Rapid deployment of a telemedicine care model for genetics and metabolism during COVID-19.

The national importance of telemedicine for safe and effective patient care has been highlighted by the current COVID-19 pandemic. Prior to the 2020 pandemic the Division of Genetics and Metabolism piloted a telemedicine program focused on initial and follow-up visits in the patients' home. The goals were to increase access to care, decrease missed work, improve scheduling, and avoid the transport and exposure of medically fragile patients. Visits were conducted by physician medical geneticists, genetic counselors, and biochemical dietitians, together and separately. This allowed the program t

Stepped Care for Opioid Use Disorder Train the Trainer (SCOUTT) initiative: Expanding access to medication treatment for opioid use disorder within Veterans Health Administration facilities.

The US is confronted with a rise in opioid use disorder (OUD), opioid misuse, and opioid-associated harms. Medication treatment for opioid use disorder (MOUD)-including methadone, buprenorphine and naltrexone-is the gold standard treatment for OUD. MOUD reduces illicit opioid use, mortality, criminal activity, healthcare costs, and high-risk behaviors. The Veterans Health Administration (VHA) has invested in several national initiatives to encourage access to MOUD treatment. Despite these efforts, by 2017, just over a third of all Veterans diagnosed with OUD received MOUD. VHA OUD specialty ca

Implementing refinements: Attitudes, experiences, barriers, and solutions identified by laboratory animal professionals.

Refinements to improve the lives of animals used in science are generally endorsed by research institutions and regulatory bodies, but laboratory animal professionals face many barriers in implementing refinements. We sought to understand 1) attitudes and perceptions of barriers and solutions, and 2) how implementation of refinement may be influenced by norms and perceived behavioural control. We conducted three focus groups with 32 attendees at a Canadian laboratory animal science symposium, using semi-structured, open-ended questions. Five themes emerged: evidence and impact on research outc

Tools developed and disseminated by guideline producers to promote the uptake of their guidelines.

BACKGROUND: The uptake of clinical practice guidelines (CPGs) is inconsistent, despite their potential to improve the quality of health care and patient outcomes. Some guideline producers have addressed this problem by developing tools to encourage faster adoption of new guidelines. This review focuses on the effectiveness of tools developed and disseminated by guideline producers to improve the uptake of their CPGs. OBJECTIVES: To evaluate the effectiveness of implementation tools developed and disseminated by guideline producers, which accompany or follow the publication of a CPG, to promot

The history of smallpox vaccination in Italy: the vaccination campaign in the Ligurian Republic between the end of the Ancien Régime and the advent of the Napoleonic period (1797-1805).

In 1798, Edward Jenner publicized his technique of smallpox vaccination, thus making obsolete what had been for centuries the most effective weapon against smallpox: variolation. News of this sensational breakthrough spread quickly throughout Europe, reaching Italy, and particularly Genoa, then the capital of the Ligurian Republic (1797-1805). Indeed, the first vaccinations in Italy were carried out in Genoa, and several doctors there, first individually and then collectively (within the "Medical Emulation Society"), went on to implement and spread the practice of vaccination. Although it was

Implementation of Models of Service Delivery Designed for the Management of People With Hip and/or Knee Osteoarthritis in Primary Care: A Scoping Review Protocol.

RATIONALE: Hip and Knee Osteoarthritis (HKOA) is a growing public health challenge, associated with high disability and socioeconomic burden. Current literature indicates that care provided to people with HKOA does not follow evidence-based recommendations, is sub-optimal, reactive, and fragmented. This suggests the need to develop structured approaches to guarantee high-value care and ensure the sustainability of healthcare systems. Models of Service Delivery (MoSD), that is, how a model of care should be adapted and implemented in a local setting or health service at an operational level, ha

A collaborative approach to advancing research and training in Public Health Data Science-challenges, opportunities, and lessons learnt.

The unprecedented availability of increasingly complex, voluminous, and multi-dimensional data as well as the emergence of data science as an evolving field provide ideal opportunities to address the multi-faceted public health challenges faced by low and middle income countries (LMIC), especially those in sub-Saharan Africa. However, there is a severe lack of well-trained data scientists and home-grown educational programs to enable context-specific training. The lack of human capacity and resources for public health data analysis as well as the dire need to use modern technology for better u

Induced abortion in Iran, Tehran University of Medical Sciences, the law and the diverging attitude of medical and health science students.

