Juvenile generalized pustular psoriasis with IL36RN mutation treated with short-term infliximab.

A 8-year-old Chinese boy with generalized pustular psoriasis (GPP) refractory to cyclosporine and methylprednisolone was treated successfully with two infusions of infliximab 3.3 mg/kg. He remained in remission for 21 months. Direct sequencing of IL36RN gene showed a homozygous mutation, c.115 + 6T>C. Juvenile GPP is a rare severe form of psoriasis occasionally associated with life-threatening complications. Like acitretin, cyclosporine and methotrexate, infliximab has been reported to be effective for juvenile GPP in case reports. However, there is a lack of data in the optimal treatment cour…

Infliximab/Plasmapheresis in vanishing bile duct syndrome secondary to toxic epidermal necrolysis.

Vanishing bile duct syndrome (VBDS) is a rare disorder characterized by loss of interlobular bile ducts and progressive worsening cholestasi … orting will lead to better regimens and outcomes. Our case report details the first reported use of infliximab and plasmapheresis, in addition to steroids, in a patient with VBDS secondary to TEN, as well as a literature review that supports a mechanism for why these modalities could be effective treatments. Unfortunately, our patient died, and the use of th …

Cat scratch disease during infliximab therapy: a case report and literature review.

Cat scratch disease may occur during etanercept therapy, but there has been no report on infliximab-associated cat scratch disease. We report a case of a 23-year-old woman who developed right inguinal lymph node enlargement following a cat scratch. The patient had received infliximab therapy for spondyloarthropathy. She was successfully managed by discontinuing infliximab and by treatment with moxifloxacin and amikacin.

Successful treatment of infliximab-associated immune-mediated sensory polyradiculopathy with intravenous immunoglobulin.

Infliximab, a tumor necrosis factor-alpha antagonist, is used to treat many inflammatory diseases. Various forms of demyelinating neuropathies have been reported as neurological complications associated with infliximab use. There have been few reports of pure sensory neuropathy associated with infliximab. We report the clinical, electrophysiological, and pathological findings of a patient with subacute sensory polyradiculopathy 1 month after infliximab therapy for psoriasis vulgaris. Immune-mediated pathogenesis was suggested by positive anti-ganglioside antibodies and rapid response to intrav…

Crohn's disease genotypes of patients in remission vs relapses after infliximab discontinuation.

AIM: To investigate genetic differences between Crohn's disease (CD) patients with a sustained remission vs relapsers after discontinuing infliximab while in corticosteroid-free remission. METHODS: Forty-eight CD patients received infliximab and were in full corticosteroid-free clinical remission but then discontinued infliximab for reasons other than a loss of response, were identified by review of an electronic database and charts. Infliximab-associated remission was defined as corticosteroid-free plus normalization of clinical disease activity [CD activity index (CDAI) < 150] during follow…

Infliximab for Crohn's disease: the first 500 patients followed up through 2009.

BACKGROUND: The aim of this study was to assess the long-term usage patterns and safety of infliximab in patients with Crohn's disease in clinical practice. METHODS: The medical records of 492 unselected patients treated with infliximab at Mayo Clinic were reviewed and abstracted for demographic features, usage patterns, and adverse events. RESULTS: The patients received a median of seven infusions and had a median follow-up of 6.3 years. Twenty-eight patients (6 %) were lost to follow-up, 63 patients (13 %) had no clinical benefit, and 401 patients (80 %) had partial or complete response. O…

A population-based study of health-care resource use among infliximab users.

OBJECTIVES: We sought to describe the characteristics of health-care utilization (HCU) among patients with Crohn's disease using infliximab (IFX). METHODS: Using the University of Manitoba Inflammatory Bowel Disease Epidemiology Database (UMIBDED), we extracted all subjects with newly prescribed IFX, newly prescribed purine analogs (azathioprine, AZA) (without IFX), newly prescribed steroids (Ster) (without IFX or purine analogs) after 2001, and those not prescribed any of these drugs (ND). All of the subjects must have had HCU data available for 5 years before initial prescription and for 3 …

Palmoplantar pustular psoriasis: successful therapy with efalizumab after non-response to infliximab.

Palmoplantar pustular psoriasis (PPP) is a chronic inflammatory skin condition mainly characterized by recurrent eruptions of sterile pustules on erythematous skin; hyperkeratosis and fissures on the palms and soles are additional clinical features. Treatment options for PPP are unsatisfactory. We present a patient with a typical course of PPP that had previously received a broad range of topical and systemic antipsoriatic therapies. They all had to be discontinued due to ineffectiveness or side effects. Being aware of the high efficacy of infliximab in generalized pustular psoriasis, we initi…

Does infliximab infusion impact results of operative treatment for Crohn's perianal fistulas?

