soft tissue defects around the ankle and heel, combined with a large segmental defect of the calcaneus, managed through a staged surgical approach. The first stage involved emergency debridement and coverage with an…

Custom-made foot orthoses with and without heel plugs and their effect on treatment outcomes and plantar pressures in patients with plantar fasciitis: A crossover study.

BACKGROUND: Plantar fasciitis (PF) is considered to be the most common cause of plantar heel pain in adults. Various types of foot orthoses (FOs) have been used to treat plantar heel pain, between flat ethylene-vinyl acetate (EVA) devices, to prefabricated orthoses, to completely custom FOs. The objective of this crossover study was to quantify both objective plantar pressures, and subjective foot function and comfort ratings for two custom foot orthoses (CFOs), one with a heel plug (HP) and one without. METHODS: Twenty-one patients with diagnosed PF participated in this study and were casted…

Treatment of Chronic Plantar Heel Pain With Radiofrequency Neural Ablation of the First Branch of the Lateral Plantar Nerve and Medial Calcaneal Nerve Branches.

From March 2012 to February 2013, 37 patients experiencing plantar heel pain for ≥6 months despite treatment with physical therapy and other conservative treatment modalities were followed up. If neurogenic heel pain originating from the first branch of the lateral plantar nerve was present, with or without the medial calcaneal nerve, diagnostic nerve blocks to these nerves were performed for confirmation. If the pain was determined to be of neurogenic origin, radiofrequency neural ablation (RFNA) was applied to the corresponding sensory nerve endings. Pain was evaluated using the visual analo…

Effects of long-term wearing of high-heeled shoes on the control of the body's center of mass motion in relation to the center of pressure during walking.

High-heeled shoes are associated with instability and falling, leading to injuries such as fracture and ankle sprain. This study investigated the effects of habitual wearing of high-heeled shoes on the body's center of mass (COM) motion relative to the center of pressure (COP) during gait. Fifteen female experienced wearers and 15 matched controls walked with high-heeled shoes (7.3cm) while kinematic and ground reaction force data were measured and used to calculate temporal-distance parameters, joint moments, COM-COP inclination angles (IA) and the rate of IA changes (RCIA). Compared with ine…

Relationship between self-reported high-heeled shoe use and bone mineral density using quantitative ultrasound at a community health fair.

This is the first known study to examine the relationship between high-heel use and bone mineral density (BMD). Because women are disproportionately affected by osteoporosis, it is important to identify possible modifiable behaviors of women that may adversely affect bone health. Many studies have shown changes in body mechanics when wearing high-heeled shoes in comparison to normal gait. Because the composition of bone changes according to mechanical load and muscle activity, this study investigates whether wearing high heels may alter BMD. Two hundred and twenty-one participants at a communi…

Comparison of usual podiatric care and early physical therapy intervention for plantar heel pain: study protocol for a parallel-group randomized clinical trial.

BackgroundA significant number of individuals suffer from plantar heel pain (PHP) and many go on to have chronic symptoms and continued disability. Persistence of symptoms adds to the economic burden of PHP and cost-effective solutions are needed. Currently, there is a wide variation in treatment, cost, and outcomes of care for PHP with limited information on the cost-effectiveness and comparisons of common treatment approaches. Two practice guidelines and recent evidence of effective physical therapy intervention are available to direct treatment but the timing and influence of physical thera…
