Influence of the trunk position on knee kinematics during the single-leg landing: implications for injury prevention.

The purpose of the present study was to investigate the influence of the trunk position during the single-leg landing on the knee angle and muscle activity. Forty healthy university students (20 men and 20 women) performed right single-leg landings from a 40 cm-high platform with the trunk in neutral, flexion, extension, and right and left lateral flexion. Knee flexion and valgus angles were determined by two-dimensional video analysis, and rectus femoris (RF) and biceps femoris (BF) muscle activities were assessed. The knee flexion angle was significantly higher in the trunk-flexion position

Investigation of knee control as a lower extremity injury risk factor: A prospective study in youth football.

This prospective study in youth football examined the relationship between frontal plane knee projection angle (FPKPA) during the single-leg squat and sustaining an acute lower extremity injury or acute non-contact lower extremity injury. Secondly, side-to-side asymmetry in FPKPA and sex as injury risk factors were explored. In addition, we investigated the influence of age, sex, and leg dominance on the FPKPA. A total of 558 youth football players (U11 to U14) participated in the single-leg squat test and prospective injury registration. FPKPA was not found as a risk factor for injuries at th

Editorial Commentary: Knee Lateral Collateral Ligament Injury Is More Common Than We Thought.

Detection of a knee lateral collateral ligament (LCL) tear, in combination with an anterior cruciate ligament tear, may be facilitated by the heel height test, which seems a modification of Hughston's external rotation recurvatum test for posterolateral rotatory instability. The radiographic and arthroscopic stress tests are also valuable. We have found that varus stress will increase varus opening by 1° to 4° for isolated LCL tears and 5° to 9° for combined (LCL) popliteus injury; an arthroscopic stress evaluation will note a normal opening of 5 to 6 mm increasing to 9 to 10 mm in combined LC

An evidence-based algorithm for the management of common peroneal nerve injury associated with traumatic knee dislocation.

Traumatic knee dislocation is a complex ligamentous injury that may be associated with simultaneous vascular and neurological injury.Although orthopaedic surgeons may consider CPN exploration at the time of ligament reconstruction, there is no standardised approach to the management of this complex and debilitating complication. This review focusses on published evidence of the outcomes of common peroneal nerve (CPN) injuries associated with knee dislocation, and proposes an algorithm for the management. Ipsilateral Hip and Knee Dislocation with Open Tibial Fracture: A Case Report of a Limb Threatening Injury.

INTRODUCTION: Hip or knee dislocations are usually treated as a surgical emergency, but ipsilateral hip and knee dislocation should be considered a dual emergency that must be addressed immediately and reduced at the earliest. We present here the sequence of events and the final functional outcome of one such rare injury manages by us. CASE REPORT: A 22-year-old male was involved in a road traffic accident. He presented to the emergency department in 4 h injury time with painful deformities of the right hip and knee, along with Type I open wound right leg with abnormal mobility suggestive of

The complement system is activated in synovial fluid from subjects with knee injury and from patients with osteoarthritis.

BACKGROUND: The complement system is suggested to be involved in the pathogenesis of osteoarthritis (OA), and proinflammatory cytokines may play a role in OA development by inducing proteases. The association between complement factors, cytokines and OA has not been investigated. The aim of the present study was to explore the involvement of the complement system after knee trauma and in OA. METHODS: C4d, C3bBbP and soluble terminal complement complex (sTCC) resulting from complement activation were immunoassayed in synovial fluid from subjects with healthy knees (reference), OA, rheumatoid a

Single leg squat test and its relationship to dynamic knee valgus and injury risk screening.

BACKGROUND: Lower extremity injuries are common in athletes. Valid tests to assess for risk of injury that are easily performed during a preparticipation sports physical examination are lacking. Two-dimensional (2D) analysis of the drop-jump test can identify athletes at risk, but it is too expensive and cumbersome to use in this setting. OBJECTIVE: To identify if those who perform a "positive"(abnormal postures) single leg squat (SLS) test also exhibit greater "dynamic valgus" on the 2D drop-jump test. Our secondary purpose was to assess whether group differences in gender, age, or body mass

Anterior cruciate ligament injury and radiologic progression of knee osteoarthritis: a systematic review and meta-analysis.

