A 61-year-old woman was completing a hemodialysis treatment when routine cleaning of the hemodialysis machine was started. BACKGROUND: The heart secretes natriuretic peptides (NPs) in response to myocardial stretch. Measuring NP concentrations is a helpful tool in guiding treatment. It has been suggested that sodium ion and hyperosmolality could affect NP excretion. If this is true, peri-operative NP measurements could be inconsistent when hypertonic solutions are used. A low-solute-load diet and large fluid intake were not able to prevent hypertonic dehydration and to control the disease. A new stimulator of intestinal peristalsis-guthymine (guanylthiourea) was used experimentally and clinically for treatment of the bowel paresis in peritonitis. Preliminary, in experiments on 53 rabbits with colibacillar peritonitis an efficacy of guthymine was compared with other stimulators of the gastrointestinal tract: proserine, a hypertonic solution of sodium chloride and sorbitol. Mannitol and lactulose were used as probe molecules to measure intestinal permeability in children with active small-bowel Crohn's disease and with untreated coeliac disease.
