Heart-to-kidney signal by extracellular vesicles promotes kidney injury in cardiorenal syndrome.

AIMS: Cardiorenal syndrome type-1 (CRS1), defined as acute kidney injury occurring in the setting of acute cardiac illness, is associated with substantial morbidity and mortality. Our objective was to determine whether extracellular vesicles (EVs) derived from the injured heart localize to the kidneys and contribute to renal injury in CRS1, and to elucidate mechanisms by which cardiac-derived EVs mediate kidney damage. METHODS AND RESULTS: Using cardiomyocyte-specific EV reporter mice, we characterized the release of cardiac-derived EVs and their biodistribution to the kidneys. Acute myocardi…

Dapagliflozin preserves kidney function and reduces albuminuria in children with chronic kidney disease.

BACKGROUND: Sodium-glucose co-transporter 2 (SGLT2) inhibitors reduce CKD progression in adults; but pediatric evidence remains limited. We … s adverse events occurred. CONCLUSION: Dapagliflozin was associated with short-term improvement in kidney function trajectory and reduction in albuminuria in children with CKD, with an acceptable safety profile.

Kidney replacement therapy trajectories of patients with kidney failure: an ERA Registry study.

BACKGROUND: Trajectories of patients receiving kidney replacement therapy (KRT), including transitions between hemodialysis (HD), peritoneal dialysis (PD) and kidney transplantation (KTx), may vary across patient subgroups and have not yet been investigated in Europe. This study aimed to: (i) describe the number of shifts across KRT modalities; and (ii) characterize the most frequent patient trajectories, including the direction of shifts across KRT modalities, the duration spent on each modality, and patient events, including death. METHODS: Data of adult patients (≥20 years) who initiated K…

Contrast-Associated Acute Kidney Injury: Balancing Kidney Risk and Access to Essential Imaging.

Iodinated contrast media are central to modern medicine, but have historically been associated with contrast-induced nephropathy (CIN), prompting widespread caution when imaging patients with kidney disease. This has led to delays, precautionary measures and avoidance of clinically indicated imaging. Contemporary evidence suggests that the risk of clinically meaningful kidney injury attributable to modern contrast agents has been overstated. We conducted a structured narrative review of the literature examining iodinated contrast media and kidney injury. Searches of PubMed/MEDLINE, Embase, Sco…

Implications of Acute Kidney Injury Requiring Dialysis Versus End-Stage Kidney Disease Status.

More than 20% of patients receiving dialysis at in-center hemodialysis facilities initiate dialysis for acute kidney injury requiring dialysis (AKI-D) rather than end-stage kidney disease (ESKD). The transition from AKI-D to ESKD status is important to reorient clinical efforts from short-term monitoring for recovery to equally important long-term goals of establishing permanent dialysis access and planning for kidney transplant. The timing of this transition is highly variable and determined by the treating nephrologist. Because of numerous subtle policy differences, the implications of trans…

Horseshoe kidney and polycystic horseshoe kidney: a forensic autopsy-based study in Portugal.

Horseshoe kidney (HSK) is the most common congenital renal fusion anomaly, while autosomal dominant polycystic kidney disease (ADPKD) is the most prevalent inherited renal disorder. Their coexistence-polycystic horseshoe kidney (P-HSK)-is exceedingly rare. This retrospective forensic study assessed the prevalence of HSK and P-HSK in a consecutive autopsy series from the three regional delegations of the Portuguese National Institute of Legal Medicine and Forensic Sciences between 2017 and 2022. A total of 31,325 autopsy reports (22,854 males and 8,471 females) were reviewed. HSK was identified…

Critical care considerations in rhabdomyolysis-associated acute kidney injury and kidney replacement therapy.

Rhabdomyolysis is a frequently seen and potentially severe condition in intensive care, resulting from causes like trauma, infection, drug toxicity, or prolonged immobility. When muscle cells break down, substances such as myoglobin enter the bloodstream, increasing the risk of acute kidney injury (AKI), which greatly affects patient outcomes. Prompt diagnosis, effective fluid management, and supportive care are crucial for reducing kidney-related complications. In more critical situations, standard treatments may not be enough, and kidney replacement therapy (KRT) may be required. This review…

Associations between CT radiomics analyses and kidney biopsy in patients with kidney disease.

BACKGROUND: Kidney disease is characterized by microstructural alterations that currently require invasive biopsy for definitive assessment. However, it remains unclear to what extent radiomics features extracted from contrast-enhanced CT can non-invasively reflect kidney function and histopathological changes. METHODS: Between October 2020 and May 2025 all patients undergoing kidney biopsies and having CT scans prior to biopsy were retrospectively analyzed. A total of 49 patients (59% female, median age 60 years) were included. Of the included patients, 35 biopsies were performed in native k…

Predictive Value of Urine Tubular Biomarkers on Kidney Outcomes: Observations from EMPA-KIDNEY.

BACKGROUND: Urine tubular biomarkers have previously been associated with chronic kidney disease (CKD) progression independently of estimated glomerular filtration rate (eGFR) and albuminuria. We evaluated whether nine urine tubular biomarkers could improve prediction of kidney failure beyond serum creatinine and urine albumin-to-creatinine ratio among adults with CKD. METHODS: Among 5100 participants in EMPA-KIDNEY, a randomized trial which assessed the effects of empagliflozin 10 mg versus matching placebo among patients with CKD at risk of progression, nine urine creatinine-indexed tubular…

Liver and kidney cross talk: liver-derived bile acids and kidney water retention.

Edema formation and ascites are common features in liver disease. The excessive accumulation of body water is caused by increased kidney water retention; yet the mechanisms linking liver and kidney function remain incompletely understood. This review explores the emerging concept of a liver-kidney axis, with bile acids (BAs) as potential mediators for kidney water handling. In cholestatic liver disease, as seen in liver cirrhosis, impaired hepatic BA flow to the duodenum elevates systemic BA concentrations, which can modulate signaling pathways through the BA receptors Farnesoid X receptor (FX…
