Comparative analysis of the gait disorder of normal pressure hydrocephalus and Parkinson's disease.

OBJECTIVES: Comparative gait analyses in neurological diseases interfering with locomotion are of particular interest, as many hypokinetic gait disorders have the same main features. The aim of the present study was (1) to compare the gait disturbance in normal pressure hydrocephalus and Parkinson's disease; (2) to evaluate which variables of the disturbed gait pattern respond to specific treatment in both diseases; and (3) to assess the responsiveness to visual and acoustic cues for gait improvement. METHODS: In study 1 gait analysis was carried out on 11 patients with normal pressure hydroc

Normal pressure hydrocephalus in a patient with systemic lupus erythematosus: a case report.

Neuropsychiatric symptoms in systemic lupus erythematosus (SLE) are varied. The exact etiology and appropriate treatment are difficult to determine. We describe a 43-year-old woman who was diagnosed 18 years earlier with SLE. She developed dementia, gait disturbance, urinary incontinence and deterioration of consciousness. Her cerebrospinal fluid pressure was not elevated. Brain computerized tomography scan revealed enlarged ventricle and cortical sulci. Normal pressure hydrocephalus (NPH) was diagnosed. She was treated by establishing a ventriculo-peritoneal shunt, resulting in the return of

Outcome of Surgery for Idiopathic Normal Pressure Hydrocephalus: Role of Preoperative Static and Pulsatile Intracranial Pressure.

OBJECTIVES: To examine the outcome of surgery for idiopathic normal-pressure hydrocephalus (iNPH) and how outcome relates to the preoperative static and pulsatile intracranial pressure (ICP). METHODS: An observational cohort study included all patients with iNPH managed at our department during the years 2002-2012 in whom overnight ICP monitoring was part of the preoperative work-up. Clinical data were retrieved from a quality registry and ICP scores from a pressure database. RESULTS: The study included 472 patients, 316 in the surgery group and 156 in the nonsurgery group. Among those treat

Indications for valve-pressure adjustments of gravitational assisted valves in patients with idiopathic normal pressure hydrocephalus.

BACKGROUND: Modern ventriculoperitoneal shunts (VPS) are programmable, which enables clinicians to adjust valve-pressure according to their patients' individual needs. The aim of this retrospective analysis is to evaluate indications for valve-pressure adjustments in idiopathic normal pressure hydrocephalus (iNPH). METHODS: Patients operated between 2004 and 2011 diagnosed with iNPH were included. Kiefer-Scale was used to classify each patient. Follow-up exams were conducted 3, 6, and 12 months after shunt implantation and yearly thereafter. Initial valve-pressure was 100 or 70 mmH2O. Planned

[Diagnosis of normal pressure hydrocephalus. A calculation model for determining intracranial pressure-dependent resistance and compliance].

Computer-aided analysis of the results of the intrathecal infusion test using our mathematical computation leads to simplification of this investigation, and therefore normal pressure hydrocephalus is more easily diagnosed. Simultaneous measurement of resistance and compliance during a single investigation allows the exertion of the patients to be minimized. In contrast to classic methods it is not necessary that the ICP reaches a plateau. Our mathematical method diverges with the description of a pressure-dependent slope of the function for the resistance from the static examination models. F

Safety and effectiveness of the assessment and treatment of idiopathic normal pressure hydrocephalus in the Adult Hydrocephalus Clinical Research Network.

OBJECTIVE: The aim of this study was to describe the processes and outcomes associated with patients at five sites in the Adult Hydrocephalus Clinical Research Network (AHCRN) who had undergone evaluation and treatment for suspected idiopathic normal pressure hydrocephalus (iNPH) and had 1-year postoperative follow-up. METHODS: Subjects with possible iNPH who had been prospectively enrolled in the AHCRN registry between November 19, 2014, and December 31, 2018, were evaluated by CSF drainage via either lumbar puncture or external lumbar drainage, consistent with recommendations of the interna

Gut Microbiome Differences Between Early Alzheimer Disease and Idiopathic Normal Pressure Hydrocephalus.