BACKGROUND: Knee osteoarthritis after anterior cruciate ligament (ACL) injury has previously been reported. However, there has been no meta-analysis reporting the development and progression of osteoarthritis. PURPOSE: We present the first meta-analysis reporting on the development and progression of osteoarthritis after ACL injury at a minimum mean follow-up of 10 years, using a single and widely accepted radiologic classification, the Kellgren & Lawrence classification. STUDY DESIGN: Meta-analysis. METHOD: Articles were included for systematic review if they reported radiologic findings o

Ultrasound-guided continuous superficial peroneal nerve block below the knee for the treatment of nerve injury.

(CRPS) describes a constellation of symptoms including pain, trophic changes, hyperesthesia, allodynia, and dysregulation of local blood flow often following trauma. It is often confined to the extremities. Treatment of this disorder consists of a variety of modalities including systemic pharmacotherapy, local anesthetic injections or infusions, psychological nonpharmacotherapy, physical rehabilitation, and surgical intervention. Chronic pain not related to CRPS can also be treated with similar interventions. Despite the array of available therapies, it can still be difficult to manage. We rep

Coordinated expression of MMPs and TIMPs in rat knee intra-articular tissues after ACL injury.

Anterior cruciate ligament (ACL) has poor healing ability and an injured ACL would induce the degeneration of other intra-articular connective tissues. However, the coordinated expression and activities of matrix metalloproteinase (MMPs) in intra-articular tissues induced by ACL rupture were poorly understood. With a rat ACL rotating injury model, we found that after ACL injury, the mRNA levels of MMP-13, TIMP-1, and CD147 were significantly elevated in ACL, posterior cruciate ligament (PCL), synovium, meniscus, and cartilage. Also, MMP-2 activity was also elevated significantly in a time-depe

Case report: subacute synovitis of the knee after a rose thorn injury: unusual clinical picture.

Synovitis secondary to penetrating plant thorn injuries is not frequently reported. Historically, it is considered aseptic and treated with removal of the intraarticular foreign body and affected synovial lining. We report a 57-year-old healthy man who was admitted 2 weeks after being injured by a rose (Rosacea) thorn with subacute and mild synovitis with effusion of his right knee. No intraarticular foreign body was retained. Pantoea agglomerans was identified in the synovial fluid. Contrary to former teaching, effusions from joints violated by thorns should not be presumed sterile. Bacterial

The effect of leg fracture level and vehicle front-end geometry on pedestrian knee injury and response.

In this study a MADYMO (mathematical dynamic modelling) model has been used to identify the influence of leg fracture on the injuries sustained by the pedestrian during front end impact with a vehicle. A factorial study of a MADYMO pedestrian and vehicle model are used to investigate the effect of different leg fracture tolerances, geometry, and vehicle compliance on the criteria measured in the European Enhanced Vehicle-safety Committee (EEVC) pedestrian safety tests. These criteria include knee bending, knee shear response, and lower leg bone (tibia) acceleration. The main study examines the

Differences in lower extremity alignment between males and females. Potential predisposing factors for knee injury.

AIM: The goal of this study was to compare measures of lower extremity alignment between males and females, which may account for gender differences in anterior cruciate ligament injury rates. Static lower extremity alignment has been implicated as predisposing individuals to anterior cruciate ligament injury and may vary between males and females. An initial step in identifying relevant risk factors for injury is to determine those factors that vary between genders. METHODS: Thirty male and 27 female college aged individuals with no history of lower extremity injury participated. Three indic

Flexor reflex responses triggered by imposed knee extension in chronic human spinal cord injury.

Hypersensitivity of the flexor reflex pathways to input from force-sensitive muscle afferents may contribute to the prevalence and severity of muscle spasms in patients with spinal cord injury (SCI). In this study, we triggered flexor reflexes with constant velocity knee movements in 15 subjects with SCI. Ramp and hold knee extension perturbations were imposed on one leg while the hip and ankle were held in an isometric position using an instrumented leg brace. Knee, ankle and hip torque responses and electromyograms from six muscles of the leg were recorded following controlled knee extension

The value of the ankle-brachial index for diagnosing arterial injury after knee dislocation: a prospective study.

BACKGROUND: The risk of arterial injury with knee dislocation is well known. The most effective method for rapidly and accurately diagnosing arterial injury in this setting remains a topic of debate. Both physical examination and arteriography have been advocated, although each of these methods has its critics. The authors propose that the ankle-brachial index (ABI) can accurately predict whether patients with knee dislocations have sustained vascular injury. METHODS: A prospective study enrolled 38 patients with knee dislocation to evaluate for potential arterial injury using clinical pulse