BACKGROUND: The topic of induced abortion has been a subject of extensive debate among various moral and religious frameworks and continues to pose significant challenges within the domains of medical ethics and policy formulation. In the context of Iran, an Islamic republic, the approach to induced abortion has been notably influenced by historical, social, and political dynamics. Following the implementation of the 'Rejuvenation of the Population and Protection of the Family Law (RPPF)' in 2021, access to induced abortion and contraceptive measures has been markedly restricted as a response

Snakes among citizens: distribution and ecology of the Aesculapian snake, Zamenis longissimus (Laurenti, 1768) in Bratislava revealed by citizen science.

BACKGROUND: Urban environments are increasingly recognized as important habitats for wildlife, yet information on how reptiles use urban environments remains scarce. The Aesculapian snake (Zamenis longissimus), a non-venomous colubrid widespread in Central Europe, is considered a habitat generalist with a frequent association with human settlements. However, data on its occurrence and ecology in metropolitan areas are limited. To address this gap, we implemented a citizen science initiative in Bratislava, Slovakia, aiming to document the distribution and habitat use of the Aesculapian snake an

Advanced Placement Courses for Medical School: A Novel AMed Track to Reduce Financial Burden and Attract Nontraditional Students.

Medical school admissions have become increasingly competitive, creating a pool of nontraditional applicants that seek postbaccalaureate training in biomedical sciences. Several postbaccalaureate and graduate programs developed curricula that, except for learning clinical skills, mirror the learning objectives of the foundational science curricula in medical schools. This education structure provides applicants with a competitive advantage when applying to medical schools. However, basic science curriculum assessments in medical schools have changed to pass/fail scoring systems. As a result, s

Understanding capacity for implementing new interventions: A qualitative study of speech and language therapy services for children with speech sound disorder.

BACKGROUND: Many speech sound disorder (SSD) interventions with a long-term evidence base are 'new' to clinical practice, and the role of services in supporting or constraining capacity for practice change is underexplored. Innovations from implementation science may offer solutions to this research-practice gap but have not previously been applied to SSD. AIM: To explain variation in speech and language therapy service capacity to implement new SSD interventions. METHODS & PROCEDURES: We conducted an intensive, case-based qualitative study with 42 speech and language therapists (SLTs) in th…

Challenges in implementing integrated undergraduate medical curricula across low- and middle-income countries: A scoping review.

OBJECTIVES: Integrated curricula have become a core element of undergraduate medical education (UGME) reforms worldwide, including low- and middle-income countries (LMICs). LMICs, which are often characterized as resource-constrained settings, encounter challenges in integrated curricula implementation. However, these challenges are reported fragmentarily. This review mapped the literature on the implementation of integrated curricula in LMICs, with attention to reported challenges, contextual variation and gaps in the evidence base. METHODOLOGY: Electronic searches were conducted between Aug

Stock inhalers in action: results of a pilot study across multiple Illinois counties.

OBJECTIVE: Pediatric asthma health disparities are common. Schools often lack asthma diagnostic information and infrastructure to handle asthma symptoms. Undesignated or stock rescue inhalers available to anyone in respiratory distress can help prevent absences. METHODS: A stock inhaler pilot program was conceptualized and evaluated using implementation science frameworks. Districts from high-asthma-burden counties were engaged and offered guided implementation over the 2022-2023 or 2023-2024 school year. Districts were required to complete training and report stock inhaler incidents. Pre/pos…

Acceptability of Cardiovascular Disease Point-of-Care Diagnostics in Primary Care Settings: A Scoping Review.

BACKGROUND AND AIMS: Cardiovascular disease (CVD) remains the leading global cause of death, with 80% of deaths occurring in low- and middle-income countries (LMICs). Improving access to screening and early diagnosis is essential. Point-of-care testing (POCT), which provides rapid results near the patient, is particularly valuable in resource-limited settings. While POCT has been successfully implemented for infectious diseases like HIV and TB, investment in CVD-focused POCT remains limited. This scoping review maps global evidence on the acceptability of CVD POCT, guided by the World Health O
