Leiomyosarcoma is a relatively uncommon soft tissue sarcoma usually presenting as a mass at the primary site. It arises in the retroperitoneal region, in the cutaneous or subcutaneous tissue of the extremities or, less commonly, from a large vein. A case is described where a patient presented with unilateral third, fourth, and sixth cranial, and ophthalmic division of trigeminal nerve palsies in whom biopsy of a longstanding lower limb mass demonstrated leiomyosarcoma. The presentation of this type of pathology has not been described previously.

Histochemical and ultrastructural features of a gastric myxoid leiomyosarcoma from a 55-year-old man were examined. At autopsy, the tumor was located mainly in the greater omentum and was directly connected to a coinsized gastric tumor. Multiple hepatic metastases and peritoneal disseminations were noted. Light-microscopically, the tumor was composed of a prominent myxoid stroma and ovoid or rounded tumor cells. The myxoid stroma was stained weakly basophilic with hematoxylin and eosin, and was mainly composed of hyaluronic acid. Tumor cells in the stomach were spindle-shaped and apparently my…

Clinically, epithelial cancers (squamous cell carcinoma and adenocarcinomas) metastasize primarily by the lymphatic route, while mesenchymal sarcomas more frequently enter the blood stream directly. Nevertheless metastatic involvement of regional nodes has been seen in 13% of patients with soft tissue sarcomas and in 7% of bone sarcomas at initial presentation (29). About one-third of the leiomyosarcomas of the gastro-intestinal tract developed metastasis and about 90% of the metastasis were intra-abdominal (45). Liver is the most common site of metastasis, followed by peritoneal seeding and l…

A case of leiomyosarcoma of the breast that metastasized to the liver 15 years after mastectomy is reported. Leiomyosarcoma appears to have less aggressive biologic behavior than other types of breast sarcomas. Analysis of the reported cases of smooth muscle tumors of the breast revealed some guidelines for the histologic diagnosis of these tumors. Tumors with three or more mitoses per 10 HPF are leiomyosarcomas, and those with no mitotic activity, necrosis, and significant cellular atypia are leiomyomas. The borderline cases are best considered smooth muscle tumors with uncertain malignant po…

Leiomyosarcoma of the inferior vena cava is a rare and usually fatal disease. Only 45 cases have been reported in the world literature. The authors add one case and review all previous reports. The disease is one primarily of elderly women, arising from the middle third of the inferior vena cava with a wide range of symptomatology, varying form the Budd-Chiari syndrome to a few asymptomatic cases. The tumor may grow very slowly or occasionally very rapidly, shows extensive local invasion, and metastasizes more frequently than previously believed. While radiotherapy and chemotherapy have not be…

PURPOSE: To analyze the safety and efficacy of drug-eluting bead transarterial chemoembolization (DEB-TACE) treatment of unresectable metastatic hepatic leiomyosarcoma.
METHODS: Medical records and imaging of patients with metastatic leiomyosarcoma to the liver who underwent doxorubicin DEB-TACE from December 2014 to September 2024 were retrospectively reviewed to evaluate survival and clinical and biochemical toxicities.
RESULTS: The study included 29 patients (9 males, 20 females; average age 60.1 ± 12.7 years). Twenty-three patients (79.3%) had bilobar and 6 patients (20.7%) had unilobar …

This is a case report of primary hepatic leiomyosarcoma. An 82-year-old Japanese man presented with fever and mild abdominal pain. Laboratory testing demonstrated marked inflammation and substantial elevations of CA19-9 and PIVKA-II. Computed tomography revealed a large necrotic mass in the right hepatic lobe with gallbladder wall thickening. Gallbladder cancer invading the liver with abscess formation was suspected, but a definitive preoperative diagnosis could not be established. Right hepatectomy and cholecystectomy achieved an R0 resection. Histopathological examination showed fascicles of…

