Baicalin protects mice from infection with methicillin-resistant Staphylococcus aureus via alleviating inflammatory response.

Sepsis was redefined as life-threatening organ dysfunction caused by a dysregulated host response to infection in 2016. One of its most common causes is Staphylococcus aureus, especially methicillin-resistant Staphylococcus aureus (MRSA), which leads to a significant increase in morbidity and mortality. Therefore, innovative and effective approaches to combat MRSA infection are urgently needed. Recently, host-directed therapy (HDT) has become a new strategy in the treatment of infectious diseases, especially those caused by antibiotic-resistant bacteria. Baicalin (BAI) is the predominant flavo…

Severe Legionnaires' disease.

BACKGROUND: Legionnaires' disease (LD) is a common but under-diagnosed cause of community-acquired pneumonia (CAP), although rapid detection of urine antigen testing (UAT) and advances in molecular testing have improved the diagnosis. LD entails intensive care unit (ICU) admission in almost one-third of cases, and the mortality rate ranges from 4% to 40%. This review aims to discuss recent advances in the study of this condition and to provide an update on the diagnosis, pathogenesis and management of severe LD. RESULTS: The overall incidence of LD has increased worldwide in recent years due

HMGB1-targeted anti-inflammatory rBox A gene therapy for viral airway infections and systemic inflammatory disease models.

UNLABELLED: High mobility group box 1 (HMGB1) is a host-derived proinflammatory molecule released during cell injury. Elevated extracellular HMGB1 levels have been associated with disease severity. HMGB1 contributes to host influenza responses by stimulating TLR4/MD-2 signaling, amplifying lung inflammation, and tissue damage via cytokine production and leukocyte infiltration. HMGB1 also plays a central role in other inflammatory diseases, including sepsis and liver toxicity, making it both a biomarker of disease severity and a potential therapeutic target. We previously reported development o

The mammosphere-derived epithelial cell secretome modulates neutrophil functions in the bovine model.

BACKGROUND: Innovative therapies against bacterial infections are needed. One approach is to focus on host-directed immunotherapy (HDT), with treatments that exploit natural processes of the host immune system. The goals of this type of therapy are to stimulate protective immunity while minimizing inflammation-induced tissue damage. We use non-traditional large animal models to explore the potential of the mammosphere-derived epithelial cell (MDEC) secretome, consisting of all bioactive factors released by the cells, to modulate host immune functions. MDEC cultures are enriched for mammary ste

Sequential High-Dose Ruxolitinib and Low-Dose Splenic Irradiation for Pretransplant Management of Splenomegaly Before Allogeneic Hematopoietic Cell Transplantation With Fludarabine/Busulfan-Based Conditioning in Myelofibrosis.

BACKGROUND: Significant splenomegaly remains a major barrier to successful allogeneic hematopoietic cell transplantation (allo-HCT) in myelofibrosis (MF), contributing to delayed engraftment and an increased risk of graft failure. Systemic JAK inhibition and splenic irradiation are commonly used as independent pretransplant strategies; however, evidence regarding their sequential use and integration into contemporary conditioning platforms remains limited. METHODS: We conducted a single-center retrospective study of nine patients with MF who underwent allo-HCT between 2020 and 2025. Patients

Regulatory Role of Host MicroRNAs in Flaviviruses Infection.

MicroRNAs (miRNAs) are small non-coding RNA that affect mRNA abundance or translation efficiency by binding to the 3'UTR of the mRNA of the target gene, thereby participating in multiple biological processes, including viral infection. Flavivirus genus consists of small, positive-stranded, single-stranded RNA viruses transmitted by arthropods, especially mosquitoes and ticks. The genus contains several globally significant human/animal pathogens, such as Dengue virus, Japanese encephalitis virus, West Nile virus, Zika virus, Yellow fever virus, Tick-borne encephalitis virus, and Tembusu virus.

Caspofungin enhances antifungal immunity in human blood through transcriptional reprogramming.

