Surgical Treatment of Acetabular Posterior Wall Fracture with Hip Arthroscopy: A Case Report.

Posterior wall fractures are the most common type of acetabular fractures. Treatment can be conservative or surgical. Operative treatment is indicated for acetabular fractures that result in hip joint instability and/or incongruity, as well injuries with incarceration of fragments of bone or soft tissue within the hip joint. Surgical treatment can range from open reduction and osteosynthesis to hip arthroplasty. Arthroscopy has recently been used as the main surgical technique or as a reduction aid. In this case a 26-year-old male with a fracture of the posterior wall who underwent a posterior

Imaging evaluation of developmental hip dysplasia in the young adult.

OBJECTIVE: The purpose of this article is to review the clinical and imaging features as well as the potential complications of hip dysplasia in the young adult. Hip dysplasia is an important cause of secondary osteoarthrosis, which accounts for a significant proportion of patients requiring total hip arthroplasty. The radiographic diagnosis of mild hip dysplasia in the young adult may be subtle and is primarily based on the detection of deficient coverage of the femoral head by the acetabulum. CONCLUSION: Cross-sectional imaging, including CT and MRI, afford improved detection and characteri

Analysing the protective potential of padded soccer goalkeeper shorts.

OBJECTIVE: In this study, the potential of currently available padded soccer goalkeeper shorts to prevent hip injuries sustained in side jumps was investigated. DESIGN: Within the scope of this study, a survey among professional and amateur goalkeepers, trainers and the medical staff was performed. In addition, commercially available shorts were impact-tested using an artificial hip model. The results were compared with requirements established for hip protectors for elderly. RESULTS: The results of the survey found that contusions and abrasions were observed most often for all players. Frac…

Bone loss in patients with spinal cord injury: Incidence and influencing factors.

PURPOSE: To investigate the incidence and influencing factors of bone loss in patients with spinal cord injury (SCI). METHODS: A retrospective case-control study was conducted. Patients with SCI in our hospital from January 2019 to March 2023 were collected. According to the correlation between bone mineral density (BMD) at different sites, the patients were divided into the lumbar spine group and the hip joint group. According to the BMD value, the patients were divided into the normal bone mass group (t > -1.0 standard deviation) and the osteopenia group (t ≤ -1.0 standard deviation). The i

Prevalence and patterns of occult hip fractures and mimics revealed by MRI.

OBJECTIVE: The diagnosis of hip fractures can be difficult on radiography alone. MRI is frequently used to confirm or deny the presence of a minimally displaced hip fracture. This study evaluates the patterns of injury seen on MRI that are difficult to diagnose on radiography. MATERIALS AND METHODS: MRIs of 73 patients who were examined for possible hip fractures and whose radiographic findings were negative or equivocal for hip fracture were reviewed. Seventy-six studies were performed in 73 patients who were between 24 and 102 years old. MRIs were evaluated for the presence and location of

PERIPHERAL NERVE BLOCKS FOR HIP FRACTURES
IN EMERGENCY MEDICINE.

Hip fractures represent a major public health issue with increasing incidence as a population ages. The aim of this review is to describe peripheral nerve block techniques (the fascia iliaca compartment block and the pericapsular nerve group block) as pain management for hip fractures in emergency medicine, and to emphasize their benefits. Hip fractures are extremely painful injuries. The pain itself is unpleasant for patients and if left untreated it can lead to multiple complications during preoperative, operative and postoperative patient management. Pain management for elderly hip fracture

The Application of Fascia Iliaca Compartment Block for Acute Pain Control of Hip Fracture and Surgery.

PURPOSE OF REVIEW: Over 300,000 patients are hospitalized annually following hip fractures in the USA. Many patients experienced inadequate analgesia. We will review the perioperative effects of the fascia iliaca compartment block (FICB) in hip fracture patients. RECENT FINDINGS: FICB by injecting local anesthetics beneath the fascia iliaca results in significant pain relief in hip fractures. Neuropathies and vascular injuries are almost unlikely. Single-shot FICB is faster to place, yet providing about 8 h of analgesia when bupivacaine is used. Continuous FICB provides prolonged titratable a

Cementless total hip replacement for severe developmental dysplasia of the hip: our experience in Crowe's group IV.

Total hip replacement in developmental dysplasia of the hip grade IV of Crowe's classification presents some difficulties. In this study, we present our results of the treatment for this pathology, also describing the surgical techniques used and the complication we had. In this paper, 18 total hip replacements in developmental dysplasia of the hip Crowe IV were studied clinically and radiologically before and after surgery, with a mean follow-up of 4.2 years (min: 1 year). The average Harris Hip Score improved from 52 to 89. The average leg lengthening was 36 mm. When a subtrochanteric shorte

Proximal Iliotibial Band Syndrome in a Runner: A Case Report.

