Evaluating the implementation of Electronic Palliative Care Co-ordination Systems (EPaCCS) and optimising future service provision for palliative and end-of-life care: a synopsis of the Optimal Care mixed-method study.

BACKGROUND: Electronic Palliative Care Co-ordination Systems are central to end-of-life care policy despite critical gaps in the evidence base underpinning their use. It is unclear how Electronic Palliative Care Co-ordination Systems are being implemented across England and there is a lack of research to understand their use in routine care. AIM: To understand how Electronic Palliative Care Co-ordination Systems are currently being used in routine care and to guide the development of interventions to support their optimal implementation and maximise patient benefit. METHOD: A mixed-method ap

Inpatient care of small and sick newborns: a multi-country analysis of health system bottlenecks and potential solutions.

BACKGROUND: Preterm birth is the leading cause of child death worldwide. Small and sick newborns require timely, high-quality inpatient care to survive. This includes provision of warmth, feeding support, safe oxygen therapy and effective phototherapy with prevention and treatment of infections. Inpatient care for newborns requires dedicated ward space, staffed by health workers with specialist training and skills. Many of the estimated 2.8 million newborns that die every year do not have access to such specialised care. METHODS: The bottleneck analysis tool was applied in 12 countries in Afr

A data-driven framework of typical treatment process extraction and evaluation.

BACKGROUND: A clinical pathway (CP) defines a standardized care process for a well-defined patient group that aims to improve patient outcomes and promote patient safety. However, the construction of a new pathway from scratch is a time-consuming task for medical staff because it involves many factors, including objects, multidisciplinary collaboration, sequential design, and outcome measurements. Recently, the rapid development of hospital information systems has allowed the storage of large volumes of electronic medical records (EMRs), and this information constitutes an abundant data resour…

Evaluating Hospital Resource Utilization for Hypoplastic Left Heart Syndrome Through Linkage of a Clinical Registry and an Administrative Database.

Resource utilization estimates for hypoplastic left heart syndrome (HLHS) in early childhood are lacking. We linked the National Pediatric Cardiology Quality Improvement (NPC-QIC) clinical registry to the Pediatric Health Information Systems (PHIS) database to estimate hospital resource utilization for a contemporary HLHS cohort. All hospitals with both PHIS and NPC-QIC data available were eligible for inclusion. Patients discharged from their Norwood procedure 10/1/2009-9/30/2015 were linked using indirect identifiers. Estimated cost and length-of-stay data for all inpatient episodes across t

Trends in procedure choice, cost, and health outcomes in infants undergoing Stage 1 palliation for hypoplastic left heart syndrome: a retrospective analysis of the Paediatric Health Information System.

Management of infants with hypoplastic left heart syndrome is resource-intensive. Trends in initial Stage 1 palliation choice and associated hospital cost and outcomes over time are unclear.Using a retrospective cohort of infants <30 days of age (2004-22) from the Paediatric Health Information Systems database, we analysed the annual prevalence of Stage 1 palliation choice, as well as the association between palliation choice and outcomes and resource use. Prevalence of palliation choice was calculated, and Mann-Kendall tests evaluated linear trends. Study outcomes were pooled across years and

Risk assessment of occupational exposure to anesthesia Isoflurane in the hospital and veterinary settings.

Despite the modern ventilation and waste anesthetic gas (WAG) scavenging systems, occupational exposure to common volatile anesthesia, isoflurane, can occur in the hospital and veterinary settings, but limited information exists on potential exposure and health risk of isoflurane. We assessed exposure dose rates and risks among clinicians and veterinary professionals from occupational exposure to isoflurane. Through a critical review of open literature (1965 to 2020), we summarized potential adverse effects and exposure scenarios of isoflurane among the professional groups, including anestheti

Assessment and Use of Electronic Patient Records for Smoking Cessation Support in Hospital Setting: A Cross-Sectional Study.

ISSUE ADDRESSED: Tobacco use remains a leading cause of preventable illness and death. Hospital admissions offer a critical opportunity to support smoking cessation. Electronic patient record (ePR) systems can enable systematic identification and coordination of cessation care; however, clinician engagement with smoking-related ePR functionality is not well understood. This study examined smoking-related assessment and documentation practices, and determinants influencing use of the ePR for smoking cessation support in a regional Australian hospital. METHODS: A cross-sectional survey was dist

Critical differences among stakeholders in definitions of success following inguinal hernia repair in older adults.

BACKGROUND: Inguinal hernia repair is one of the most common elective operations performed in adults aged 65 years and older, but there is little information on how different groups of stakeholders define successful inguinal hernia repair. The purpose of this study was to compare how older veterans, surgeons, anesthesiologists, and Veterans Affairs leadership define success after inguinal hernia surgery. METHODS: Semistructured interviews and focus groups explored the relative importance of several hernia-related outcomes with 40 veterans aged ≥65 years who recently underwent inguinal hernia

Occurrence of coccidioidomycosis in a tertiary hospital in the Brazilian semi-arid region: diagnostic challenges.

Coccidioidomycosis is a systemic fungal infection that primarily affects the lungs in mammals. It is endemic to certain areas of the Americas. In Brazil, the disease occurs exclusively in the Northeast of the country, and the only etiological agent described is Coccidioides posadasii, typically found in the soil of arid regions with low rainfall. Given the scarcity of cases in Brazil, a descriptive study was conducted based on cases diagnosed with coccidioidomycosis between 2012 and 2022 at a tertiary hospital in the northeastern region of Brazil. The study analyzed medical records to identify

Does location matter? A proposed methodology to evaluate neighbourhood effects on cardiac arrest survival and bystander CPR.

BACKGROUND: Traditional variables used to explain survival following out-of-hospital cardiac arrest (OHCA) account for only 72% of survival, suggesting that other unknown factors may influence outcomes. Research on other diseases suggests that neighbourhood factors may partly determine health outcomes. Yet, this approach has rarely been used for OHCA. This work outlines a methodology to investigate multiple neighbourhood factors as determinants of OHCA outcomes. METHODS: A retrospective, observational cohort study design will be used. All adult non-emergency medical service witnessed OHCAs of