PURPOSE: Infliximab is an effective treatment for active intestinal Crohn's disease; however, the efficacy of infliximab in perianal Crohn's disease is controversial. This study was designed to compare patients with Crohn's disease who underwent perianal fistula surgery with or without infliximab infusion. METHODS: A retrospective chart review of 226 consecutive patients with Crohn's disease who underwent operative treatment with or without infliximab (3-6 infusions of 5 mg/kg) from March 1991 through December 2005 was completed. Patients were classified as completely healed, minimally sympto…

Clinical and radiological responses after infliximab treatment for perianal fistulizing Crohn's disease.

OBJECTIVES: Infliximab is an effective therapy for fistulizing Crohn's disease of the perineum. We sought to determine whether the clinical improvement after infliximab is associated with radiological closure of fistula tracts. METHODS: Clinical responses and radiological imaging studies by transperineal ultrasound were evaluated in 35 patients with Crohn's disease perianal fistulas after treatment with infliximab 5 mg/kg up to 48 wk. Paired comparison of baseline and follow-up imaging studies at 8 wk and at 56 wk or discontinuation were assessed by an imaging score of perianal fistula severi…

Efficacy of infliximab for disc herniation-induced sciatica: one-year follow-up.

STUDY DESIGN: An open-label trial. OBJECTIVES: To test the long-term efficacy of infliximab, a monoclonal antibody against tumor necrosis factor-alpha (TNF-alpha), in disc herniation-induced sciatica. SUMMARY OF BACKGROUND DATA: Our recent trial indicated that a single infusion of 3 mg/weight-kg of infliximab produced a rapid curative effect in disc herniation-induced sciatica. Here, we describe the 1-year effect of a 3 mg/kg of infliximab in these 10 patients and our experience with a lower dose of 1 mg/kg of infliximab for the same indication in 2 additional patients. METHODS: Patients wi…

[Anti-TNF alpha monoclonal antibodies (infliximab) and tuberculosis: apropos of 3 cases].

INTRODUCTION: Monoclonal TNF alpha antibodies are a new treatment of severe rheumatoid arthritis. One of the possible side effects is the appearance of opportunistic infections. We report here on three cases of disseminated tuberculosis observed in patients undergoing treatment with infliximab. EXEGESIS: A 45-year-old woman, treated with infliximab, was hospitalised after five infusions for fever and dyspnoea. The exams showed pulmonary and peritoneal tuberculosis. The second case is a 75-year-old woman whose symptoms were fever, cough and cervical adenopathy after three infliximab infusions.…

Organizing pneumonia potentially caused by infliximab in a pediatric patient with ulcerative colitis.

BACKGROUND: Organizing pneumonia (OP) is a rare interstitial lung disease characterized by intra-alveolar fibroblastic plugs and chronic inflammation. Although most cases are idiopathic, OP may occur secondary to infections, autoimmune disease, or drug exposure. Pulmonary complications of tumor necrosis factor-alpha (TNF-α) inhibitors are uncommon, and infliximab-associated OP has not yet been described in the pediatric population. CASE PRESENTATION: A 12-year-old girl with ulcerative colitis (UC) on infliximab presented with three weeks of dyspnea, fever, and pleuritic chest pain. Computed t…

Utility of tolerance assays for real-time management of infliximab infusion reactions.

Failure of infliximab (IFX) therapy is often associated with pharmacokinetic challenges, frequently linked to the development of anti-infliximab antibodies (ATI). ATI formation is a key factor contributing to therapeutic failure and significant safety concerns, as these antibodies can trigger severe infusion-related reactions that may require treatment discontinuation. In Spain, most available ATI detection assays are sensitivity-based, identifying antibodies only in the absence of circulating IFX. In contrast, tolerance assays can detect ATI even in the presence of IFX, offering distinct adva…

Fulminant Tuberculous Pericarditis Presenting as Cardiac Tamponade in an Infliximab-Treated Patient.

Tuberculous pericarditis (TBP) is a rare but potentially life-threatening manifestation of extrapulmonary tuberculosis (TB), representing a clinically important subset of pericarditis cases. Diagnostic uncertainty arises from variable symptoms and the low sensitivity of available tests. Although culture is the gold standard, it is reliable but too slow to enable a timely diagnosis. Acute and fulminant presentations have been reported in immunosuppressed patients. A 61-year-old woman, recently receiving infliximab and corticosteroids for Crohn's disease, presented to the emergency department wi…