Candida albicans is a leading cause of invasive candidiasis, a life-threatening infection with high mortality despite antifungal therapy. Echinocandins are first-line agents due to their fungicidal activity and low toxicity profile. Beyond direct killing, they may also modulate host immunity, e.g. through fungal cell wall remodeling. However, the systemic impact of echinocandins on human immune cells remains incompletely understood, particularly in the context of drug resistance. Using an ex vivo human whole-blood infection model, we assessed the effects of caspofungin during infection with ec

Immunology of vector-borne diseases: the role of immunopharmacology in controlling viral and parasitic infections.

BACKGROUND: Vector-borne diseases (VBDs) pose a significant global health threat, driven by complex interactions between pathogens, vectors, and host immune responses. Understanding the immunopharmacological mechanisms underlying these interactions is crucial for developing novel therapeutic and preventive strategies. This review explores the immunomodulatory effects of vector saliva, pathogen-induced immune evasion, and host-directed immunotherapies to provide a comprehensive perspective on immune regulation in VBDs. METHODS: A systematic review of peer-reviewed literature was conducted usin

Vacuolar-ATPase inhibitors are antimicrobial agents active against intracellular mycobacteria.

Mycobacterium tuberculosis (Mtb) evades host defenses by inhibiting phagosome acidification in part through the secreted phosphatase PtpA, binding to the vacuolar ATPase (v-ATPase), and disrupting downstream cellular events. We investigated the antimicrobial effects of three v-ATPase inhibitors, Bafilomycin A1 (BafA1), Bafilomycin D, and Cladoniamide B (ClaB), on the growth of Mtb, M. abscessus (Mabs), and M. bovis BCG in THP-1 and murine infection models. We found potent inhibition of intracellular growth with MIC50 in the nanomolar range, with compounds showing a bacteriostatic inhibition of

Target the Host, Kill the Bug; Targeting Host Respiratory Immunosuppressive Responses as a Novel Strategy to Improve Bacterial Clearance During Lung Infection.

The lung is under constant pressure to protect the body from invading bacteria. An effective inflammatory immune response must be tightly orchestrated to ensure complete clearance of any invading bacteria, while simultaneously ensuring that inflammation is kept under strict control to preserve lung viability. Chronic bacterial lung infections are seen as a major threat to human life with the treatment of these infections becoming more arduous as the prevalence of antibiotic resistance becomes increasingly commonplace. In order to survive within the lung bacteria target the host immune system t

Host-virus interaction: the antiviral defense function of small interfering RNAs can be enhanced by host microRNA-7 in vitro.

Small interfering RNAs (siRNAs) directed against poliovirus (PV) and other viruses effectively inhibit viral replication and have been developed as antiviral agents. Here, we demonstrate that a specific siRNA targeting the region between nucleotides 100-125 (siRNA-100) from the 5'-untranslated region (5'-UTR) of PV plays a critical role in inhibiting PV replication. Our data demonstrate that siRNA-100 treatment can greatly reduce PV titers, resulting in up-regulation of host microRNA-7 (miR-7), which in turn, leads to enhance inhibition of PV infection further. Moreover, our results suggest th

The emering role of helminths in treatment of the inflammatory bowel disorders.

The problem of immune-mediated diseases, such as inflammatory bowel disorders (IBDs), still remains a significant clinical and therapeutic problem. Therefore, the tendency to search for safer and more effective methods of reducing their incidence and increasing the efficiency of therapy of this group of diseases is understandable. Recently, attention has been drawn to the potential therapeutic influence of intestinal helminths on the inflammatory process induced by the immune response, as well as the observed significant potential of these organisms for modulating the host immune response, whi

Dexamethasone inhibits Mycobacterium tuberculosis-induced glycolysis but preserves antimicrobial function in primary human macrophages.

Glucocorticoids (GC) are useful adjunctive host directed therapies (HDT) for sub-types of tuberculosis (TB). Macrophages play a central role in controlling Mycobacterium tuberculosis (Mtb) infection, relying on glycolytic reprogramming to support an effective host defence, yet the influence of GC on these important phagocytes is poorly understood. Here, we examined the impact of dexamethasone on metabolic and functional responses of primary human airway macrophages (AM) from bronchoalveolar lavage fluid and monocyte-derived macrophages (MDM). We found that dexamethasone significantly reduced b

Diet-microbiota-kidney axis reprogramming: mechanistic insights into microbial metabolite-driven precision nutrition for chronic kidney disease.

