Effect of chronic endogenous hypercalcemia on prolactin and thyrotropin responsiveness in man.

To investigate whether chronic endogenous hypercalcemia influences TSH and/or PRL release from pituitary thyrotrophs and lactotrophs in man, 10 patients with endogenous hypercalcemia, due either to cancer or to primary hyperparathyroidism, were injected with 25 micrograms TRH iv. The TSH and PRL responses were compared with those obtained in an age-, sex-, and weight-matched group of patients comprised of 10 normocalcemic individuals with other diseases. The mean maximal TSH response in the hypercalcemic group (3.7 +/- 0.4 microU/ml) was 46% lower than in the normocalcemic group (6.8 +/- 1.2 m…

Therapy of hypercalcemia due to parathyroid carcinoma with intravenous dichloromethylene diphosphonate.

Dichloromethylene diphosphonate (Cl2MDP) a potent inhibitor of osteoclast-mediated bone resorption, lowers serum calcium in hypercalcemia associated wit malignancies and with primary hyperparathyroidism, and reduces excess calcium mobilization from bone in multiple myeloma and in Paget's disease. We have evaluated the effectiveness of intravenously administered Cl2MDP in five patients with parathyroid carcinoma, a disorder characterized by severe hypercalcemia, very high parathyroid hormone (PTH) levels, and marked osteoclast-mediated bone resorption. All patients had biopsy-proved metastatic …

Subcutaneous fat necrosis of the newborn and idiopathic hypercalcemia. Report of a case.

A female infant who was anoxic at birth had lesions of subcutaneous fat necrosis at the age of 16 days. She also had problems with poor feeding, frequent vomiting, and failure to thrive. Hypercalcemia was discovered at the age of 28 days and she died at 11 1/2 weeks despite return of the serum calcium level to normal with oral prednisone therapy and a diet low in calcium and devoid of vitamin D. Fourteen other cases of idiopathic hypercalcemia in patients with subcutaneous fat necrosis of the newborn have been reported. Most of these infants were full-term with prenatal complications. The hype…

Histoplasmosis with hypercalcemia, renal failure, and papillary necrosis. Confusion with sarcoidosis.

A 56-year-old man with a three-month history of fever, malaise, anorexia, mental confusion, and weight loss had hypercalcemia and azotemia. The chest roentgenogram was normal. Biopsy material removed 2 1/2 years previously showed noncaseating granulomas. Sarcoidosis was diagnosed, and prednisone was administered. Fever persisted, and the patient died 49 days after admission. Postmortem examination showed evidence of extensive disseminated histoplasmosis, interstitial nephritis, and papillary necrosis. This communication emphasizes the difficulty in diagnosing the etiology of disseminated, nonc…

The influence of the parathyroid glands on the hypercalcemia of experimental magnesium depletion in the rat.

Magnesium-deficient rats develop significant hypercalcemia, hypophosphatemia, and hyperphosphaturia. These changes suggest a state of hyperparathyroidism. This study examines the regulation of parathyroid gland activity in magnesium-deficient rats. Magnesium deficiency was induced in intact and chronically parathyroidectomized animals by feeding them a diet free of this cation. Control animals were pair fed and treated identically except for the inclusion of magnesium in their gavage solution.Magnesium-deficient rats with intact parathyroid glands developed significant hypercalcemia and hypoph…

[IDIOPATHIC HYPERCALCEMIA IN THE CHILD: 2 NEW CASES AND REVIEW OF THE LITERATURE].

Several reports of idiopathic hypercalcemia of childhood have been published since Lightwood in 1932 described dwarfism associated with mental retardation, strabismus, hypercalcemia, nephrocalcinosis and osteosclerosis. The present paper adds two new cases. The first patient was 15 months old when first seen with stunted physical and mental growth, a systolic murmur, and a serum calcium value of 13 mg. %. This child suffered several severe infections and in spite of a low calcium intake and the administration of chelating agents her blood calcium rose until death six months later. The other pa…

Isolated nonparathyroid hormone-mediated hypercalcemia: a rare presentation of retroperitoneal sarcoidosis.

