Cementless revision total knee arthroplasty.

This is a report of the outcome of cementless revision total knee arthroplasty (TKA), and describes the technique for bone grafting of massive femoral and tibial defects. The technique relies on rigid fixation of the implants using the remaining shell of distal femur and proximal tibia and the diaphysis of each bone for support of the implant. Fifty-six cementless revision TKAs in 56 patients were performed using cementless technique, long-stemmed components, and morselized allograft between April 1985 and April 1989. All knees had major loss of femoral and tibial bone stock. Fixation of the s…

An alternative bent-knee prosthesis.

Prosthetic fitting in patients with below-knee amputations and concurrent knee flexion contractures poses inherent difficulties to the prosthetic rehabilitation team. The standard bent-knee prosthesis is bulky and awkward. It treats the patient functionally as a knee disarticulation and yields no potential for improving the patient's degree of contracture. This paper describes a case report with an alternative to the bent-knee prosthesis that not only offered improved function relative to the standard bent-knee prosthesis, but also acted therapeutically by reducing the patient's knee-flexion c…

Total knee arthroplasty in osteonecrosis.

Thirty-eight total knee arthroplasties in 36 patients were studied at 4 +/- 2 (mean, +/- SD) years follow-up evaluation. The Hospital for Special Surgery knee score improved from 59 +/- 9 (mean, +/- SD) before surgery to 83 +/- 11 (mean, +/- SD) (p less than 0.0001) at last evaluation. Good or excellent results were achieved in 87%. Implant survivorship at five years was predicted to be 85% using an endpoint of revision, or 68% using an end point of revision for moderate or severe pain. There was one failure for deep infection but no clinical or roentgenographic evidence of loosening.

Jumper's knee. Diagnosis and treatment.

"Jumper's knee" or patellar tendinitis was first described in an article by Blazina in 1973. He noted an increasing incidence of peripatellar tendinitis, affecting either the quadriceps tendon or the patellar tendon insertion onto the patella, most commonly in jumping athletes. Since that time, numerous articles have further defined the etiology of this condition and the actual pathologic lesion, along with classifying the stages of its development and outlining appropriate methods of treatment and prevention. Although jumper's knee is quite common, its treatment remains somewhat controversial…

Anterior Knee Pain (Chondromalacia Patellae).

In brief: Anterior knee pain (chondromalacia patellae) may be the most troublesome, ubiquitous overuse syndrome in the sports world. Complaints of insidious onset of vague, activity-related pain coupled with evidence of wasting of the vastus medialis muscle usually establish the diagnosis. The two major goals of treatment-alleviating the symptoms and strengthening the vastus medialis-can often be attained with minimal intrusion into the patient's athletic endeavors. Treatment may include nonsteroidal anti-inflammatories, icing, and physical therapy modalities (eg, high-intensity galvanic stimu…

Cytological assessment of knee effusions.

Effusion fluid from 80 knee joints was obtained from patients prior to arthroscopy and arthroscopic surgery and submitted to independent physicochemical analysis and cytological examination. The majority of the effusions were secondary to osteoarthrosis and traumatic mechanical derangement. These two conditions gave nonspecific findings on physicochemical analysis and cytology, and the cytologist diagnosed on 13% of these correctly. Cytological examination of fluid from rheumatoid knees revealed ragocytes in just over half the cases, and on this basis, the cytologist was able to correctly diag…

Knee rehabilitation following arthroscopic meniscectomy.

Guidelines for knee rehabilitation after conventional open meniscectomy are generally well known. Less agreement exists on rehabilitation following arthroscopic meniscectomy. The program is sometimes arbitrary. Rehabilitation should follow a logical progression regardless of the type of surgery performed. Rehabilitation following knee surgery can be divided into four phases: immediate post-operative period, early healing phase, late healing phase, and conditioning for return to preinjury activity. Following arthroscopic surgery, the overall recovery time can be significantly shortened compared…

Locked knee and osteochondritis dissecans.

Two patients presenting with a locked knee are described. In both cases a loose body from an osteochondritis dissecans defect had wedged beneath the anterior horn of the lateral meniscus, creating a block to extension which could not be reduced. Arthroscopy and arthroscopic methods proved unsuccessful in the removal of the loose body.

[Results with Guepar knee endoprostheses].

From 1977 to 1982 40 patients suffering from severe bilateral gonarthrosis, mostly caused by rheumatoid arthritis, underwent the implantation of 42 Guepar-endoprostheses of their knee joint. It was only in 1 case that a prosthesis got loose by infection and had to be removed without replacement. In 2 other cases of aseptic loosening a reimplantation of the prosthesis could be performed successfully. The authors emphasize the encouraging results.

Failed polycentric total knee prostheses.

Four failed polycentric total knee prostheses were evaluated by clinical examination of the patients and surface analysis of the removed prosthetic components. The failures were secondary to a poor relationship of the prosthetic components in three cases and to abutment of the tibial spine against the femoral condyle in the fourth. A poor relationship of the prosthetic components was not necessarily secondary to malpositioning, because the positioning may have been accurate but the relationships became abnormal when the knee was unstable. Scanning electron-microscope examination revealed vario…

Congenital deformities about the knee.

Congenital deformities about the knee are the result of abnormalities of all the anatomical structures that make up this joint. In some cases the structural changes observed are due to primary disorders of formation and growth of one or more of the anatomical components of the knee. Disorders are known to occur in the integumentary, muscular, ligamentous supporting, and skeletal systems. Examples of these disorders are seen in the bone dysplasias and joint laxity syndromes. In other cases exaggerated intrauterine positions of the limb seem to bring about certain adaptive changes in the embryol…

Lymphatic filling during knee arthrography.

Six cases of lymphatic filling during knee arthrography have been recorded in literature, all of which have had advanced rheumatoid arthritis. We report a case of lymph vessel filling from a popliteal cyst in a young male nonrheumatoid patient. It is concluded that abnormal synovial-lymphatic connection in the knee joint may occur also in other forms of synovial pathology and such a finding does not indicate rheumatoid inflammation.
