Student Performance and Instructional Complexity Across Team-Based and Mastery Learning Course Formats.

OBJECTIVE: To compare cumulative final examination performance between mastery learning (ML) and team-based learning (TBL) and conduct an exploratory characterization of differences in instructional complexity between the 2 models. METHODS: This retrospective study compared 7 years of student performance data from a TBL course format (2012-2018, n = 996) with 7 years following implementation of ML (2019-2025, n = 1078). Course content, sequencing, learning objectives, and cumulative examination blueprint remained relatively stable across years. The primary outcome was student performance on a

The role of Advanced Practice Providers in improving primary care capacity: practice perspectives on challenges and opportunities.

BACKGROUND: In Virginia and across the United States, primary care workforce shortages limit access to care and exacerbate adverse health outcomes. We conducted a statewide survey of primary care practices in 2022 and found that 42% of practices reported losing a physician or Advanced Practice Provider (APP), including Nurse Practitioners (NPs) and Physician Assistants (PAs), in the past year. Adding APPs to primary care teams is one strategy practices are using to mitigate the shrinking availability of primary care physicians. However, APPs face workforce challenges that increase their suscep

Development and Clinical Deployment of the Systematic Urgent Team Response in the Operating Room (SUTO) Protocol for Intraoperative Massive Hemorrhage: A Technical Report.

Massive intraoperative hemorrhage is a time-critical event requiring simultaneous hemodynamic resuscitation, bleeding source control, transfusion support, equipment preparation, and escalation to multidisciplinary support. This technical report provides, to our knowledge, the first English-language description of the Systematic Urgent Team Response in the Operating Room (SUTO) protocol for intraoperative massive hemorrhage. The protocol was previously developed and reported in Japanese as an institution-specific response system. This English-language report was prepared with permission from th

Moral disengagement in psychiatric inpatient care for individuals who engage in self-injury: a conceptual framework for understanding staff responses and patient experiences.

Psychiatric inpatient care for individuals who engage in self-injury involves tensions between preventing harm, respecting autonomy, sustaining relational care, and managing organizational demands. Staff may experience emotional burden and moral distress, while patients may experience care as invalidating, coercive, or insufficiently responsive to their needs. This article develops a theory-informed conceptual framework applying Albert Bandura's theory of moral disengagement to psychiatric inpatient care for self-injury. Drawing on an interpretive synthesis of Bandura's theory and clinical lit

Goals of Care Discussions in Orthopaedic Surgery: Geriatric Hip Fractures.

BACKGROUND: Due to the severity of hip fracture complications in the geriatric population, including major morbidity and mortality, it is crucial to establish treatment goals and manage expectations as a patient-centered multidisciplinary team. Goals of care (GOC) are personalized treatment programs designed to align with the individual patient's values and preferences. There is a paucity of literature on the topic of GOC discussions, especially regarding orthopaedic injuries. Therefore, this narrative review aims to provide an account of GOC discussions related to geriatric hip fractures. ME

Silent burdens: global perspectives on grief and psychological strain among neuro-oncology neurosurgeons.

Neuro-oncology neurosurgeons (NONs) manage patients with progressive neurological decline, make high-stakes decisions, and face repeated exposure to death. While burnout has been recognized as a significant concern, grief and psychological strain remain underexplored as related, partly overlapping phenomena. To address this gap, we convened an exploratory international cohort of NONs to develop a dialogue-informed perspective on grief. Insights from semistructured roundtable discussions were used to explore how grief and psychological strain are experienced, their key triggers, coping approach

Treatment of e-cigarette use among hospitalised adolescents and young adults: a protocol for intervention development and evaluation of preliminary efficacy and implementation outcomes in a randomised controlled trial.