Non-parathyroid hormone (PTH)-mediated hypercalcemia has diverse etiologies, including granulomatous disorders such as sarcoidosis, in which extrarenal 1-α-hydroxylase activity leads to excess production of 1,25-dihydroxyvitamin D (1,25(OH)₂D). We report a rare case of sarcoidosis presenting as isolated hypercalcemia without pulmonary involvement, complicated by initially normal 1,25(OH)₂D levels and a false-negative core biopsy. A 44-year-old man with hypertension and type 2 diabetes presented with malaise, polyuria, and polydipsia. Laboratory evaluation showed severe hypercalcemia with suppr…

Sarcoid Nodular Myopathy Presenting With Hypercalcemia: A Rare Presentation-Case Report.

An 18-year-old female patient with no past medical history presented with nonanginal chest pain 2 months after her delivery. She did not report any other symptoms such as cough, dyspnea, nausea, or vomiting. Her vital signs were stable. The only positive finding on physical examination was tenderness and edema, with a palpable lump on her right thigh. Other examinations did not provide any additional information. Her lab results showed elevated calcium (12.5 mg/dL), low parathyroid hormone (5.6 pg/mL), and near-normal 25-(OH) Vitamin D levels (27.4 ng/dL). The core needle biopsy of the right t

Hypercalcemia Causes More Severe Acute Pancreatitis: An International Multicenter Cohort Study.

Background: Hypercalcemia is a rare etiology of acute pancreatitis; only a few cases have been reported in the literature, and the severity of hypercalcemia-induced AP is unknown. We aimed to assess the occurrence and severity of hypercalcemia-induced AP and compare it with the clinical characteristics of AP caused by other etiological factors. Methods: We collected data from patients from the Hungarian Acute Pancreatitis Registry who had AP, a serum calcium level above 2.6 mmol/L, and no other AP etiology. AP patients with etiologies other than hypercalcemia served as control. Results: A tota…

Hypercalcemia associated with relapsed medulloblastoma due to bone metastasis: illustrative case.

BACKGROUND: Medulloblastoma can cause extraneural metastasis, particularly to the bones. However, only a few reports have shown that hypercalcemia occurs with medulloblastoma, and there are no reports on hypercalcemia causing consciousness disturbance. OBSERVATIONS: A 17-year-old male patient was treated for medulloblastoma with spinal and intracranial metastasis. He underwent subtotal resection, followed by radiotherapy to the craniospinal axis, posterior fossa, and primary site, as well as high-dose chemotherapy with autologous hematopoietic stem cell transplantation. He achieved complete r…

Case report of hypercalcemia-related kidney complications after discontinuation of denosumab.

BACKGROUND: The use of the osteoclastogenesis inhibitor denosumab is increasing in pediatrics, especially in the treatment of giant cell tumor or granuloma of bone or jaw, aneurysmal bone cyst, and other rare bone disorders. Particularly in pediatric patients, adverse kidney effects-such as acute kidney injury (AKI), hypertension, and nephrocalcinosis-are a significant concern that has received little attention. CASE-DIAGNOSIS/TREATMENT: In this report, we present three children who developed hypercalcemia-related AKI six months after discontinuation of denosumab treatment. Treatment of the h…

Cinacalcet Reverses Short QT Interval in Familial Hypocalciuric Hypercalcemia Type 1.

CONTEXT: Familial hypocalciuric hypercalcemia type 1 (FHH-1) defines an autosomal dominant disease, related to mutations in the CASR gene, with mild hypercalcemia in most cases. Cases of FHH-1 with a short QT interval have not been reported to date. OBJECTIVE: Three family members presented with FHH-1 and short QT interval (<360 ms), a condition that could lead to cardiac arrhythmias, and the effects of cinacalcet, an allosteric modulator of the CaSR, in rectifying the abnormal sensitivity of the mutant CaSR and in correcting the short QT interval were determined. METHODS: CASR mutational an…