Medical nutrition therapy serves as the cornerstone in the management of chronic kidney disease (CKD). While conventional approaches emphasize macronutrient restriction and meal timing adjustments, research highlights the critical mediating role of gut microbiota in translating dietary patterns into physiological effects through metabolite production. Meanwhile, CKD progression is closely associated with dynamic interactions between gut microbiota and their metabolic derivatives. This review introduces the "diet-microbiota- metabolite-kidney axis" framework to elucidate how nutritional compone

Resistance-associated substitutions and response to treatment in a chronic hepatitis C virus infected-patient: an unusual virological response case report.

BACKGROUND: Direct-Acting agents (DAAs) target and inhibit essential viral replication proteins. They have revolutionized the treatment of Hepatitis C virus (HCV) infection reaching high levels of sustained virologic response. However, the detection of basal resistance-associated substitutions (RASs) to DAAs in naïve patients could be important in predicting the treatment outcome in some patients exhibiting failures to DAA-based therapies. Therefore, the aim of this work was to evaluate the presence of RASs as minority variants within intra-host viral populations, and assess their relationship

Hijacking the cytoskeleton: association of HCV polymerase with α/β-tubulin and its potential relation to the viral replication and cell proliferation.

Hepatitis C virus (HCV) manipulates host cellular pathways to create favourable conditions that support its replication and persistence. Identifying virus-host interactions helps to prevent disease progression and supports the development of host-directed antiviral therapies (HDTs) with a reduced risk of drug resistance due to viral mutations. In this study, a proteomic approach using in vitro pulldown assay and mass spectrometry identified α- and β-tubulin as novel cellular proteins physically associated with the viral RNA-dependent RNA polymerase (NS5B). This association was validated in hep

Differential effects of GVHD therapies on intestinal epithelium.

Graft-versus-host disease (GVHD), an often-fatal complication of allogeneic hematopoietic cell transplantation, is driven by inflammatory injury that damages target organs of the gastrointestinal (GI) tract, skin, and liver. Effective therapies must not only suppress GVHD reactivity of donor lymphocytes but also permit regeneration of the damaged epithelium, particularly in the GI tract. Systemic corticosteroids are the standard first-line treatment for GVHD because of their powerful immunosuppressive and anti-inflammatory properties but may retard epithelial repair. Ruxolitinib, a selective J

Pharmacogenetics of ribavirin-induced anemia in HCV patients.

Dual therapy (pegylated interferon plus ribavirin) was considered the standard of care for hepatitis C virus (HCV) treatment until 2011, when the first-wave direct-acting antivirals were added to this regimen for HCV genotype-1 patients to increase the sustained virological response rate. The second-wave direct-acting antivirals entered the clinical use also in some ribavirin (RBV)- and/or interferon-free combinations. Nevertheless, since some of the new therapeutic regimens also include RBV and its use results still associated with hemolytic anemia, this requires countermeasures to be prevent

Pharmacological exposure to ribavirin: a key player in the complex network of factors implicated in virological response and anaemia in hepatitis C treatment.

Ribavirin remains today a pivotal drug in the treatment of hepatitis C; in standard double therapy, as well as in triple combination with direct antiviral agents, ribavirin reduces relapse and can double the sustained virological response obtained with peginterferon alone or in association with direct antiviral agents. In the complex network of interacting factors determining sustained virological response independently of known predictive factors related to host and virus, two modern tools are emerging: polymorphisms in the IL28B gene and very early exposure to ribavirin. The use of a pharmac

Recent Advances in Antiviral Medicinal Chemistry against Flaviviruses Driven by Multistrategic Approaches.

Flaviviruses constitute a global health threat, mainly because of the increasing geographical spread of vectors and the suboptimal efficacy of existing prophylactic vaccines. Despite the substantial clinical burden imposed by these pathogens, a critical gap remains in the approved specific antiviral therapies. This Perspective critically evaluates recent advances in antiflaviviral drug discovery, focusing on viral nonstructural proteins (NSs), host-targeted therapeutic targets, and emerging targeted protein degradation (TPD) technologies. We highlight direct-acting antivirals (DAAs) against NS