INTRODUCTION: Hospitalisation represents an opportunity to identify and treat e-cigarette use among adolescents and young adults (AYAs). Knowledge on how to provide this care is lacking. We aim to fill this gap by developing an e-cigarette use intervention and evaluating preliminary efficacy and implementation outcomes among hospitalised AYAs. METHODS AND ANALYSIS: We will enrol 144 hospitalised AYAs (14-21 years) who report past 30-day e-cigarette use and randomise 2:1 to intervention or control arms. We will develop an evidence-based intervention that includes education, motivational interv

Visual analysis of abdominal aortitis treatment using the CiteSpace bibliometric method.

BACKGROUND: Abdominal aortitis can induce aneurysms, and tumor rupture can lead to organ ischemia or even sudden death. At present, there is a lack of extensive understanding and identification of key problems in the treatment of abdominal aortitis, which needs to be further analyzed using bibliometric analysis. AIM: To discuss the research hotspot and development trend of abdominal aortitis treatment. METHODS: We searched the English literature (published from January 1, 2000 to March 12, 2024) on the treatment of abdominal aortitis in the Web of Science database. Then, we identified and sc

[Ethical Decision-Making in Late-Termination Requests: Recommendations for Developing In-Hospital Policies].

BACKGROUND: Late terminations of pregnancy (after 14 weeks of gestation) can present physicians and other healthcare professionals with complex medical and ethical challenges. In Germany, such terminations require a specific form of medical indication (a danger of grave impairment to the pregnant woman's physical or mental health). Despite these legal provisions, the decision-making process in individual cases often remains characterized by uncertainty, a lack of transparency and insufficient structural support. AIM: This article offers recommendations for the development of institutional eth

Improving Safe Thermoregulation Practices in an Urban Outpatient Surgery Center Through a Quality Improvement Initiative.

PURPOSE: Inadvertent perioperative hypothermia, defined as a core temperature below 36 °C, can lead to complications but is preventable when nursing interventions follow clinical practice recommendations. At an urban outpatient surgery center in Kirkland, Washington, a quality improvement initiative was implemented to improve thermoregulation practices to 80% over 8 weeks. DESIGN: A rapid cycle quality improvement project, guided by the Institute of Medicine's safety aim, was carried out using the Plan-Do-Study-Act model. METHODS: Quantitative data were collected via a chart review with the

Diagnosis and management of Stevens-Johnson syndrome/toxic epidermal necrolysis.

Stevens-Johnson syndrome/toxic epidermal necrolysis (SJS/TEN) is a rare, immunologically mediated cutaneous adverse reaction characterized by mucous membrane and epidermal detachment, with a mortality ranging from 15% to 25%. Risk factors for the development of SJS/TEN include immune dysregulation, active malignancy, and genetic predisposition. Medications are the most common cause, particularly antimicrobials, antiepileptics, allopurinol, and nonsteroidal anti-inflammatory medications. Drug-specific CD8 T-cells and natural killer cells are thought to be the major inducers of keratinocyte apop

The recognition and management of intrapartum fetal heart rate emergencies: beyond definitions and classification.

The ongoing and well-documented debate about the value of electronic fetal monitoring has focused primarily on the fact that most variant fetal heart rate (FHR) patterns are poor predictors of fetal acid-base status. Most recently, much of this attention has been focused on the implications for clinical management of FHR patterns that the National Institute of Child Health and Human Development has classified as indeterminate: neither normal nor abnormal. Given that a majority of fetuses will have an FHR pattern considered indeterminate at some point in labor, this is an important and worthwhi

Sentinel lymphadenectomy in node negative breast cancer.

SLND, regardless of method, can precisely predict the status of the axillary lymph nodes. Despite differences in technique, the consistent results support the sentinel node hypothesis in breast cancer. The procedure is well tolerated, and staging can be achieved accurately with minimal morbidity. SLND is a minimally-invasive procedure that provides tissue for the pathologist that represents the site most likely to harbor metastases. If a negative sentinel node is removed at SLND, it equates to truly node-negative breast cancer in almost all cases when done by experienced surgeons familiar with