RATIONALE: Hepatic metastases from abdominal leiomyosarcoma are an extremely rare clinical condition. The multimodal imaging features of this condition have not been systematically described in the current literature, which may lead to delayed diagnosis or misdiagnosis and adversely affect patient prognosis. We herein report a case of hepatic metastases from abdominal leiomyosarcoma that occurred 5 years after surgical resection, and describe in detail its imaging features on ultrasonography, computed tomography (CT) and magnetic resonance imaging (MRI). Through literature review, we aim to en…

Uterine leiomyosarcoma (uLMS) is the most common subtype of uterine sarcomas. They have a poor prognosis with high rates of recurrence and metastasis. The five-year survival for uLMS patients is between 25 and 76%, with survival rates approaching 10-15% for patients with metastatic disease at the initial diagnosis. Accumulating evidence suggests that several biological pathways are involved in uLMS pathogenesis. Notably, drugs that block abnormal functions of these pathways remarkably improve survival in uLMS patients. However, due to chemotherapy resistance, there remains a need for novel dru…

Leiomyosarcomas of the lower extremity are heterogeneous tumors with high recurrence and metastasizing rates, which, rarely, can suffer a dedifferentiation process, making them extremely aggressive and unpredictable. We report the case of a 72-year-old female patient who presented the clinical and histopathological features of a rare leiomyosarcoma of the foot that was completely excised, but because of the interruption of adjuvant oncological treatment and lack of compliance for the follow-up, the patient suffered a dedifferentiation process and the tumor metastasized in the fingers-an unprec…

Leiomyosarcoma of the pulmonary artery is a rare but potentially fatal disease. Due to its rarity, the treatment algorithm is not well-established. While there may be a role for both chemotherapy and radiotherapy, surgical management is the most definitive method. Unfortunately, when the disease process is advanced, surgery may not be curative. However, it may still be a palliative treatment option. In this case report, we present a patient who suffered from respiratory symptoms that were initially attributed to pulmonary embolism (PE). However, upon the diagnosis of pulmonary artery leiomyosa…

BACKGROUND: Leiomyosarcoma is a highly malignant soft-tissue sarcoma with a poor prognosis. In recent years, treatment for leiomyosarcoma has not shown much progress. Primary intracranial leiomyosarcoma (PILMS) is a much rarer type of neoplasm, which occurs more frequently in immunocompromised patients. PILMS cases reported in the literature are scarce and treatment strategy and prognosis are still under debate. In this study, a case of PILMS secondary to the total resection of giant cell glioblastoma is reported.
CASE DESCRIPTION: A 38-year-old male was hospitalized with a three-month histor…

Leiomyosarcomas (LMSs) are extremely rare and comprise only 1.2% of small bowel malignancies. Advancements in immunohistochemical techniques have allowed for the differentiation between LMSs and gastrointestinal stromal tumors. LMSs remain difficult to detect via endoscopy and require a more intricate diagnostic approach. The staging and sizing of these tumors are important prognostic indicators. We report a case of a 67-year-old male who presented with bulging lower extremity veins, abdominal bloating, and weight loss. A CT of the abdomen and pelvis revealed a pelvic mass arising from the sma…

We present here a case of a 66-year-old lady who was diagnosed with right iliac fossa retroperitoneal leiomyosarcoma at Hospital Umum Sarawak. The challenge in this case was the extension of tumour with the involvement of her right ureter causing proximal hydroureter and hydronephrosis. After resection of tumour en-block with the involved segment of ureter, it was not possible to repair the ureteric defect directly. We used interpositional vascularized appendix graft to repair this large (7 cm) ureteric defect. We describe here this uncommon technique of ureter reconstruction.

Leiomyosarcoma arising from renal pelvis is a very rare disease. A patient was admitted to hospital with left renal colic due to nephrolithiasis, with a filling defect in renal pelvis that was considered to be a blood clot. Diagnosis of leiomyosarcoma was made after visual inspection and biopsy of the exophytic lesion. Laparoscopic radical nephrectomy was performed, histological and immunohistochemical investigation confirmed leiomyosarcoma with mixomatoid component. No adjuvant treatment was performed, the patient remains healthy 5 years after surgery without recurrence. Herein we provide lit…