Overuse injuries of the hip are frequent among runners and often present a diagnostic dilemma requiring imaging modalities beyond radiographs, delayed diagnosis, and prolonged time away from sport and activity. We report a case of a 38-year-old female recreational runner with progressive lateral hip pain and magnetic resonance imaging findings of edema along the gluteal aponeurotic fascia and origin of the tensor fascia lata muscle consistent with the diagnosis of enthesopathy of the proximal iliotibial band. LEVEL OF EVIDENCE: V.

Strain on the human sciatic nerve in vivo during movement of the hip and knee.

Injuries to the sciatic nerve are an occasional complication of surgery to the hip and acetabulum, and traction is frequently the causative mechanism. In vitro and animal experiments have shown that increased tensile strain on peripheral nerves, when applied for prolonged periods, impairs nerve function. We have used video-extensometry to measure strain on the human sciatic nerve during total hip replacement (THR). Ten consecutive patients with a mean age of 72 years undergoing primary THR by the posterior approach were recruited, and strains in the sciatic nerve were measured in different com

Three part posterior fracture dislocation of the hip without fracture of the femoral head: review of literature and a case report.

This case report describes a rare posterior dislocation of the hip with fractures of the ipsilateral femoral neck and greater trochanter, without fracture of the femoral head, in a young adult male following a railway accident. This patient was managed within 6h of injury by open reconstruction. This unusual injury has not been reported previously. Cases of posterior fracture dislocation of the hip with a fracture of the femoral neck without fracture of the femoral head were reviewed from the literature. Mechanism of injury, controversies regarding operative procedures and appropriate surgical

Traumatic dislocation of hip joint following low-velocity trauma, similarities to glenohumeral instability.

Acute posterior hip dislocation following low velocity trauma in a young healthy adult is a rare injury. Almost 30 cases previously have been described in the literature. They are usually associated with prior trauma to the hip joint and will lead to recurrent instability after relocation. This injury is similar to shoulder instability: lesions and anomalies present in the hip joint with posterior instability are identical to the ones in the shoulder with instability. These are Bankart-like lesion of the posterior labrum, defect in ischiofemoral ligament similar to inferior glenohumeral ligame

Changes on magnetic resonance images after traumatic hip dislocation.

Fourteen patients with traumatic hip dislocation had serial magnetic resonance imaging and routine radiographic studies from the time of injury through 24 months after injury. One experienced radiologist interpreted all images prospectively for abnormalities suggesting osteonecrosis of the femoral head and posttraumatic arthritis. Eight hips demonstrated abnormal marrow signals on T1 and T2 weighted images within 6 weeks of injury. These changes progressed in 3 hips, and osteonecrosis was confirmed subsequently by plain radiography. The abnormal marrow signals in the remaining 5 hips proved to

Ipsilateral open anterior hip dislocation and avulsion fracture of the greater trochanter: An unusual case report.

Approximately 3% of all joint dislocations involve the hip joint, and only 8-10% of these will be anterior. Traumatic anterior open dislocation of the hip is rare in children and prone to be associated with injuries, extensive soft tissue damage, and avascular necrosis of the femoral head. We present a case of a 13-year-old boy who had an open anterior dislocation of the hip with ipsilateral avulsion of the greater trochanter after a tractor wheel crush in an agricultural accident. Additional lesions included a diaphyseal closed fracture of the contralateral femur. We report this case because

Open Anterior Dislocation of the Hip in Togo.

Anterior traumatic dislocations of the hip are much less common than posterior dislocations. To date, 14 cases of open anterior dislocation of the hip associated with such injuries, acetabular and femoral head fractures and femoral vascular and nerve damage have been reported. We present a case of a 23-year-old male who sustained open anterior dislocation of the hip with ipsilateral fracture of the greater trochanter after an accident on the public highway. Additional lesions included an iliac wing fracture and a perineal wound. We report this case because of the rarity and seriousness of this

Surgical dislocation of the hip for fractures of the femoral head.

Traumatic dislocations of the hip are high-energy injuries that frequently occur with fractures of the femoral head. Controversy exists regarding many aspects of the treatment of these fractures, but following reduction, large displaced head fragments require open reduction and internal fixation. Traditionally, an anterior approach to the hip joint has been used for surgical access to the femoral head, but this incision often results in limited visualization and subsequent difficulty with anatomic reduction. Recently, a surgical hip dislocation technique has been described for acetabular fract