BACKGROUND: Alzheimer disease (AD) and idiopathic normal pressure hydrocephalus (iNPH) are neurodegenerative diseases causing memory decline. Previous studies have demonstrated an altered gut microbiome (GM) in both conditions. In this study, we compared the GM composition between the groups to find out how if the GM composition differed between the cognitively healthy individuals (CO) and AD groups, as well as between the AD and iNPH groups. METHODS: Thirty-seven CO participants, 21 mild AD patients and 10 participants with shunted iNPH gave fecal samples, which were subjected to 16S amplico

Radiological features of gait phenotypes in patients with idiopathic normal pressure hydrocephalus.

INTRODUCTION: According to the higher-level gait disorder (HLGD) pattern, patients with idiopathic normal pressure hydrocephalus (iNPH) can be divided into two motor phenotypes; a disequilibrium (wide-based gait) subtype and a parkinsonian (locomotor) subtype. We aimed to understand the neuroimaging correlates of iNPH phenotyping into different gait patterns, by assessing specific radiological features and their correlations with clinical scores. METHODS: We enrolled 86 probable iNPH patients (53 males; age range: 69-88 years), who underwent a comprehensive clinical assessment, including neur

Predictive Values of Lumbar Infusion Testing in Idiopathic Normal Pressure Hydrocephalus Diagnosis.

BACKGROUND AND OBJECTIVES: Idiopathic normal-pressure hydrocephalus (iNPH) remains a diagnostic challenge because of the lack of a single reliable diagnostic test. The aim of this study was to evaluate the performance of the lumbar infusion test (LIT) in the decision-making process for shunting in patients with suspected iNPH. METHODS: A total of 201 patients with suspected iNPH underwent complete clinical, radiological, and hydrodynamic evaluation, including LIT, between January 2016 and January 2024 at Lille University Hospital. Patients were categorized into unlikely, possible, or probable

A wearable gait-analysis device for idiopathic normal-pressure hydrocephalus (INPH) monitoring.

Idiopathic Normal Pressure Hydrocephalus (iNPH) is a progressive neurologic disorder (fluid build-up in the brain) that affects 0.2%-5% of the UK population aged over 65. Mobility problems, dementia and urinary incontinence are symptoms of iNPH but often these are not properly evaluated, and patients receive the wrong diagnosis. Here, we describe the development and testing of firmware embedded in a wearable device in conjunction with a user-based software system that records and analyses a patient's gait. The movement patterns, expressed as quantitative data, allow clinicians to improve the n

Forget About Memory: Disentangling the Amnestic Syndrome in Idiopathic Normal Pressure Hydrocephalus.

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) can present with both episodic amnestic syndrome and biomarkers of Alzheimer's disease (AD) pathology. OBJECTIVE: To examine the associations between amnestic syndrome and cerebrospinal fluid (CSF) AD biomarkers in iNPH and the CSF tap test response in iNPH patients with amnestic syndrome. METHODS: We used the Free and Cued Selective Reminding Test to divide iNPH into amnestic and non-amnestic patients. We compared their clinical, biological, and radiological characteristics and examined the reversibility of gait spatiotemporal para

Decrease of excessive daytime sleepiness after shunt treatment for normal pressure hydrocephalus.

Sleepiness and apathy are often reported in patients with normal pressure hydrocephalus. However, research on outcomes after shunt surgery has mainly focused on the classical triad symptoms, that is, gait, cognition, and bladder dysfunction. This study aimed to describe the effects of shunt treatment on excessive daytime sleepiness and whether there was a relation to changes in ventricular volume. Pre- and postsurgical excessive daytime sleepiness was investigated using the Epworth sleepiness scale in a sample of 32 patients with normal pressure hydrocephalus who underwent shunt surgery. Data
