A controlled study of the effects of 1,25-dihydroxycholecalciferol (1,25[OH]2D3) and vitamin D3 (D3) was performed in 18 non-dialyzed patients with chronic renal failure (CRF) (creatinine clearance below 35 ml/min) and mild renal osteodystrophy. To study the effects of acute and chronic cholestasis on vitamin D metabolism we investigated six cases of acute extrahepatic obstructive jaundice and eight cases of primary biliary cirrhosis (PBC) (three supplemented with vitamin D). Hyperparathyroidism is the main cause of renal bone disease. At the moment its progress can be retarded by controlling plasma calcium and phosphate. But the prevention and cure of hyperparathyroidism without surgery eludes us. There is a suggestion that the administration of some metabolite of Vitamin D may be more successful. Osteomalacia on the other hand does not appear to be an important problem and is easily prevented and cured.

Patients with severe symptomatic renal osteodystrophy were treated with either 1,25(OH)2D3 or 1 alpha(OH)D3. In 39 instances, there was either reversal of symptoms and/or a marked fall in plasma alkaline phosphatase.
