Preinvasive carcinoma of the cornea and conjunctiva.

Greyish-white spots, varying in size, caused diagnostic problems in 2 patients with pre-invasive corneal carcinoma. Keratectomy specimens permitting light and electron microscopy indicated that the smaller spots predominating in one patient correlated with epithelial microcysts and vacuoles, while areas of parakeratosis accounted for the macroscopic white patches found in the other. A variety of ultrastructural abnormalities, including excessive basement membrane fibrillogenesis, were present in both cases.

[Cross-linking and neurodermitis: prolonged re-epithelisation with severe corneal vascularisation after cross-linking in a patient with neurodermitis and keratoconus].

This case report describes a patient with keratoconus and neurodermitis suffering from a significantly prolonged postoperative time interval to re-epithelisation after corneal cross-linking. The development of corneal calcifications and vascularisations additionally inhibited proper re-epithelisation. Therefore the patient received four subsequent subconjunctival injections of Bevacizumab and an additional keratectomy to remove the calcifications. This therapeutic scheme led to a significant reduction of corneal vascularisation and finally a full rehabilitation of the epithelium.

Development of EKC after eximer laser photorefractive surgery and subsequent recurrence of EKC-like keratitis.

This research focuses on four cases of patients having undergone eximer laser photorefractive surgery who were diagnosed with adenoviral keratoconjunctivitis during the postoperative period and who later developed epidemic keratoconjunctivitis (EKC)-like keratitis. Two of the patients had undergone laser-assisted subepithelial keratectomy (LASEK), one had undergone laser in situ keratomileusis and one had photorefractive keratectomy. After the surgery adenoviral keratoconjunctivitis and recurrent late-developing EKC-like keratitis were observed in the patients. Recurrent late-developing EKC-li…

Interpreting corneal changes after PRK: Integrating biomechanics with tomographic remodeling.

Photorefractive keratectomy (PRK) alters corneal biomechanical and tomographic measurements, but postoperative change, an abnormal index classification, and progressive ectasia are distinct. Saif et al. reported biomechanical changes at 6 months; a separate Pentacam cohort showed tomographic changes at 1 month. Clinically and topographically normal eyes assessed approximately 25 years after PRK also had frequent abnormal classifications. These different cohorts and observation periods cannot establish a common postoperative trajectory or show that an abnormal result is benign in an individual …

Refractive approaches to visual rehabilitation in patients with keratoconus.

PURPOSE OF REVIEW: As keratoconus is a chronic disease affecting young people, vision-related quality of life is often significantly impaire … uccessfully fitted with scleral lenses. Although laser ablative procedures, such as photorefractive keratectomy (PRK) have been traditionally contraindicated in patients diagnosed with or suspected of having keratoconus, PRK has been attempted to partially correct refractive errors in keratoconus. Although phototherapeutic keratectomy and radial keratotomy ha …

Refractive Surgery for Special-Needs Children with High Myopia.

Refractive surgery is useful in children with high isoametropic or anisometropic myopia who are aversive to spectacles or contact lenses. The majority have a neurodevelopmental disorder and the equivalent of legal blindness before surgery. Correction of myopia < 6 Diopters (D) is achieved by photorefractive keratectomy (PRK). Those with myopia > 6 D benefit from phakic IOL implantation, clear lens extraction (CLE), or refractive lens exchange (RLE). The rate of adverse events is low.

Unusual causes of giant papillary conjunctivitis.

We report two unusual causes of giant papillary conjunctivitis (GPC). One patient developed GPC secondary to and elevated filtering bleb. Another patient developed focal GPC secondary to elevated calcific plaques (band keratopathy). The first patient's symptoms improved after treatment with sodium cromolyn drops. GPC resolved in the second patient after superficial keratectomy was performed. These cases represent two unusual causes of GPC and emphasize the role of mechanical trauma as a causative factor.

Nodular Keratitis Associated With a Previous Diagnosis of Chronic Superficial Keratitis in a German Shepherd Dog: A First Case Report.

OBJECTIVE: To describe the clinical and morphological features of a case of superficial nodular keratitis in a dog. CASE DESCRIPTION: A German shepherd dog previously diagnosed with chronic superficial keratitis (CSK) presented with a nonpainful, unilateral corneal nodule. After a complete ophthalmic examination, topical treatment with dexamethasone (0.1% dexamethason, WZW) was prescribed. However, the patient did not respond to this treatment; thus, a superficial keratectomy was performed. The lesion was examined histologically and immunohistochemically. RESULTS: The ophthalmic examination …

Corneal papilloma associated with papillomavirus in a one-humped camel (Camelus dromedarius).

A 15-year-old male dromedary camel with a history of chronic severe keratoconjunctivitis and corneal mass in the left eye of 6 months' duration was referred to the Veterinary Medical Teaching Hospital at Adnan Menderes University. A superficial keratectomy was performed and biopsy material submitted for histopathology. The diagnosis was corneal papilloma. There has been no recurrence of the neoplasm to date (6 months, 1 year). Corneal papilloma has not been reported previously in camels and seems to be associated with papillomavirus.

Paradoxical response to photorefractive treatment for postkeratoplasty astigmatism.

A 42-year-old woman with functionally disabling postkeratoplasty astigmatism in her right eye 3 years after corneal transplantation for keratoconus received photorefractive keratectomy for high astigmatism in that eye. This did not reduce her astigmatism. The procedure was repeated and corneal astigmatism paradoxically increased. The forces within the pseudo-optical ring that cause and maintain astigmatism in corneal transplants may be a significant factor in the unpredictable response of some corneal transplants to excimer photorefractive astigmatism correction.

Refractive error in cataract surgery after previous refractive surgery.

Bilateral cataract extraction with posterior chamber intraocular lens (IOL) implantation was performed in a patient after previous photorefractive keratectomy, radial keratotomy (RK) combined with astigmatic keratotomy, and retreatment of RK. Significant hyperopic error was observed after cataract surgery, and the IOLs were eventually exchanged in both eyes. A review of this case found that the refractive error was smaller when a refraction-derived keratometric value was selected for IOL power calculation. Nevertheless, hyperopic error still occurred.

[Squamous cell carcinoma of the "pure" cornea].

Squamous cell carcinoma of the cornea without involvement of the conjunctiva and limbus is a very rare condition. We report on a 68-year-old female patient. Lamellar keratectomy was performed combined with brachytherapy with strontium-90 for a total dose of 120 Gy (single daily dose: 10 Gy). Two months after treatment, a regression of neovascularization was noted. During 3.5 years of follow-up, no rubeosis of the iris, secondary glaucoma, or evidence of tumor recurrence was seen. There were no regional or distant metastases.

Cross-Linked Carboxymethyl Cellulose and Silk Proteins in Corneal Re-Epithelialization: A Case Series.

Background/Objectives: Corneal re-epithelialization is a critical process following surgical procedures such as photorefractive keratectomy (PRK), phototherapeutic keratectomy (PTK), and corneal UV cross-linking (CXL), as well as cases of corneal abrasion. Delayed epithelial healing can lead to increased discomfort, a higher risk of infection, and suboptimal visual outcomes. This retrospective case series aims to evaluate the efficacy of a novel ophthalmic solution containing cross-linked carboxymethyl cellulose (CX-CMC) and silk proteins in promoting corneal re-epithelialization and improving…
