Factors Associated With Patient's Refusal of Recommended Cancer Surgery: Based on Surveillance, Epidemiology, and End Results.

OBJECTIVES: Most non-metastatic cancer patients can harvest a preferable survival after surgical treatment, however, patients sometimes refuse the recommended cancer-directed surgery. It is necessary to uncover the factors associated with patent's decision in taking cancer surgery and explore racial/ethnic disparities in surgery refusal. METHODS: Based on the Surveillance, Epidemiology and End Results (SEER)-18 program, we extracted data of non-metastatic cancer patients who didn't undergo surgery. Ten common solid cancers were selected. Four racial/ethnic categories were included: White, bla

Predictors of Surgical Site Infection After Nonemergent Craniotomy: A Nationwide Readmission Database Analysis.

OBJECTIVE: Surgical site infections (SSIs) carry significant patient morbidity and mortality and are a major source of readmissions after craniotomy. Because of their deleterious effects on health care outcomes and costs, identifying modifiable risk factors holds tremendous value. However, because SSIs after craniotomy are rare and most existing data comprise single-institution studies with small sample sizes, many are likely underpowered to discern for such factors. The objective of this study was to use a large hetereogenous patient sample to determine SSI incidence after nonemergent craniot

5-Factor Modified Frailty Index predicts 30-day complications after amputation for open lower-extremity fracture: an American College of Surgeons Trauma Quality Improvement Program database study.

PURPOSE: Open lower-extremity fractures are high-energy injuries, with severe cases necessitating amputation. While frailty predicts adverse surgical outcomes, its role in postoperative risk among patients undergoing amputation for open lower-extremity fractures has not been defined. This study aimed to determine whether frailty, measured by the 5-Item Modified Frailty Index (mFI-5), predicts 30-day postoperative complications following surgical amputation for open lower-extremity fractures. METHODS: This retrospective cohort study utilized the American College of Surgeons Trauma Quality Impr

Sex Differences in Clinical Outcomes and Surgical Interventions for Infective Endocarditis: A Nationwide Registry.

BACKGROUND: Sex differences in infective endocarditis (IE) remain underexplored in large-scale studies. Current findings on clinical outcomes, particularly in surgical IE, are inconsistent, highlighting critical knowledge gaps. METHODS: We conducted a nationwide cohort study using the Taiwan National Health Insurance Research Database (2001-2022) and a total of 28 976 adults with IE were identified. Subgroup analysis focused on 4133 patients undergoing valve surgery. Primary outcomes included in-hospital mortality and long-term survival, analyzed using logistic regression and Cox models. RES

Cardiovascular function following surgical repair of pectus excavatum: a metaanalysis.

BACKGROUND: Despite numerous published reports, there is no consensus in the literature as to whether the surgical repair of the pectus excavatum improves cardiovascular function. As a result, it has been suggested that correction should be considered a cosmetic procedure, and therefore, many health insurance companies have questioned whether the repair of the pectus excavatum improves cardiovascular function and thus are reluctant to authorize the procedure. The purpose of this study was to apply metaanalysis methodology to generate a quantitative synthesis of the effects of surgical repair o

Surgeon involvement in the care of patients deemed to have "preventable" conditions.

BACKGROUND: As of October 1, 2008, the Centers for Medicare and Medicaid Services stopped payment for 8 "preventable" conditions: retained foreign body, air embolism, blood incompatibility, catheter-associated urinary tract infection, pressure ulcer, vascular catheter-associated infection, mediastinitis, and hospital fall. Our hypothesis was that surgeons are frequently involved in the care of patients deemed to have "preventable" conditions. STUDY DESIGN: This is a retrospective cohort of patients with "preventable" conditions identified from 2004 California and New York discharge data. The

Airway Endoscopy Findings in Children with Recurrent Croup.

INTRODUCTION: Microlaryngoscopy and bronchoscopy (MLB) is frequently performed to identify subglottic pathology in recurrent croup. However, the reported incidence of positive findings that could alter management or require intervention varies widely. We sought to determine the utility of endoscopic airway evaluation in this setting. METHODS: All MLBs performed by three pediatric otolaryngologists at a tertiary care academic children's hospital from January 1, 2019, to December 31, 2023, were retrospectively reviewed. RESULTS: Sixty-two MLBs were performed for recurrent croup. Forty-four (71

Comparative Analysis of Cervical Disc Arthroplasty and Anterior Cervical Discectomy and Fusion: Trends, Demographics, and Clinical Outcomes in a Nationwide Inpatient Sample.

Introduction: Cervical disc disease is a common cause of disability worldwide. Two surgical options for refractory CDD are anterior cervical discectomy and fusion (ACDF) and cervical disc arthroplasty (CDA). While ACDF is well established, CDA offers motion preservation and has shown promising outcomes. This study compared utilization trends, patient characteristics, and hospitalization outcomes of ACDF and CDA using a large national dataset. Methods: We analyzed patients in the Nationwide Inpatient Sample (2016-2019) undergoing ACDF or CDA, identified using ICD-10 codes. After exclusions, 97,
