Use of the enteroscope for colo-ileoscopy: low yield in unexplained lower gastrointestinal bleeding.

BACKGROUND AND STUDY AIMS: The small intestine is a potential origin of bleeding in patients with unexplained gastrointestinal tract hemorrhage or iron-deficiency anemia. Most reports on the investigation of these patients describe the use of upper tract enteroscopy. The diagnostic yield of combined upper and lower enteroscopy has not been widely assessed and remains to be clarified. The aim of this study was to assess the benefit of lower gastrointestinal tract enteroscopy in occult digestive bleeding. PATIENTS AND METHODS: Between 1 December 1995 and 15 January 1998, 54 patients with gastro

Gastrointestinal endoscopy in Nigeria--a prospective two year audit.

INTRODUCTION: Gastrointestinal (GI) endoscopy is currently performed by different specialties. Information on GI endoscopy resources in Nigeria is limited. Training, cost, availability and maintenance of equipment are some unique challenges. Despite these challenges, the quality and completion rates are important. METHODS: Prospective audit of endoscopic procedures by an endoscopist in a Nigerian hospital over a 24 month period. RESULTS: One hundred and ninety endoscopic procedures were performed in 187 patients (109 male, 78 female) by a surgeon during this period. Mean age was 47.6 years (

To characterize the incidence of airway misplacement of nasogastric tubes in anesthetized intubated patients by using a manometer technique.

This study characterized the incidence of airway misplacement of nasogastric (NG) tubes in surgical patients, and the benefit of using a manometer to discriminate gastric placement from airway placement of NG tubes. Subjects included adult patients scheduled for abdominal surgery. After tracheal intubation, a 16 Fr. NG tube was inserted blindly through the nostril, and its position was assessed using the auscultation (10-ml air insufflation) or manometer (attached to NG tubes) techniques. Briefly, a biphasic pressure change synchronous with airway pressure during mechanical ventilation indicat

A Potentially Lethal Course of a Nasogastric Tube: Case Report.

Nasogastric tube (NGT) insertion is a common procedure, often performed blindly. We report a rare, near-fatal complication in a 44-year-old woman where an NGT traversed the internal jugular vein, superior vena cava, and perforated the right atrium into the pericardial cavity. Prompt imaging and surgical retrieval via sternotomy prevented tamponade or exsanguination. No esophageal injury was found; the likely entry point was the nasopharynx. A sealed duodenal perforation was managed conservatively. This case emphasizes the importance of verifying NGT placement before use and the role of confirm

Blind nasoenteric tube placement risks: A case report of misplacement into the pleural cavity.

RATIONALE: Enteral nutrition is a critical component of care for critically ill patients. However, the blind insertion of a nasoenteric tube, despite being a simple procedure, carries inherent risks that necessitate a reevaluation of the technique. PATIENT CONCERNS: A case of a 60-year-old female experienced the rare yet critical complication of a misplaced nasoenteric tube entering the thoracic cavity during a blind insertion procedure for enteral nutrition following a liver transplant. DIAGNOSIS: Following liver transplantation, the patient was diagnosed with severe pneumonia, a right-side

How High-Flow Nasal Cannula Is Impacted by Nasogastric/Esophageal Tube: A Bench Study.

High-flow nasal cannula (HFNC) is a common noninvasive respiratory therapy for respiratory failure, offering positive airway pressure and dead space clearance. In critically ill patients, additional nasal tubes for feeding or monitoring are often required, but their effect on HFNC performance is not well understood. This bench study evaluated the impact of nasal tube placement on dead space clearance, nasopharyngeal pressure, and airway resistance using standard and asymmetrical cannulas. Inserting a 5-Fr tube with a standard cannula had very little effect, whereas bigger sizes (12-Fr and 16-F

Effectiveness of left nasal nasogastric tube placement with right head-tilt in anesthetized patients: a randomized controlled trial.

BACKGROUND: Patients undergoing general anesthesia frequently encounter difficulties when the conventional nasogastric tube (NGT) is placed, resulting in low success rates. This study investigates a novel, free-hand, and blind technique aimed at enhancing the first-attempt success rate without the need for any medium. METHODS: One hundred and thirty-two eligible patients were randomly allocated to either the experimental group or the control group. In the experimental group, patients' heads were turned 45-90° to the right while in the supine position, and the NGT was inserted vertically throu

Optimizing Endoscopic Transpyloric Feeding Tube Placement in Low Birth Weight Infants: Practical Insights from Clinical Experience.

Background and Objectives: Transpyloric (TP) feeding tube placement is a viable nutritional strategy in low birth weight infants (LBWIs) with severe gastroesophageal reflux or feeding intolerance. However, technical challenges are encountered in patients of this age group due to their small body size and the limited availability of appropriately sized equipment. Materials and Methods: We retrospectively reviewed 15 endoscopic TP tube placements performed in 12 LBWIs weighing less than 2.5 kg between May 2017 and March 2025. Emphasis was placed on procedural techniques, equipment selection, and

Gastrojejunal Fistula Following Gastrostomy Tube.

Gastrostomy tubes are often placed in patients with poor voluntary intake, oropharyngeal dysphagia, or chronic illness to provide definitive nutritional access. Despite the widespread use of gastrostomy tubes, some patients can experience complications associated with this procedure including gastrocolic-cutaneous fistula and dislodgement of gastrostomy tube. This case discusses an instance of gastrojejunal fistula formation over one year after gastrostomy tube placement likely due to tube dislodgement. Imaging showed gastrostomy tube traversing the posterior wall of the stomach and creating a

Nasoenteric tube doppler guided insertion. A case report and review of literature.

Nasoenteric tube placement is needed in patients with gastric enteral feeding intolerance or high risk of aspiration. Several techniques are described. Ultrasound is used more commonly for gastric tube placement, but also is described in enteral tube feeding placement. Doppler technique with cold water injection is not described in the literature. We describe a new and easy to realize technique. A case of a 65 years old male patient in the intensive care unit (ICU) with gastroparesis is described. Before beginning a prokinetic drug was administered. In this technique gastric tube placement was

Nasogastric Intubation as Health and Safety Risk in Equine Practice-A Questionnaire.

Placing a nasogastric tube can be a life-saving act for a horse but is considered an occupational hazard for veterinarians. An online questionnaire was performed to assess and specify potential risks. 123 equine veterinarians completed the survey, and the majority admitted using the mouth to handle the end of the nasogastric tube (sucking or blowing air) and having accidentally swallowed or aspirated stomach content or medications. This can potentially lead to aspiration pneumonia or pneumonitis. Mineral oil seems to be especially dangerous as aspiration may be asymptomatic at the beginning an

Feeding tubes in children.

PURPOSE OF REVIEW: A practical guide to different feeding tubes available for nutritional support in children, focused on indications, placement methods and complications. RECENT FINDINGS: Enteral nutritional support refers to the delivery of nutrition into the gastrointestinal tract distal to the oesophagus. Different feeding tubes are available for exclusive or supplemental nutritional support in children who are unable to independently sustain their own growth, nutritional and hydration status. Gastric feeding is the first choice; however, jejunal feeding provides a good alternative route
